Kamuda çağdaş yaklaşımlar temelinde sağlık sektöründe dönüşüm örneği olarak anne dostu hastane İslahiye Devlet Hastanesi

Abstract

Sosyal devlet uygulamalarının gelişimine paralel olarak devletin aşırı büyümesi, artan merkezileşme, kamu harcamalarının artması, liberal-kapitalist ekonomilerde yaşanan krizlerin yansıması olan ağır borçlar sonucunda devlette büyüklükten ziyade etkinliği esas alan yeni anlayışların ortaya çıkmasını zorunlu kılmıştır. Devletler bu yönetimsel anlayış devinimiyle hizmet sunan yapıdan, düzenleyici, denetleyici ve yol gösterici yapıya evrilerek piyasa modeli temelli kamu sektörü oluşturulmaya çalışılmıştır. Bu mantıkla, devletteki verimsiz ve hantal düşünülen yapı, para kazanma amacını ön planda tutan ve bu amaçla başarılı planlamalar geliştiren özel sektör uygulamalarının kamu sektörüne uyarlanmasıyla yeni kamu yönetimi ortaya çıkmıştır. Sağlık hizmetleri, toplumu oluşturan bireyleri çeşitli hastalık ve sakatlıklardan koruyarak, ruhsal, iktisadi ve sosyal bağlamda yaşamlarını huzur içerisinde sürdürebilmelerini temin edebilme amaçlı faaliyetler bütünüdür. Özellikle bu temelde sağlık sektöründeki yenilenme son yıllarda gözle görülür bir şekilde kendini göstermiştir. Bu bağlamda fiziki ve teknolojik yenilenme ve gelişmenin yanı sıra hizmet sunmada yeni metod ve yaklaşımların geliştirilmesi ve uygulanması da son derece önemlidir. Kamu sağlık sektöründeki bu gelişmeler öncelikle merkezden taşra teşkilatlarına doğru evrilmiştir. Ve günümüzde taşra teşkilatlarının en önemli noktasını oluşturan ilçe hastaneleri bu çağdaş yaklaşımların uygulayıcısı olma yolunda pek çok uygulamayı devreye sokmaktadır. Buna örnek olarak daha iyi ve kaliteli hizmet sunma açısından çağdaş yaklaşım örneği olarak `Anne dostu hastane` unvanını almak için İslâhiye Devlet Hastanesinin geçirdiği yapısal, fiziksel, sosyal ve mental değişiklikleri ve bunların yapılma sebebi olan hasta ile özelde anne adaylarının düşünce, eleştiri ve yeni önerilerini irdeleyeceğiz. Bu amaçla hastane yönetimini buna iten nedenlerin anlaşılması için öncelikle yönetim ve daha sonra ilgili sağlık çalışanları ile anketler gerçekleştirildi. Daha sonra bu unvanın alınması için izlenen hukuki prosedür ortaya konularak, hastane yönetiminin kurum içinde gerçekleştirdiği değişiklikler irdelendi. Sağlık çalışanları ile yapılan anketler aracılığı ile bu işleme karşı düşünceleri ve kendilerine düşen görevler ortaya konuldu. En son olarak hizmet sunmada hedef olan anne adaylarının bu unvanın alınmadan önce ve sonraki hastane koşulları ve sağlık çalışanlarının kendilerine yaklaşımlarının karşılaştırılması için anket ve bire bir görüşmeler yapılarak sonuçlar ortaya konuldu. The emergence of new approaches based on governmental efficiency rather than size have become necessary due to the excessive growth of the state, increasing centralization, increasing public expenditures, and heavy debts resulting from the crises in liberal-capitalist economies in parallel with the development of social state practices. With this change of administrative understanding, states have evolved from the structure that serves to the regulatory, supervisory and guiding structure, and efforts have been made to form a public sector based on a market model. In this way, the new public administration has emerged with the adaptation of the private sector practices, which prioritize the purpose of earning money and developing successful plans for this purpose, to the public sector that was previously considered to be inefficient and cumbersome. Health services are a set of activities aimed at protecting individuals, who comprise society, from various diseases and disabilities in order to ensure that they can live their lives peacefully in spiritual, economic and social terms. So, the renewal in the health sector has been visible in recent years. In addition to physical and technological renewal and development, in this context, it is extremely important to develop and implement new methods and approaches in service delivery. These developments in the public health sector have expanded primarily from central to provincial organizations. Today, district hospitals, which constitute the most important part of provincial organizations, are implementing gradually more practices based on these contemporary approaches. As an example of the contemporary approach in terms of providing better and better service, the current study will examine the structural, physical, social and mental changes which İslâhiye State Hospital has undergone for the purpose of obtaining the title of `Mother-friendly hospital` and the thoughts, criticisms and new suggestions of patients in general and expectant mothers in particular, who are the reason for making these changes. For this purpose, questionnaires were conducted first with the management and then with the related health workers in order to understand the reasons that forced the hospital management to this. Then, the legal procedure for obtaining this title was explained and the changes made by the hospital management within the institution were examined. Through the questionnaires conducted with the health care workers, their thoughts about this process and their duties in this regard were revealed. Finally, a questionnaire and one-to-one interviews were conducted to compare the hospital conditions and the approach of the health care workers to the expectant mothers before and after obtaining this title, and results were stated.Keywords: İslâhiye State Hospital, Mother-Friendly Hospital, Health Transformation, Contemporary Governmental Approaches
