Erzurum bölgesinde miyadındaki gebelerin hemoglobin, serum ferritin değerlerinin bebeklerin hemoglobin, serum ferritin değerleri ile bebek ve plasenta ağırlıklarına etkisi

Abstract

IV ÖZET Gebelikte bazı besin maddelerine ve demire ihtiyaç artar. Bu ihtiyacın karşılanabilmesi için annenin yeterli ve dengeli beslenmesi sağlanmalı, özellikle gebeliğin ikinci yarısında demirden zengin gıdalar önerilmeli ve gerektiğinde demir desteği verilmelidir. Çalışmamız Erzurum bölgesinde miyadındaki gebelerde Hemoglobin, Serum Ferritini ve Serum Demir değerlerinin bebek Hemoglobin^ Serum Ferritini ve Serum Demir değerlerine, bebek ve plasenta ağırlığına etkisini araştırmak amacıyla planlanmıştır. Çalışmamızın evrenini, Erzurum İl merkezinde bulunan Numune Hastanesi, Sosyal Sigortalar Kurumu Hastanesi ve Üniversite Hastanesinde miyadında canlı doğum yapan 15-49 yaş arası anneler ve bebekleri oluşturmuştur. Kura yöntemi ile seçilen Sağlık Bakanlığına bağlı Numune Hastanesi Doğum ve Kadın Hastalıkları Kliniğinde miyadında, vaginal yolla, canlı doğum yapan toplam 121 anne çalışma kapsamına alınmış ve 26 soruluk bir anket formu uygulanmıştır. Doğumdan hemen önce gebelerden doğumdan hemen sonra da bebek kordonundan biyokimyasal ve hematolojik çalışmalar için 7'şer mi kan alınmıştır. İlgili laboratuvarlarda değerlendirildikten sonra anket formuna kaydedilmiştir. Çalışmamızdan elde edilen verilere göre deneklerin % 65.3'ünün 15-24 yaş grubunda yoğunlaştığı, % 53.7'sinin daha önce hiç doğum yapmadığı, % 62.8'inin okur-yazar veya ilkokul mezunu olduğu, % 98.3'ünün ev hanımı olduğu belirlenmiştir. Deneklerimizin % 40.3'ünün 12 aydan daha kısa aralıklarla gebe kaldığı, % 42.1 'inin son gebeliğinde hiç antenatal kontrole gitmediği, % 82.7'sinin demir preparatı kullanmadığı, % 73.6'sının beslenmesinde değişiklik yapmadığı saptanmıştır. Gebelerin % 12.4'ünde hemoglobin değerleri, % 85.1'inde Serum Ferritin değerleri normalin altında bulunmuştur. Bebeklerin tümünde hemoglobin değerleri normal sınırlarda, %10.7'sinde Serum Ferritin değerleri normalin altında bulunmuştur. Çalışmamızda ortalama bebek ağırlığı 3308 gr ve 2500-3500 gr ağırlığındaki bebek oranı % 67.7 olarak bulunmuştur. Ortalama plasenta ağırlığı 71 1 gr olupbebeklerin % 73.6'sında plasenta ağırlığı 601 gr'ın üzerinde olduğu belirlen miştir. Anne ve bebek hemoglobinleri arasında pozitif korelasyon saptanmasına rağmen diğer parametreler arasında anlamlı korelasyon saptanamamıştır. Anne Serum Ferritin değerlerinin bebek ağırlığını ters yönde etkilediği diğer kan parametrelerinin ise önemli derecede etkilemediği belirlenmiştir. Anne kan değerlerinin plasenta ağırlığına etkisi saptanamamıştır.

VI SUMMARY The need for some nutrients as well as iron increases during pregnancy In order to meet this need, the mother should have a sufficient and balanced nutrition; especially during the second half of the pregnancy, she should be provided with ferric food supplement whenever necessary. This study aims to find out the effect of hemoglobin, serum ferritin and serum iron levels of mothers on hemoglobin, serum ferritin and serum iron levels of babies as well as baby and placenta weights in due pregnancies in the Erzurum region. In our investigation we have concentrated on the mothers aged 15-49 and their babies born in two state hospitals and the University Teaching Hospital in Erzurum. 121 mothers who gave birth to babies in normal conditions, were randomly selected in the state Hospital's Gynecology Clinic, and a questionare consisting of 26 questions related to their conditions during the pregnancy period was also applied. 7 ml blood was taken from each of the pregnant women just before bearing their babies, and 7 ml from umbilical cord just after the birth for biochemical and hematologic tests. These were evaluated in laboratories and were recorded in questionaire forms. The study has revealed that 65.3 % of the subjects are between 15-24 years old, 53.7 % of them had given birth to no babies before hand, 62.8 % of them are literate or primary school graduates, and finally 98.3 % of them are housewives. It has been found out that 40.3 %of our subjects got conceived within a year of their last pregnancy and 42.1% of them never refered to hospital for antenatal control, 82.7 % of them used no iron preparation, 73.6 % of them didn't change their way of nutrition. Hemoglobin levejs were found to be below average in 12.4 % of the subjects and serum ferritin n. 85.1 % of them. Hemoglobin levels were in normal range in ail babies, but serum ferritin was below average in 10.7 % of them. Average weight of babies in our study was 3308 gr, and 67.7 % of them were between 2500-3500 gr. The averageVII weight of placenta was 71 1 gr, and in 73.6 % of the babies this weight was above 601 gr. Although a positive correlation between mother and baby hemoglobin levels was found, there was no positive correlation for other parameters. It was also found out that mother ferritin levels had a counter effect on weights of babies, but other blood parameters did not have a significant effect. Besides, blood values of the mothers were not effective on the weight of placenta.
