Göğüs cerrahisi hastalarında müzik terapinin ameliyat öncesi anksiyete ve fizyolojik parametreler üzerine etkisi

Abstract

Araştırma, göğüs cerrahisi hastalarında müzik terapinin ameliyat öncesi anksiyete ve fizyolojik parametreler üzerine etkisini değerlendirmek amacı ile deneysel olarak yapıldı. Araştırma Trabzon İl merkezinde bulunan KTÜ Farabi Hastanesi Göğüs Cerrahisi Kliniğine yatışı yapılan 80 hasta (40 deney, 40 kontrol) ile gerçekleştirildi. Veriler `Hasta Bilgi Formu`, `Fizyolojik Parametre Ölçüm Formu` ve `Durumluk/Sürekli Anksiyete Ölçeği` ile ameliyat öncesi dönemde toplandı. Deney grubuna CD çalar ile müzik dinletilirken, kontrol grubuna müzik dinletilmedi. Araştırmada, deney grubunda ilk ve son ölçümde DAPO ve SAPO arasında istatistiksel olarak anlamlı farklılık (p=0.000) olup, DAPO ve SAPO değerlerinin müzikle azaldığı tespit edilirken kontrol grubunun anksiyete puan ortalamalarının aynı kaldığı saptandı. Deney grubunda ilk ve son ölçümde SKBO, KH, SS ve SPO2 yönünden istatistiksel fark bulundu (p<0.005). Deney grubunda ilk ölçüm ile son ölçüm arasında SKBO, DKBO, KH ve SS ve değerlerinin azaldığı ve SPO2'nun arttığı belirlendi. Kontrol grubunda hiçbir parametrede fark olmadı. Araştırma sonuçlarına göre, ameliyat öncesi dönemde müzik terapinin anksiyete düzeyinin azaltılması ve fizyolojik parametreler üzerine olumlu yönde etkisi olduğundan hemşirelik uygulamalarında kullanımının yaygınlaştırılması önerilebilir.

The study was planned as an experimentally study in order to review the effect of music therapy on preoperative anxiety and physiological parameters in thoracic surgery patients. The study was undertaken with 80 patients (40 experiments, 40 controls) who were hospitalized in Karadeniz Technical University Farabi Hospital, Thoracic Surgery Department. The data were collected preoperatively with `Patient Information Form`, `Physiological Parameter Measurement Form` and `State/Trait Anxiety Inventory`. The experimental group was listening to the music with the CD player and the music was not played to the control group. In the study, there was a statistically significant difference (p=0.000) between SASA and TASA in the first and last measurements in the experimental group, while SASA and TASA values decreased with music, while the mean anxiety score of the control group remained the same. In the experimental group, there was a statistically significant difference in terms of SBPA, HR, RR and SPO2 in the first and last measurements (p <0.005). In the experimental group, it was determined that the values of SBPA, DBPA, HR and RR and between the first and last measurements decreased and SPO2 increased. No difference was observed in the control group. According to the results of the study, it can be suggested to decrease the anxiety level of music therapy in preoperative period and to increase its use in nursing applications since it has a positive effect on physiological parameters.
