Fonksiyonel endoskopik sinüs cerrahisi sonrası submukozal trombositten zengin plazma enjeksiyonunun yara iyileşmesi üzerine etkilerinin değerlendirilmesi

Abstract

Amaç: Endoskopik sinüs cerrahisi sonrası submukozal trombositten zengin plazma (TZP, Platelet Rich Plasma- PRP) enjeksiyonunun yara iyileşmesi üzerine etkilerini araştırmak.Gereç ve yöntem: 24 adet Yeni Zelanda beyaz tavşanında 32 adet burun kullanılarak sayıları 8'li 4 grup oluşturuldu. 3 gruba endonazal cerrahi uygulama yapıldı. Endoskopik olarak ventral konka mukozalarında 3 mm'lik punch forseps ile hasar oluşturuldu. Birinci gruba cerrahi sonrası submukozal PRP (PRP grubu), ikinci gruba submukozal serum fizyolojik (SF) enjekte edildi (SF grubu), üçüncü gruba herhangi bir enjeksiyon yapılmadı (hasarlı kontrol grubu). Dördüncü gruba ise herhangi bir cerrahi uygulanmadı ve normal kontrol grubu olarak tayin edildi. Cerrahi uygulamadan 14 gün sonra denekler sakrifiye edildi ve toplam 4 grubun mukozalarında histopatolojik inceleme ve yaş dokuda hidroksipirolin ölçümleri yapıldı.Bulgular: Epitelyal kalınlık, enflamatuar hücre sayısı, siliyer hücre sayısı ve kollajen yoğunluğu parametreleri; PRP grubunda, SF grubu ve hasarlı kontrol grubuna oranla istatistiksel olarak anlamlı düzeyde düşük saptandı. Goblet hücre sayımı tüm gruplarda normal kontrol grubuna göre anlamlı düzeyde artmış idi. Subepitelyal fibrozis ve yaş dokuda hidroksipirolin ölçümleri açısından gruplar arası anlamlı fark saptanmadı.Tartışma ve Sonuç: Literatürde yara iyileşmesi üzerine olumlu etkileri insan ve hayvan çalışmalarıyla gösterilmiş olan PRP'nin, bu çalışma ile nazal mukozada da iyileşme üzerine pozitif etkileri olduğu görülmüştür. Çalışmamız PRP'nin hasarlanmış nazal mukoza üzerinde antienflamatuar özelliği olduğunu, epitelyal bütünlük ve siliyer fonksiyonların korunmasında, kollajen yoğunluğunu azaltarak sineşi oluşumunun azaltılmasında etkili olduğunu göstermektedir. PRP'nin endonazal cerrahi girişimlerden sonra submukozal enjeksiyonunun nazal fizyolojinin korunması açısından etkili bir uygulama olduğu düşünülmektedir.

Purpose: To investigate the effects of submucosal platelet-rich plasma (PRP) injection on nasal mucousal wound healing after endoscopic sinus surgery.Materials and Methods: 32 noses of 24 New Zealand white rabbits were divided into four groups, each group had 8 noses. In the first three groups, damage was created by punch forceps on ventral turbinate mucousa. Submucousal PRP was injected (PRP group, n=8) at the damaged mucousa in the first group, where %0,9 saline was injected (saline group, n=8) in the second group and no injection in the third one (damaged control group, n=8). No surgical operation was performed in the fourth group (normal control group, n=8). Subjects have been sacrificed 14 days after surgery; histopathological examination and hydroxypiroline measurements were performed on the mucousa of 4 groups.Results: Epithelial thickness, inflammatory cell count, ciliary cell count and collagen density parameters were found to be significantly lower in the PRP group than both the saline group and the damaged control group. Goblet cell count was significantly increased in all groups compared to normal control group. There were no statıstıcally sıgnificant difference of subepithelial fibrosis and hydroxyproline levels between all groups.Conclusions: There are many clinical and experimental studies which shows positive effects of PRP on wound healing in the literature. We have seen the positive effects in the nasal mucousa. According to our results, PRP injection at the damaged nasal mucosa; has been shown to be effective in preserving epithelial integrity and ciliary functions, reducing the formation of synechia by decreasing collagen density. It is shown that submucosal injection of PRP after endonasal surgical procedures is an effective application for maintaining nasal physiology.
