İskeletsel sınıf II olguların herbst ve xbow apareyleri ile tedavi etkilerinin sefalometrik olarak incelenmesi

Abstract

Bu çalışmanın amacı, alt çene gelişim geriliğine bağlı iskeletsel Sınıf II malokluzyonlu olguların tedavisinde, pubertal büyüme atılımı döneminde kullanılan Herbst ve Xbow apareylerinin iskeletsel, dentoalveolar yapılar ve yumuşak dokular üzerine etkilerini karşılaştırılmalı olarak incelemektir.Çalışma grubu alt çene gelişim geriliğine bağlı Sınıf II malokluzyona sahip pubertal büyüme atılımı döneminde bulunan ortalama yaşı 12.08 ±1.23 yıl olan 46 bireyi içermiştir. Bireyler randomize bir şekilde gruplara ayrılmıştır. 16 bireye Herbst, 19 bireye Xbow sabit fonksiyonel apareyleri uygulanmıştır. Kontrol grubu da 11 hastanın arşivdeki kayıtları kullanılarak retrospektif olarak oluşturulmuştur. Keser dişleri başbaşa gelinceye kadar aktive edilen apareyler 7 ay süreyle aylık kontrolleri yapılarak ağızda tutulmuştur. Tedavi başlangıcında ve apareylerin sökümünden hemen sonra alınan lateral sefalometrik filmler ve kontrol grubundan elde ettiğimiz 7 ay aralıkla alınmış lateral sefalometrik filmler karşılaştırmalı olarak değerlendirilmiştir. Elde edilen verilerin istatistiksel olarak karşılaştırılması sonucu her iki apareyle de overjet ve molar ilişki düzeltiminin büyük oranda dişsel olarak sağlandığı ve Xbow grubundaki dişsel etkinin Herbst grubuna göre çok daha fazla etkili olduğu görüldü. Kontrol grubuyla karşılaştırıldığında Herbst apareyinin mandibulanın sagital yön gelişimini stimüle edici bir etkiye sahip olduğu, Xbow apareyinin ise mandibulanın sagital yön gelişimine etkisinin sınırlı olduğu tespit edilmiştir.Sonuç olarak, Xbow ve Herbst apareylerinin büyüme gelişim dönemindeki bireylerde alt çene gelişim geriliğine bağlı Sınıf II malokluzyonu düzeltmeye yönelik etkili apareyler olduğu görülmüştür. The aim of this study was to compare the effects of Herbst and Xbow appliances on skeletal, dentoalveolar structures and soft tissues in the treatment of skeletal class II malocclusion cases in pubertal growth spurt due to deficiency of mandibular growth. The study sample included 46 patients with a mean age of 12.08 ±1.23 years in the period of pubertal growth spurt with Class II malocclusion due to deficiency of mandibular growth. The sample was randomly divided into groups. Herbst fixed functional appliance was applied to 16 patients and Xbow fixed functional appliance was applied to 19 patients. The control group was retrospectively constructed using the archival records of 11 patients. Appliances were kept activated in all cases until incisors came to edge-to-edge contact and hold in place for 7 months by monthly checks. Lateral cephalometric films taken at the beginning of treatment and just after removal of the appliances and lateral cephalometric films obtained from the control group for 7 months were evaluated comparatively. Statistical comparison of the obtained data showed that the overjet and molar relationship corrections were largely dentally by both appliances and the dental effect in the Xbow group was more effective than the Herbst group. Compared with the control group, the Herbst appliance had a stimulating effect on the development of the sagittal direction of the mandibula, whereas the Xbow appliance had limited effect on the sagittal direction development of the mandibula. As a result, it was seen that Xbow and Herbst were effective appliances for correcting Class II malocclusion due to deficiency of mandibular growth in the patients with growth development period.
