dc.description.abstractAmaç: İzole koroner arter ektazisi olan hastalarda arteryel stifness artışı araştırılarak ikisi arasındaki etyopatogenetik ilişkiyi incelemek.Yöntem: Çalışmaya anjiografik olarak izole koroner ektazi saptanan 24 hasta (15 erkek, 9 kadın, ortalama yaş 55 ± 13 yıl), ektazi ile birlikte stenotik koroner arter hastalığı olan 25 hasta (20 erkek, 5 kadın, ortalama yaş 61 ± 10 yıl) ve normal koroner arterleri olan 24 hasta (10 erkek, 14 kadın, ortalama yaş 55 ± 9 yıl) alındı. Tüm hastalara SphygmoCor cihazıyla non-invaziv olarak arteryel stifnes ölçümü yapıldı. Karotis-femoral nabız dalga hızı (karotis-femoral PWV), augmentasyon basıncı (AP), augmentasyon indeksi (AIx) ve kalp hızına normalize edilmiş augmentasyon indeksi (AIx@75) srteryel stifnes parametreleri olarak kullanıldı.Bulgular: Sigara ve cinsiyet dışındaki demografik özellikler ile lipit paneli ve medikasyon açısından her üç grup arasında fark yoktu. AP, AIx ve AIx@75 açısından her üç çalışma grubu arasında anlamlı farklılık bulunamadı. Karotis-femoral PWV ektazi+KAH grubunda normal koroner grubuna göre anlamlı olarak yüksek bulundu.Bununla birlikte, izole ektazi ile ektazi+KAH ve normal koroner grubu arasında anlamlı farklılık bulunamadı.Sonuç: Koroner ektazi olan hastalarda arteryel stifnes artışı olmaktadır. Bu durum ektazinin lokal bir hastalık olmakdan çok sistemik bir patoloji olduğunu ve etyolojide ateroskleroz gibi sistemik bir sebebin rol aldığını düşündürür.Anahtar kelimeler : arteryel stifnes, koroner ektazi, pulse wave velosite
dc.description.abstractAim: Assesment of arterial stiffness in patients with isolated coronary ectasia and to investigation the etiologic relationship between coronary ectasia and arterial stiffness.Method: Study population included 24 patients with isolated coronary ectasia (15 male, 9 female, mean aged 55 ± 13 years) and 25 patients who have coronary ectasia and atherosclerotic coronary stenoses (20 male, 5 female, mean aged 61 ± 10 years) diagnosed by coronary angiography. 24 patiens who have normal coronary epicardial arteries in coronary angiography also were recruited as a control group (10 male, 14 female, mean aged 55 ± 9 years). Arterial stiffness measures was performed by non-invasive SphygmoCor device in all patients. Carotid-femoral pulse wave velocity (PWV), augmentation pressure (AP), augmentation index (AIx) and augmentation index normalized for heart rate of 75 bpm (AIx@75) was used as arterial stiffness parameters.Results: The demographic characteristics (except for smoking and gender), parameters of lipid status and medications of three groups were similar. There were no differences statistically among three study groups for AP, AIx ve AIx@75. Carotid-femoral PWV was found significantly higher (p:0.004) in ectasia plus coronary artery disease group compared with normal epicardial coronary group. However, among the isolated ectasia group and ectasia plus coronary artery disease group or normal coronary group were not found differences statistically for carotid-femoral PWV.Conclusions: Arterial stiffness have increased in patient with coronary ectasia. This condition suggest that ectasia is a diffuse pathology rather than a local disease and its etiology consist a systemic cause as atherosclerosis.Keywords : arterial stiffness, coronary ectasia, pulse wave velocityen_US
