dc.description.abstractBu çalışmanın amacı; spinal anestezi uygulanacak elektif sezaryen ameliyatlarında hiperberik bupivakain ve fentanilin birlikte ya da ardışık verilmesinin spinal blok üzerine etkilerinin karşılaştırılmasıdır.Çalışma elektif sezaryen ameliyatı planlanan 90 hastada yapıldı. Hastalar randomize 3 gruba ayrıldı. Bütün hastalara boy ve kilosuna uygun dozda hiperbarik bupivakain % 0.5 ve 20 µg fentanil uygulandı. Grup M' de hiperbarik bupivakain % 0.5 ve fentanil birlikte, Grup MF' de önce hiperbarik bupivakain 30 sn (saniye) sonra fentanil, Grup FM' de önce fentanil 30 sn sonra hiperbarik bupivakain uygulandı.En üst duysal blok seviyesi Grup M' de T2, diğer gruplarda T5 olarak bulundu (p=0.001). Motor blok sonlanma zamanı Grup M' de 147±49, Grup MF' de 187±34 ve Grup FM' de 187±45 olup Grup M' de daha kısa bulundu (p=0.001). İki segment regresyon zamanı Grup M' de 57±13 dk, Grup MF' de 73±11 dk ve Grup FM' de 75±12 dk olup Grup M' de daha kısa bulundu (p=0.001).Sonuç olarak hiperberik bupivakain ve fentanil aynı anda uygulandığında duysal blok seviyesi daha yüksek olmakta ve motor blok daha önce sonlanmaktadır.ANAHTAR KELİMELER; spinal, lokal anestezik, bupivakain, fentanil, gebe, sezaryen
dc.description.abstractThe purpose of this study is to compare the effects of giving hyperbaric bupivacaine and fentanyl mixed or sequentially in elective cesarean operations performed under spinal anesthesia.The study was performed on 90 patients who were planned to have elective cesarean operation. Patients were randomly allocated in 3 groups. All patients were given appropriate doses of hyperbaric bupivacaine %0.5 and 20 µg fentanyl according to their height and weight. In group M hyperbaric bupivacaine 0.5% and fentanyl were given as mixture, in group MF first hyperbaric bupivacaine 0.5% and then after 30 seconds fentanyl was given, in group FM first fentanyl then after 30 seconds hyperbaric bupivacaine 0.5% was given.The peak sensorial block level was T2 in group M, and T5 in other groups (p=0.001). Motor block regression time was 147±49 minutes in group M, 187±34 minutes in group MF, 187±45 minutes in group FM and it was shorter in group M (p=0.001). Two segment regression time was 57±13 minutes in group M, 73±11 minutes in group MF and 75±12 minutes in group FM and it was shorter in group M (p=0.001).As conclusion when hyperbaric bupivacaine and fentanyl was given as mixture at the same time, sensorial block level was higher and motor block ended earlier.KEY WORDS; spinal, local anesthetic, bupivacaine, fentanyl, pregnant, cesareanen_US
