Tonsilla hipertrofisinin doğal baş,hyoid kemiği, mandibula ve dil konumu üzerine etkilerinin incelenmesi

Abstract

II ÖZET Bu araştırmanın amacı, temsilleri hipertrofık ve normal çocuklar arasında baş, hyoid kemiği, mandibula ve dil postüründe herhangi bir farklılık olup olmadığını incelemektir. Tonsilleri büyümüş 50 çocuk, normal tonsillaya sahip aynı sayıda çocuktan oluşan kontrol grubuyla karşılaştırıldı. Araştırma ve kontrol grupları eşit sayıda kız ve erkeklerden oluşturulmuştur. Çalışmamızda, 7-10 yaş aralığındaki bireylerin sefalometrik filmleri doğal baş pozisyonunda elde edilmiştir. Bireylerin seçiminde şu kriterler esas alınmıştır. - Herhangi bir ortodontik tedavi görmemiş olması, - Baş, yüz ve boyun bölgesinde yara, yanık veya skatris dokusu bulunmaması, - Görme ve işitme kaybı bulunmaması, - Kabul edilebilir yüz dengeli ve normal oklüzyonlu olması, - Herhangi bir büyüme bozukluğu olmaması. Yapılan metod hatası kontrollerinde sefalometrik çizim ve ölçümlerin biometrik olarak önemli düzeyde olmayan bir hata ile tekrarlanabildiği saptanmıştır. Tonsil ve cinsiyet faktörünün baş, hyoid kemiği, mandibula ve dil üzerine etkileri varyans analizi uygulanarak araştırılmıştır. Baş postürüyle ilgili ölçümlerden NSL/VER, NSL/CVT, NSL/OPT ve CVT/HOR; hyoid kemiğiyle ilgili ölçümlerden H-Gn, HJ.SN-S ve H-S; mandibular ölçümlerden N-Me, S~Go ve RL/HOR ölçümlerinde cinsiyet farklılıkları biyometrik olarak önemli düzeyde bulunmuştur. Varyans analizi sonucunda doğal baş ve hyoid kemiği konumunu belirleyen ölçümlerin tonsilla hipetrofisinden etkilenmedikleri gözlenirken, mandibula ve dil konumunu belirleyen ölçümlerden ML/VER, N-Me ve tl-PTM ölçümlerinin biyometrik olarak önemli düzeyde tonsiller hipertrofiden etkilendikleri gözlenmiştir.

Ill SUMMARY The purpose of this study has been to analyse whether there were any differences between the children with and without enlarged tonsils with regard to the postures of head, hyoid bone, mandible, and tongue. Fifty children with enlarged tonsils were compared with a matched control group. The study and control groups were composed of males and females at equal numbers. The ages of subjects ranged from 7 to 10. Cephalometric head films of them were taken at natural head position. The subjects selected for this study were expected: - To have undergone no previous orthodontic treatment, - not to have any wound, burn, or scar tissue in the head, face, and neck regions, - not to have any visual or hearing disorder, - to have accebtable facial balance and a normal occlusion, - to have no developmental problem. The method errors associated with radiographic tracings and measurements were investigated and no error was found. The effects of tonsils and sex on the posture of head, hyoid bone, mandible, and tongue were investigated by means of variance analysis. It has been observed that NSL/VER, NSL/CVT, NSL/OPT, and CVT/HOR of the head posture measurements; H-Gn, ELLSN-S, and H-S of the hyoid bone measurements; N-Me, S-Go, and RL/HOR of the mandibular measurements; and no measurements regarding tongue were affected by sex. It has also been found by this analysis that the measurements used to determine the natural head position and the position of the hyoid bone were not affected by tonsil enlargement, and that three measurements regarding mandibular and tongue posture, ML/VER, N-Me, and tl-PTM were affected at a statistically significant level by tonsillar hypertrophy.
