dc.description.abstractÖZET: Çalışmamızda, klinik örneklerden izole edilen maya türlerinin insidansının belirlenmesi, idantifikasyon yöntemlerinin karşılaştınnlması ve çeşitli antifungal ajanların in vitro aktivitelerinin saptanması amaçlanmıştır. Çalışmaya alman maya kültürü pozitif 210 klinik örneğin 40 (% 19.1)' m boğaz sürüntüsü, 40 (% 19.1)' m gaita, 40 (% 19.1)' m idrar, 40 (% 19.1)' m vajinal sürüntü, 40 (% 19.1)' nı deri ve tırnak kazmtısı ve 10 (% 4.5)' nu BOS örneği oluşturmuştur. Maya mantarlarının idantifikasyonu karbonhidratları asimile ve fermante etme yetenekleri, Pirinç ekstresi-Tween 80 ağardaki mikroskopik morfolojik görünümleri ve klamidospor oluşturma özelliklerine göre yapılmıştır. Klinik örneklerden 109 (% 51.91) C.albicans kökeni izole edilmiş olup, bunu sırasıyla C. pseudotropicalis, 27 (% 12.86); T.glabrata, 23 (% 10.96); C.krusei, 14 (%6.66); C.tropicalis, 14 (%6.66); Trichosporon spp., 14 (%6.66); C.parapsilosis, 8 (% 3.81) ve C.neoformans, 1 (% 0.48) türleri izlemiştir. C.albicans, incelenen klinik örnek gruplarının tümünde ilk sırada yer almış, bunu boğaz sürüntüsü ve gaita örneklerinde C. pseudotropicalis; idrar ve vajinal sürüntü örneklerinde T.glabrata; BOS örneklerinde C.tropicalis ve C.neoformans ve deri ve tırnak kazıntı örneklerinde Trichosporon spp. izlemiştir. Germ tüp testi ile C.albicans türleri % 91.74 oranında idantifıye edilmiştir. Pirinç ekstresi-Tween 80 ağardaki morfolojik idantifikasyon sonuçları, C.albicans türünde biyokimyasal sonuçlarla % 89.3 oranında uyumlu bulunurken diğer maya türlerinde bu oran % 80.0 olarak belirlenmiştir. Çalışmanın ikinci bölümünde 20 C.albicans, 20 C. pseudotropicalis ve 20 T.glabrata kökenin amfoterisin B, 5-florositozin, nistatin, ekonazol, mikonazol ve klotrimazole karşı duyarlılıkları makrobroth dilüsyon tekniği ile MIC ve MFC değerleri araştırılarak incelenmiştir. MIC ve MFC aralıkları sırasıyla amfoterisin B için 0.06-1 ug/ml / 0.06-2 ug/ml, 5-florositozin için 0.125-0.50 u.g/ml / 0.125-1 ug/ml, nistatin için 1.25-20 ug/ml / 1.25->40 ug/ml, ekonazol için 0.125-32 U-g/ml / 0.125->64 ug/ml, mikonazol için 800.125-16 ig/ml / 2->64 (ig/ml ve klotrimazol için 0.125-32 ug/ml I 2->64 ug/ml olarak belirlenmiştir. Çalışmamızda amfoterisin B ve 5-florositozinin en etkili antifungal ajanlar olduğu tespit edilmiştir. 81
dc.description.abstractSUMMARY: The aim of this study was to determine the incidence of yeast species, isolated from clinical materials, to compare identification methods and to determine in vitro activity of various antifungal agents. This study was carried out on six different groups. Each of five groups was consisting of 40 (19.1%) clinical materials of positive yeast culture, which were including throat smears, faeces samples, urine samples, vaginal smears, skin and nail samples groups. Sixth group was inculuding 10 CSF samples (4.5%). The yeast species were identified according to carbonhydrate assimilation, fermantation tests, germ tube production, microscopic morphologic differences and chlamydospore production in Rice Extract-Tween 80 agar. We defined 210 yeast species. C.albicans was the most common yeast (51.91%) in all clinical materials in all groups and it was followed by C.pseudotropicalis (12.86%), T.glabrata (10.96%), C.krusei (6.66%), C.tropicalis (6.66%), Trichosporon spp. (6.66%), C.parapsilosis (3.81%), C.neoformans (0.48%) in this study. C.pseudotropicalis was the second most common identified yeast in throat smears and faeces samples. Trichosporon spp. was the second most common identified yeast in nail and skin speciment. C.tropicalis and/or T.glabrata were the second most common identified yeast in urine samples and in vaginal smear. The identification of C.albicans by the methods of morphologic differences and chlamydospore production were comparable with biochemical results 89.3%. Positivity rates of germ tube was 91.74% for C.albicans. The morphological identification of other species was comparable with biochemical results 80%. C. albicans was the most common yeast in all clinical materials in all groups. The microscobic morphologic differences, chlamydospore and germ tube pruduction test are useful for identification. Sixty yeast strains including 20 of C.albicans, 20 of C.pseudotropicalis and 20 of T.glabrata were isolated from clinical specimens. In vitro sensitivity tests using amphotericine B, nystatine, econazole nitrate, miconazole nitrate, clotrimazole and 5- flourocytosine were carried out by using a macrobroth susceptibility test. The method 82allowed us to determine the minimum inhibitory concentration (MIC) of the six antifungal drugs used. We used different media for each drug. The MIC ranges (ig/ml) of six antifungal drugs were: 0.06-1 for amphotericine B, 2.5-10 ng/ml for nystatine, 0.125-32 for econazole nitrate, 0.125-16 for miconazole nitrate, 0.125-32 for clotrimazole and 0.125-0.50 for 5-flourocytosine. Amphotericine B and 5-fluorocytosine were found to be the most effective antifungal drugs. 83en_US
