Psödoeksfoliasyonlu olan hastalarda ameliyat öncesi ve sonrası oküler parametrelerin scheimpflug camera (sirius) ile değerlendirilmesi

Abstract

Amaç: Psödoeksfoliasyonlu kataraktlı hastalarda sorunsuz fakoemülsifikasyonunSirius-Scheimflug ile elde edilen ön segment parametreleri ve göz içi basıncı (GİB)üzerine etkisini değerlendirmek.Hasta-Metod: Bu prospektif çalışmaya fakoemülsifikasyon ameliyatı yapılanpsödoeksfoliasyonu olmayan 35 hastanın 35 gözü ( grup 1) ve fakoemülsifikasyonameliyatı yapılan psödoeksfoliasyonu olan 37 hastanın 40 gözü ( grup 2) dahil edildi.Hastalar ameliyat öncesi ve sonrası 1.hafta, 1. ve 3. aylarda Sirius-Scheimflug cihazıile ön segment değerlendirildi ve Goldmann aplanasyon tonometrisi ile GİB'larıölçüldü. Ön segment parametrelerinden santral kornea kalınlığı (SKK), ön kamarahacmi (ÖKH), ön kamara derinliği (ÖKD), ön kamara açısı (ÖKA) ve kornea hacmi(KH) değerlerini değerlendirdik.Bulgular: ÖKD, ÖKH, ÖKA, KH ve GiB değerlerinde ameliyat öncesi ile ameliyatsonrası 1. hafta, 1. ay ve 3. aylarda değerler arasında istatistiksel olarak anlamlı farkvardı (p<0.05). SKK ölçümlerinde ise ameliyat öncesi ve ameliyat sonrası 1. hafta, 1.ay ve 3. aylarda değerler arasındaki fark istatistiksel olarak anlamlıydı (p<0.05).Sonuç: Sorunsuz fakoemülsifikasyon, belirgin GİB düşüşüne, ÖKD, ÖKH, KH'daartışa ve ÖKA'da genişlemeye neden olmaktadır. SKK ve KH'daki değişim ameliyatsonrası 1. aydan itibaren stabilleşmektedir. Grup 2'de eksfolyatif materyalininendotel üzerinde yaptığı strese bağlı olarak parametreler'deki değişim daha dramatikolmuştur. Bizim çalışmamız göstermektedir ki 3 ayda parametrelerdeki değişimstabil olmuştur.

Purpose: To evaluate the effects of uneventful phacoemulsification inpseudoexfoliation cataract patients on the anterior segment parameters obtained withSirius-Scheimflug and intraocular pressure (IOP).Methods: Thirty-five patients without pseudoexfoliation who underwentphacoemulsification surgery and forty eyes of 37 patients with pseudoexfoliationwho underwent phacoemulsification surgery were included in this prospective study.Patients were evaluated preoperatively and postoperativly at the 1st week, 1st and3rd months with an Sirius-Scheimflug device, and IOPs were measured withGoldmann applanation tonometry. We evaluated central corneal thickness (CCT),anterior chamber volume (ACV), anterior chamber depth (ACD), anterior chamberangle (ACA), and corneal volume (CV) from the anterior segment parameters.Results: The difference between preoperative ACV, ACD, ACA, CV and IOP valuesand postoperative 1st week, 1st and 3rd months ACV, ACD, ACA, CV and IOPvalues were statistically significant (p<0.05). The differences between preoperativelyand postoperatively at the 1st week, 1st and 3rd months CCT measurement werestatistically significant (p<0.05).Conclusions: Uneventful phacoemulsification significantly reduced IOP, increasedACV, ACD, CV and widened the ÖKA. CCT and CV alteration remains stable afterfirst month of surgery. The change in parameters has become more dramaticdepending on the stress that the exfoliative material makes on the endothelium in thegrup 2. Our work shows that in 3 months the change in the parameters has becomesteady.
