Alt çene gelişim geriliğine bağlı geç dönem iskeletsel sınıf II hastalarda iskeletsel ankraj desteği ile birlikte kullanılan forsus FRD apareyinin etkilerinin sefalometrik olarak incelenmesi

Abstract

Bu çalışmanın amacı alt çene gelişim geriliğine bağlı iskeletsel Sınıf II maloklüzyonun tedavisi için kullanılan mini plak ankrajlı Forsus FRD apareyinin iskeletsel, dentoalveolar ve yumuşak doku etkilerini incelemekti. Çalışma grubu, dahil edilme kriterlerini taşıyan ve alt çene geriliğine bağlı Sınıf II maloküzyona sahip 17 bireyi (11 kız ve 6 erkek, ortalama yaş: 12.96±1.23 yıl) içermiştir. Üst dental ark seviyelendikten ve üst dental arka 0.019x0.025 inç paslanmaz çelik tel yerleştirilip distal sonu kıvrıldıktan sonra, mandibular simfiz bölgesine çift taraflı olarak 2 adet mini plak yerleştirilmiştir. Daha sonra Forsus FRD apareyi, üst çenede üst molar dişin headgear tüpüne ve alt çenede mini plaklara uyarlanmıştır. Seviyeleme ve iskeletsel ankraj destekli Forsus FRD safhalarında meydana gelen değişiklikler lateral sefalometrik röntgenler kullanılarak Paired-t testi ve Student t-testi ile değerlendirilmiştir. Üst çenenin öne doğru gelişiminin önemli derecede frenlendiği (p<0.001), alt çenenin öne doğru gelişiminin önemli derecede stimüle edildiği (p<0.001) tespit edilmiştir. Overjet düzeltiminin (-5.11 mm) büyük oranda iskeletsel olarak meydana geldiği (A-VRD: -1.16 mm ve Pg-VRD: 2.62 mm; yaklaşık %74), kalan düzelmenin ise dentoalveolar olduğu bulunmuştur. Üst ve alt kesiciler önemli oranda retrüze olmuştur (p<0.001).Sonuç olarak bu yeni yaklaşım alt çene geriliğine bağlı Sınıf II maloklüzyonun tedavisinde iskeletsel ve dentoalveolar değişimler ile birlikte etkili bir yöntem olduğu bulunmuştur.Anahtar sözcükler: Ġskeletsel Sınıf II maloklüzyon, Forsus FRD, Kemik ankrajı2

The aim of this study was to evaluate the skeletal, dentoalveolar and soft tissue effects of the Forsus FRD appliance with mini plate anchorage used for the treatment of skeletal Class II malocclusion due to the mandibular retrusion.The study group included 17 patients (11 females and 6 males; mean age: 12.96±1.23 years) who met the criteria with Class II malocclusion due to mandibular retrusion. After leveling upper dentition and 0.019 x 0.025-inch stainless steel archwire was inserted and cinched back in the maxillary arch, two mini plates were placed bilaterally on mandibular symphysis. Then, Forsus FRD was adjusted to headgear tubes of molar bands in maxilla and to the mini plates in mandibula. The changes in the leveling and skeletal anchoraged Forsus FRD phases were evaluated by means of the Paired t- and Student's t-tests using the cephalometric lateral films. It was found that significantly restraint in the sagittal position of maxilla (P<0.001), significantly forward in the sagittal position of the mandible (P<0.001). The overjet correction (-5.11 mm) was found to be mainly by skeletal changes (A-VRL: -1.16mm and Pog-VRL: 2.62mm; approximately 74%), while the remaining changes were due to the dentoalveolar contributions. The maxillary and mandibular incisors were significantly retruded (P<.001).Finally, this new approach was found to be an effective method for the treatment of skeletal Class II malocclusion due to the mandibular retrusion via a combination of skeletal and dentoalveolar changes.Key Words: Class II malocclusion, Forsus FRD, Skeletal anchorage
