dc.description.abstractGiris ve Amaç: Akut apandisit tanısı için en sık kullanılan laboratuvar testleri, kanlökosit sayımı (BK) ve serum C-reaktif protein (CRP) düzeyi ölçümüdür. Negatifapandektomi ve perforasyon oranlarını düsürmek için yardımcı tanı yöntemlerigelistirilmektedir. Bu çalısmada akut apandisitteki inflamasyon ile leptin seviyeleriarasındaki iliski ve en sık görülen semptom olan istahsızlıgın mekanizmasındaleptinin rolünün belirlenmesi amaçlanmıstır.Hastalar ve Yöntem: Ocak 2006-Subat 2007 tarihleri arasında akut apandisitdüsünülen ve operasyon sırasında makroskobik olarak akut apandisit tanısı konulan40 hasta (Grup I) ile özgeçmisinde ve çalısmaya alındıgı sırada inflamatuvar hastalıgıolmayan 20 çocugun (Grup II) serum leptin seviyeleri, BK sayıları ve CRPdüzeyleri ölçüldü. Grup I `deki hastaların yirmiyedisi erkek, onüçü kız, yasortalamaları 7.3 yas (2?10) idi. Grup II'deki çocukların onbesi erkek, besi kız, yasortalamaları 5.6 yas (2?10) idi. Ölçümler preoperatif, peroperatif ve postoperatif 2.gün yapıldı. Verilerin degerlendirilmesinde Kolmogrov-Smirnov, Student-t testi veMann-Whitney U testi kullanıldı.Bulgular: Hastaların 26'sında (% 65) perfore apandisit, 14'ünde (% 35) perforeolmamıs akut apandisit tespit edildi. Perfore apandisiti olan ve olmayan hastalarınBK degerleri arasında anlamlı fark bulunamadı. Hastaların CRP degerleri kontrolgrubuna göre yüksek saptandı. Perfore apandisiti olan hastaların CRP degerleri,perfore olmayanlara göre yüksekti. iki grup arasındaki preoperatif ve peroperatifleptin degerleri istatistiksel olarak anlamlı degildi. Hastaların postoperatif 2.günleptin degerleri kontrol grubuna göre daha düsüktü ve sonuçlar istatistiksel olarakanlamlı bulundu. Perfore apandisiti olmayan hastaların leptin degerleri, perforeolanlara göre daha yüksekti ve sonuçlar istatistiksel olarak anlamlı idi. Vücut kitleindeksi ile leptin arasında hem hasta hem de kontrol grubunda pozitif yöndekorelasyon saptandı.Sonuç: Akut apandisit tanısında BK ve CRP yararlı iken, perforasyona gidisigöstermede BK etkisiz, CRP etkili bir parametre olarak bulundu. istahsızlık üzerindeetkili oldugu bilinen ve hastalıgın akut döneminde arttıgı görülen leptinininflamatuvar cevap sırasında önemli rol oynadıgı düsünüldü.
dc.description.abstractIntroduction and Aim: The most common laboratory tests for the diagnosis of acuteappendicitis (AA) are white blood cell count (WBCC) and measurement of serum Creactive protein (CRP) levels. To decrease the ratio of negative appendectomy andperforations, additional diagnostic tests are being developed. In this study it wasaimed to determine both the relationship between the inflammation at AA and theleptin levels and the role of leptin at anorexia, which is the most common symptom.Patients and Methods: This study was carried out from January 2006 to February2007. The serum leptin levels, WBCC and CRP levels of 40 patients (Group I) whowere dignosed with AA macroscopically during the operation and of 20 children(Group II) who did not have an inflammatory disease in their background and atpresent were investigated. 27 of the patients in Group I were boys and 13 of themwere girls. The average age of the patients in Group I was 7.3 with an age range of2?10 years. 15 of the children in Group II were boys and 5 of the them were girls.The average age of the children in Group II was 5.6 with an age range of 2?10years. Serum leptin levels, WBCC and CRP levels of these children were measuredpreoperatively, perioperatively and at 48 h after the start of the operation. For theexamination of the data Kolmogrov-Smirnov, Student-t test and Mann-Whitney Utest were used.Results: It was found out that 26 of the patients (%65) were diagnosed withperforated AA and 14 of the patients (%35) were diagnosed with non-perforated AA.There was no significant difference in the WBCC between the patients with theperforated appendicitis and the non-perforated AA. The CRP levels of the patientswere higher than the control group. The CRP levels were higher in patients withperforated AA than those patients with non-perforated AA. The difference betweenpreoperative and perioperative leptin levels of the groups was not statisticallysignificant. But, leptin levels at 48 h after the start of the operation were lower thanthe Group II and the difference was statistically significant. Leptin levels of thepatients with non-perforated appendicitis were higher than those of the patients withperforated appendicitis and the difference was statistically significant. There waspositive correlation between the body mass index and the leptin levels in bothgroups.Conclusion: As a result of the study, it can be concluded that the WBCC andmeasurement of serum CRP are useful in the diagnosis of AA. While the WBCC isnot able to determine the perforation, CRP is an effective parameter in determiningthe perforation. On the other hand, it can be concluded that since leptin, which has aneffect on anorexia, increased during the acute inflammatory period of the disease, ithas an important role during inflammatory response.en_US
