dc.description.abstractÖZET Son yıllarda birçok ülkeden penisiline orta düzey (MİK>0.12 ug/mL) veya yüksek düzey (MİK>2 u-g/mL) dirençli Streptococcus pneumoniae suşları bildirilmektedir. Tüm dünyada bu dirençli izolatlann çoğunun bulunduğu beş serogrup veya serotip 6, 9, 14, 19 ve 23'tür. Penisiline dirençli pnömokok insidansının çoğunlukla bazı dirençli klonlann yayılması nedeniyle arttığı bilinmektedir. Bu çalışmada klinik örneklerden izole edilen 193 S. pneumoniae susu değerlendirildi. Penisiline duyarlılık E-test ile belirlendi. Yüksek düzey penisilin direnci saptanamazken, orta düzey direnç % 23 olarak bulundu. Serotiplendirme Statens Serum Institute'den elde edilen antiserumlarla `Quellung` reaksiyonu ile yapıldı. Dirençli suşlarda, izolatlann çoğunun sırasıyla serogrup 19, 23, 14 ve l'e ait olduğu görülürken; duyarlı suşlarda en çok serogrup 19, 1, 6, 23 ve 3 saptandı. `Pulsed-field gel electrophoresis` ile penisiline dirençli suşlarda sekiz farklı ana klon saptandı. Yöremizdeki penisiline orta düzey pnömokoklarda genetik heterojenite belirlendi.
dc.description.abstractSUMMARY Penicillin Susceptibility and Scrotyping of Streptococcus pneumoniae Strains; Investigation of Clonality in Penicillin-resistant Strains Isolates of Streptococcus pneumoniae with intermediate-level penicillin resistance (MIC>0.12 ug/mL) or high-level resistance (MIC>2 ug/mL) have been reported from many countries in recent years. The five serogroups or serotypes found most frequently in these isolates all over the world are serogroup/serotype 6, 9, 14, 19, and 23. It has been known that a major part of the increase in incidence of penicillin- resistant 5`. pneumoniae was due to the spread of some pneumococcal clones. In this study we evaluated 193 isolates of S. pneumoniae isolated from clinical specimens. E- test was used to evaluate susceptibility to penicillin. We found the prevalence of intermediate-level penicillin resistance as 23 %. There were no high-level resistant pneumococci. Serotyping was performed with antisera from Statens Serum Institute by `Qellung` reaction. In susceptible strains, the majority of isolates were of serogroup/serotype 19, 1, 6, 23 and 3; while the majority of penicillin-resistant strains were of serogroup/serotype 19, 23, 14, and 1, respectively. It was seen that there were eight major clones in penicillin-resistant pneumococci with `pulsed-field gel electrophoresis`. It was found that there was genetic heterogenity in intermediate-level penicillin-resistant pneumococci in our region. nen_US
