Vitiligo hastalığında topikal kalsipotriol darbant UVB kombinasyon tedavisi ile darbant UVB tedavisinin etkinliğinin karşılaştırılması

Abstract

ÖZET Darbant UVB (311 nm) fototerapisi vitiligo tedavisinde başarı ile kullanılmaktadır. Bir vitamin D3 analoğu olan kalsipotriol, hücre farklılaşması ve proliferasyonu üzerine, immünolojik ve inflamatuar mediatörler aracılığıyla etkili olmaktadır. Bu çalışmada darbant UVB-kalsipotriol kombine tedavisi ile tek başına darbant UVB tedavisinin klinik etkinliklerini karşılaştırmak istedik. Jenaralize vitiligolu 24 hastaya haftada üç kez darbant UVB tedavisi verilirken vücutlarının sağ yarısına topikal kalsipotriol krem tedavisi uygulandı. Tedaviye 6 ay devam edildi. Tedavi etkinliği tedavi başlangıcında ve 3 aylık peryotlarla çekilen fotoğraflarla değerlendirildi. Darbant UVB-kalsipotriol kombine tedavisi alan grup ve yalnız darbant UVB tedavisi alan gruptaki cevap oranları sırasıyla, ortalama %51.0±19.6 ve %39.0±18.9 idi. Kombine tedavi uygulanan tarafta, repimentasyonun başladığı total doz ortanca 6345.0 (2930.0- 30980.0) mj/cm2, repigmentasyonun başladığı seans sayısı ortanca 18.0 (12.0-67.0) idi. Yalnız darbant UVB uygulanan tarafta, repigmentasyonu başlatan total doz ortanca 8867.5 (2500.0-30980.0) mj/cm2, repigmentasyonun başladığı seans sayısı ortanca 24.0 (15.0-67.0) olarak bulundu. Bizim sonuçlarımız vitiligo tedavisinde topikal kalsipotriolün darbant UVB tedavisinin etkinliğini. artırdığım göstermiştir. Kombine tedavi ile daha düşük UVB dozu ile daha kısa sürede pigmentasyona ulaşılmıştır. Böylece hastaların tedavi süresi, maliyeti ve UVB 'e bağlı gelişebilecek yan etkilerin azaltılabileceği düşünülmektedir. Anahtar kelimeler: Vitiligo, darbant UVB, topikal kalsipotriol. IV

ABSTRACT Background: The use of narrowband UVB phototherapy has been used successfully for the treatment of vitiligo. Calcipotriol has affects on cellular proliferation, differation, and immunomodulatory and inflammatory mediators. Objective: In the present study, we want to compare the clinical efficacy of the narrowband UVB treatment with the combine treatment of narrowband UVB- calcipotriol. Methods: Narrowband UVB treatment was given to 24 patient with genaralize vitiligo three times a week, topical calcipotriol cream was applied to right side of their body. Treatment was continued for six mounth. Treatment efficacy was evaluated at the begining of the treatment and at each three mounths by photographing. Results: The average response rates in lesions with the combination of narrowband UVB and calcipotriol and with only narrowband UVB were 51.0±19.6% and 39.0±18.9%, respectively. The average cumulative UVB dose and number of UVB exposures for initial repigmentation were 6345.0 (range; 2930.0-30980.0) and 18.0 (range; 12.0-67.0) on the combined treatment side, and 8867.5 (range; 2500.0-30980.0) and 24.0 (range; 15.0-67.0) on the only narrowband UVB, respectively. Conclusions: Our results have shown that concurrent topical calcipotriol potentiates the efficacy of narrowband UVB in the treatment of vitiligo, and that this combination achieves earlier pigmentation with a lower total UVB dosage. Therfore treatment period, cost and adverse effects due to UVB may be decreased. Key words: Vitiligo, narrowband UVB, topical calcipotriol. V
