dc.description.abstractBu çalışmada, total abdominal histerektomi cerrahisi geçirecek hastalara preemptif olarak uygulanan intravenöz (i.v.) parasetamolün postoperatif ağrı üzerine etkisini araştırmayı amaçladık.Randomize ve çift kör olarak yapılan çalışmamıza, genel anestezi altında elektif total abdominal histerektomi cerrahisi planlanan ASA I-II grubu 18-75 yaş arası, 50 hasta dahil edildi. İndüksiyondan 15 dakika önce Grup I (n=25)'deki olgulara 1 gr. (100 cc) i.v. parasetamol (perfalgan), Grup II (n=25)'deki olgulara 100 cc serum fizyolojik verildi. Her iki gruptaki hastalara postoperatif 0. dakikada, 4 mg. yükleme dozu, bazal infüzyon yok, bolus dozu 1 mg., kilit süresi 5 dakika, 4 saatlik limit 40 mg. olacak şekilde i.v. morfin HKA başlandı.Postoperatif 24 saat izlenen hastaların; 30., 60. dakikalarda ve 2., 4., 6., 12., 24. saatlerdeki Visual Analog Scale (VAS) ve Verbal Rating Scale (VRS) değerleri, tüm hastaların 30. dk., 60. dakika ve 2., 4., 6., 12., 24. saatlerde toplam morfin kullanım dozları, HKA istek sayısı, ilk morfin istem zamanı (Efektif Analjezi Süresi) kaydedildi. 0-6 saat ve 6-24 saatleri arasında bulantı- kusma skorları kaydedildi.Parasetamol grubunda hareket ve dinlenme halindeki ağrı skorları ve 24 saatlik toplam morfin tüketimi anlamlı olarak düşük bulundu (p<0,05). Efektif Analjezi Süresi parasetamol grubunda daha uzun bulundu (p<0,05).Sonuç olarak; Total abdominal histerektomi operasyonu olan olgularda uygulanan preemptif 1 gr. i.v. parasetamolün ameliyat sonrası dö¬nemde etkin bir analjezi sağladığı ve ameliyat sonrası morfin tüketimini azalttığı saptandı.Anahtar sözcükler: preemtif analjezi, intravenöz parasetamol, postoperatif ağrı, TAH
dc.description.abstractIn this study, we aim to investigate the effect of preemptive intravenous (i.v.) paracetamol on postoperative pain in patients undergoing total abdominal hysterectomy surgery .A total of 50 patients between the ages of 18- 75 years were included in the research done using randomized and double blind study methods. The patients, who were divided into ASA I-II groups,underwent elective total abdominal hysterectomy under general anesthesia . 15 minutes before induction,patients in Group I (n=25) were given 1 gr. (100 cc) i.v. paracetamol (perfalgan) while those in Group II (n=25) were given 100 cc serum physiologic . In both groups during the postoperative period, 4 mg. loading dose at 0 minutes, no basal infusion, then a bolus dose of 1 mg., a lock out period of 5 minutes, and finally a 4 hour limit 40 mg. of i.v. morphine HKA was started .During the next 24 hours the patients were followed up and the following parameters were recorded: Visual Analogue Scale(VAS) and Verbal Rating Scale (VRS) values after 30., 60. minutes and after 2., 4., 6., 12., 24. hours, the total amount of morphine doses used by all the patients after 30.minutes., 60. minutes and after 2., 4., 6., 12., 24. hours, the number of HKA requests, and the time when morphine was first requested (Effective Analgesia Time). Nausea - Vomiting scores between 0-6 hours and 6-24 hours were also recorded .In the Paracetamol group, both the pain scores at rest / during motion and the 24 hour morphine consumption were found to be significantly lower (p<0,05) .The Effective Analgesia Time in the paracetamol group was found to be longer (p<0,05).As a result; we can conclude that the preemptive 1g. i.v. paracetamol given to patients undergoing total abdominal hysterectomy provides an effective postoperative analgesic effect and also reduces the amount of morphine consumption.Key words: preemptive analgesia, intravenous paracetamol, postoperative pain, TAHen_US
