Preeklampsinin şiddetinin değerlendirilmesinde total hCG ve serbest beta-hCG`nin yeri

Abstract

ÖZET Çalışmanın Amacı : Bu çalışmanın amacı; preeklampsili olgularda serum total hCG ve serbest B-hCG seviyelerini belirlemek ve bulguların hastalığın şiddeti ile olan ilişkisini ortaya koymaktır. Çalışmanın Yöntemleri: Bu çalışma Mayıs 2001 -Mayıs 2002 tarihleri arasında Ondokuz Mayıs Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalında 75 preeklampsili (24 hafif preeklampsi, 39 şiddetli preeklampsi, 12 eklampsi ) gebe olgu ve her hasta grubuna eşit sayıda toplam 75 sağlıklı gebeden oluşan kontrol grubu üzerinde yapıldı. Kontrol grubu olguları, hasta grubu olguların gebelik haftalarına uygun olacak şekilde ve sağlıklı gebelerden oluşturuldu (27-40 hafta). Total hCG ve serbest B-hCG için alman serum örnekleri ELİSA yöntemi ile çalışıldı. Elde edilen veriler istatistiksel olarak Student T, x2 ve Mann Whitney U testleri ile değerlendirildi. Çalışmanın Bulguları: Serum total hCG düzeyleri hafif preeklampsi ve kontrol grubunda sırasıyla (48661+45559 ve 31316±60364 mlU/mL), şiddetli preeklampsi ve kontrol grubunda sırasıyla (26483±35430 ve 12641±9948 mlU/mL), eklampsi ve kontrol grubunda ise sırasıyla (67514+55184 ve 20751+36477 mlU/mL) olarak bulundu. Hafif preeklampsi ile kontrol grubu olgularının serum total hCG düzeyleri arasında istatistiksel olarak anlamlı fark yoktu (p>0,05). Şiddetli preeklampsi ve eklampsili olgularda kontrol grublarına göre serum total hCG düzeyleri arasında istatistiksel olarak anlamlı fark vardı (p<0,05). Preeklampsili olgularda serum total hCG düzeyleri ile çalışmada kullanılan diğer parametreler (serbest P-hCG, sistolik ve diastolik kan basıncı, ürik asit, kreatinin) arasında anlamlı korelasyon saptanamadı. Serum serbest P-hCG düzeyleri hafif preeklampsi ve kontrol grubunda sırasıyla (21,6±28,8 ve 17,1±28,3 mlU/mL), şiddetli preeklampsi ve kontrol grubunda sırasıyla (22,0±40,4 ve 6,7±7,5 mlU/mL), eklampsi ve kontrol grubunda ise sırasıyla (42,9±43,5 ve 16,0±39,7 mlU/mL) olarak bulundu. Hafif preeklampsi ile kontrol grubu olgularının serum serbest P-hCG düzeyleri arasında istatistiksel olarak anlamlı fark yoktu (p>0,05). Şiddetli preeklampsi ve rveklampsili olgularda kontrol grublarına göre serum serbest P-hCG düzeyleri arasında istatistiksel olarak anlamlı fark vardı (p<0,05). Preeklampsili olgularda serum serbest 0- hCG düzeyleri ile çalışmada kullanılan diğer parametreler (total P-hCG, sistolik ve diastolik kan basıncı, ürik asit, kreatinin) arasında anlamlı korelasyon saptanamadı. Serum total hCG düzeyleri açısından hafif preeklampsi ile şiddetli preeklampsi arasında fark yokken (p>0,05), şiddetli preeklampsi ile eklampsi arasında istatistiksel olarak anlamlı fark saptandı (P<0,05). Serum serbest P-hCG düzeyleri açısından ise hafif, şiddetli preeklampsi ve eklampsi grupları arasında istatistiksel olarak anlamlı fark yoktu (p>0,05). Sonuç: Çalışmamızda serum total hCG ve serbest B-hCG düzeylerinin hem şiddetli preeklampsi hem de eklampsi grubunda, kontrol grubu hastalarına göre istatistiksel olarak anlamlı düzeyde yüksek olduğu saptandı. Bu da serum total hCG ve serbest B-hCG düzeylerinin şiddetli preeklampsi ve eklampsi teşhisinde destekleyici belirteçler olabileceğini göstermektedir. Bununla birlikte hafif preeklampsi ile kontrol grubu arasında serum total hCG ve serbest p-hCG seviyeleri yönünden istatistiksel olarak anlamlı fark bulunmaması (p>0,05), genel anlamda preeklampsinin belirlenmesinde veya öngörülmesinde bir belirteç olarak fazla