dc.description.abstractÖZETAmaç: Hiperhomosisteinemi ve vasooklüzif hastalıklar arasındaki ilişki uzun süredenberi bilinmektedir. Homosistein sülfir içeren bir aminoasit olup metionin metabolizmasındarol oynar ve homosisteinin metionine remetilasyonunda folik asit ve vitamin B12 gereklidir.Viamin B12 ve folik asit eksikliğinde homositein düzeyleri yükselebilir. Hiperhomosisteinemivasküler endotel hasarına ve buda ateroskleroza neden olur.Bu çalışmanın amacı valproik asit, okskarbazepin ve karbamazepinin serum homosistein,folik asit ve vitamin B12 düzeyleri üzerine etkisini araştırmaktır.Gereçler ve Yöntem: Son bir yıldır karbamazepin, valproik asit ve okskarbazepintedavisi alan 30'ar hasta ile kontrol gurubu olarakta aynı yaş gurubunda sağlıklı 30 hastaçalışmaya alındı. Oniki saatlik açlıktan sonra serum homosistein, vitamin B12 ve folik asitdüzeyleri ölçüldü ve istatistiksel olarak kontrol gurubu ile karşılaştırıldı.Bulgular: Ortalama homosistein, folik asit ve vitamin B12 düzeyleri kontrolgurubunda sırasıyla 34µmol/L (4.27-26.88), 10.04ng/ml (6.09-21.33), 600pg/ml (158-911),valproik asit gurubunda 11.30µmol (5.82-41.82), 5.43ng/ml (3.38-7.46), 665.50pg/ml (502-911), karbamazepin gurubunda 10.95µmol/L (5.01-32.77), 5.31 ng/ml (3.16-7.92), 702pg/ml(500-985) okskarbazepin gurubunda ise 10.24 µmol/L(6.15-116.19), 5.78ng/ml(3.30-7.64),665.50pg/ml(413-912) idi.Valproik asit, karbamazepin ve okaskarbazepin kullanan hastalarda homosistein düzeylerikontrol gurubundan daha yüksek idi (p<0.05).Serum folik asit düzeyleri valproik asit, karbamazepin ve okskarbazepin gurubunda kontrolgurubuna göre düşük bulundu (p<0.05).Valproik asit, karbamazepin ve okskarbazepin gurubunda vitamin B12 düzeyleri kontrolgurubuna göre anlamlı yüksek bulundu (p<0.05). Folik asit ile homosistein arasında negatifbir ilişki olduğu saptandı.Sonuç:Bu çalışmada valproik asit, karbamazepin ve okskarbazepin gibi antiepileptikilaç tedavisi alan hastalarda homosistein düzeyinin yükselebileceği ve folik asit düzeylerininde azalabileceği tespit edildi.Anahtar kelimeler: Epilepsi, antiepileptik ilaçlar, homosistein, Vitamin B12, folikasit
dc.description.abstractABSTRACTBackground: The relationship between hyperhomocysteinemia and vasoocclusivedisease is known for a long time. Homocysteine is a sulfur containing amino acid, whichplays a role in methionine metobolism. Folic acid and vitamin B12 are essential forremethylization of homocysteine to methionine. Homocysteine levels increases in thedeficiency of these vitamins. Hyperhomocysteinemia causes vascular endotelial damage,which causes atherosclerosis.Aim: The aim of this study is to investigate the effect of valproate, carbamazepine andoxcarbazepine on the serum levels of homocysteine, vitamin B12 and folic acid.Material and Methods: Thirty children receiving carbamazepine, 30 childrenreceiving valproic acid, 30 children receiving oxcarbazepine for the last 1-year period and 30healthy children as control were the population of tis study. After 12 h of fasting serumhomocysteine, vitamin B12 and folic acid levels were measured and results were comparedstatistically.Results: Mean values of homocysteine, folic acid and vitamin B12 levels in controlgroup were 7.34µmol/L (4.27-26.88), 10.04ng/ml (6.09-21.33), 600pg/ml (158-911), invalproic acid group 11.30µmol (5.82-41.82), 5.43ng/ml (3.38-7.46), 665.50pg/ml (502-911)in carbamazepine group 10.95µmol/L (5.01-32.77), 5.31 ng/ml (3.16-7.92), 702pg/ml(500-985), in oxcarbazepine group 10.24 5.78ng/ml(3.30-7.64),µmol/L(6.15-116.19),665.50pg/ml(413-912) respectively.Serum homocysteine levels were higher in valproic acdi, oxcarbazepine and carbamazepinegroups than control group (p<0.05). Serum folic acid levels were lower in valproic acid,oxcarbazepine and carbamazepine groups than control group (p<0.05).Serum levels of vitamin B12 were higher in valproic acid, oxcarbazepine and carbamazepinegroups than control group (p<0.05). A significiant inverse relationship was found betweenfolic acid levels and homocysteine levels in the patient group.Conclusion: Our study demonstrates that prolonged treatment with valproic acidoxcarbazepine and carbamazepine increases plasma concentrations of homocysteine anddecrease folic acid.Key words: Epilepsy, antiepileptic drugs, homocysteine, Vitamin B12, folic aciden_US
