dc.description.abstractAmaç: Aseptik menenjit birçok nedene bağlı olarak gelişebilen,inflamatuar bir meninks hastalığıdır. Virüsler akut aseptik menenjitin öndegelen etkenidir. Enterovirüsler viral menenjitlerin %80-95'inden sorumludur.HSV tüm aseptik menenjit vakalarının %0.5-3'ünü oluşturur. Bu çalışmada,aseptik menenjitli hastaların BOS örneklerinde hücre kültürü ve PCRyöntemleri ile enterovirüs, HSV, VZV ve CMV araştırılması amaçlandı.Gereç ve yöntem: Mart 2004-Haziran 2005 dönemleri arasındaErciyes Üniversitesi Gevher Nesibe Hastaneleri Kliniklerinde takip edilenaseptik menenjit ön tanılı 30 hastadan alınan BOS örnekleri çalışmaya dahiledildi. BOS örneklerinde HSV-DNA HSV QL 1.0 (Fluorion, Iontek, Türkiye);CMV DNA CMV QL 1.0 (Fluorion, Iontek, Türkiye); VZV-DNA VZV DNARealArt (Artus, Almanya) ve Enterovirus-RNA ise RealArt enterovirüs LCkitleri (Roche Diagnostics, ABD) ile araştırıldı.Enterovirüs, HSV, VZV ve CMV izolasyonu için Hep-2 ve MRC-5hücre kültürleri kullanıldı. Otuz örneğin 23'ünden Hep-2 ve MRC-5 hücrekültürlerine ekim yapıldı.VIIBulgular: Çalışmaya alınan 30 örneğin 15'inde HSV-DNA pozitifbulundu. Örneklerin hiçbirinde CMV-DNA, VZV-DNA ve enterovirüs-RNAgösterilemedi. Hep-2 hücre kültürüne ekimi yapılan 23 örneğin 7'sinde 2.günde HSV ile uyumlu sitopatik etki görüldü. Bu örneklerin hepsinde HSV-DNA pozitif idi. HSV-DNA pozitif bulunan 1 örnekte ise HSV ile uyumlusitopatik etki izlenmedi. MRC-5 hücre kültürüne ekimi yapılan 23 örneğinsadece birinde HSV ile uyumlu sitopatik etki izlendi, bu örnekte Hep-2hücresinde HSV ile uyumlu sitopatik etki gözlendi ve HSV-DNA pozitifbulundu.Sonuç: Sonuç olarak; aseptik menenjit ön tanılı hastaların BOSörneklerinde enterovirüs, HSV, CMV ve VZV'yi hücre kültürü ve PCRyöntemi ile araştırılması uygundur. Aseptik menenjit etkenlerinin hızlı vedoğru olarak belirlenmesinde her iki yöntemin birlikte kullanımıönerilmektedir.Anahtar kelimeler: Aseptik menenjit, CMV, Enterovirüs, HSV,Hücre kültürü, PCR, VZVVIII
dc.description.abstractAim: Aseptic meningitis is an inflammatory disease of the meningesthat caused by variety of agents. Viruses are the major cause of the acuteaseptic meningitis. Enteroviruses are responsible for the 80-95% of viralmeningitis. Herpes simplex meningitis consists of 0.5-3% of the all asepticmeningitis cases. In this study the aim was to investigate enterovirus, HSV,VZV and CMV in cerebrospinal fluid of the aseptic meningitis patients by cellculture and PCR methods.Materials and methods:Cerebrospinal fluid specimens of the 30 patients with suspiciousaseptic meningitis treated in Erciyes University Gevher Nesibe HospitalsClinics between march 2004 and june 2005 were included in this study. HSV-DNA was investigated in cerebrospinal fluid with HSV QL 1.0 (Fluorion,Iontek, Turkey); CMV DNA was investigated in cerebrospinal fluid with CMVQL 1.0 (Fluorion, Iontek, Turkey); VZV-DNA was investigated inIXcerebrospinal fluid with VZV DNA RealArt (Artus, Germany) andEnterovirus-RNA was investigated in cerebrospinal fluid with RealArtenterovirus LC (Roche Diagnostics, USA) kits.Hep-2 and MRC-5 cell cultures were used for isolation of enterovirus,HSV, VZV and CMV. Twentythree of the 30 specimens were inoculated toHep-2 and MRC-5 cell cultures.Results:HSV-DNA was found positive in 15 of the 30 specimens. CMV-DNA,VZV-DNA and enterovirus-RNA was not found in any of the specimens.Seven of the HSV-DNA positive specimens could not be inoculated to cellculture because of the inadequate specimen. Cytopathic effect consistent withHSV was observed on the second day of inoculation in 7 of the 23 specimenswhich were inoculated to Hep-2 cell culture. HSV-DNA was found positive inall of these 7 specimens. Cytopathic effect consistent with HSV was notobserved in one of the HSV-DNA positive specimens. Cytopathic effectconsistent with HSV was observed in one of the 23 specimens whichinoculated to MRC-5 cell culture, cytopathic effect consistent with HSV inHep-2 cell culture was shown in this specimen and HSV-DNA was foundpositive.Conclusion:As a result; it is appropriate to investigate the cerebrospinal fluidspecimens of the patients with suspicious aseptic meningitis for enterovirus,HSV, VZV and CMV by cell culture and PCR methods. It is suggested thatthese two methods be applied together for a rapid and correct detection of theaseptic meningitis agents is.Key words: Aseptic meningitis, CMV, Enterovirus, HSV, Cellculture, PCR, VZVXen_US