bir anlam ifade etmediği sonucuna götürmektedir. ABSTRACT Objective: The aim of this study is to determine the serum total hCG and free 0-hCG levels in preeclamptic patients and to find out whether these levels are related to the severity of the diseases. Study Design: The study group comprised of 75 pregnant women with preeclampsia ( 24 mild preeclamptic, 39 severe preeclamptic and 12 eclamptic cases ) and a total of 75 healthy pregnant controls ( the number of cases in the control groups fitting each respectively ) who attended the Ondokuz Mayıs University of Medicine Faculty Obstetrics and Gynecology Department between May 2001 and May 2002. The control case with a similar gestational age was selected for each study case among the healthy pregnant women (27-40 weeks). Serum total hCG and serum free p-hCG levels were determined by the ELİSA method. Statistical analysis were done using Student T, x2 and Mann Whitney U tests. Results: The serum total hCG levels in the mild preeclampsia group and its control were (48661±45559 and 31316±60364 mlU/mL), severe preeclampsia group and its control were (26483±35430 and 12641±9948 mlU/mL), eclampsia group and its control were (67514±55184 and 20751±36477 mIU/mL).There were no significant differences in serum levels of total hCG between patients with mild preeclampsia and those of their macthed controls (p>0,05). Patients with preeclampsia and eclampsia had significantly different serum levels of total hCG when compared to their control group (p<0,05). There were not any significant correlation between the serum total hCG levels and any other parameter studied (free 3-hCG, systolic and diastolic blood pressure, uric acid, creatinine) in the preeclamptic cases. The serum free P-hCG levels in the mild preeclampsia group and its control were (21,6±28,8 - 17,1±28,3 mlU/mL), severe preeclampsia group and its control were (22,0+40,4 - 6,7+7,5 mlU/mL), eclampsia group and its control were (42,9±43,5 - 16,0+39,7 mlU/mL). There were no significant differences in serum levels of free P-hCG between patients with mild preeclampsia and VIthose of their macthed controls (p>0,05). Patients with severe preeclampsia and eclampsia had significantly different serum levels of free P-hCG when compared to their control groups (p<0,05). There were not any significant correlation between the serum free P-hCG levels and any other parameter studied (total P-hCG, systolic and diastolic blood pressure, uric acid, creatinine) in the preeclamptic cases. There were not any significant differences in serum free P-hCG levels among mild, severe preeclamptic and eclamptic groups (p>0,05). There was not any significant difference in serum total hCG levels between mild and severe preeclamptic groups (p>0,05), but we observed significant difference between severe preeclamptic and eclamptic groups (p<0,05) Conclusion: In our study, serum total hCG and free P-hCG levels in both severe preeclamptic and eclamptic groups were observed to be statistically higher than their control groups. This indicates that serum total hCG and free P-hCG levels could be supportive markers in the diagnosis of severe preeclampsia and eclampsia. On the other hand, there was not any significant difference between the mild preeclamptic group and its control in term of total hCG or free P-hCG levels (p>0,05). Thus increased serum total hCG and free P-hCG levels might indicate the severity of the disease but they can not be used, as predictive markers or determinant of preeclampsia in general sense.
