Otoskleroz cerrahisinde kullanılan teflon piston protezlerinin çaplarının işitme sonuçları üzerine etkisi: otoakustik emisyonlar ile değerlendirme

Abstract

Bu çalısmanın amacı, otoskleroz cerrahisinde kullanılan farklı çaplardaki teflon pistonprotezlerinin isitme sonuçlarına olan etkilerini otoakustik emisyonlar (OAE) ile incelemektir.2002-2005 yılları arasında stapedotomi yapılan 0.3mm (grup A) ve 0.6mm (grup B)çaplı teflon piston yerlestirilen toplam 20 otoskleroz olgusu ve saglıklı bireylerden olusan 10olguluk kontrol grubu (grup K) retrospektif olarak incelendi. Hastalara OAE ile ölçümleryapıldı. Geçici uyarılmıs OAE (TEOAE), distorsiyon ürünleri OAE (DPOAE) ve DPOAEinput/output ölçümlerinin sonuçları, her iki grup ve kontrol gurubu ile istatistiksel olarakkarsılastırıldı.TEOAE amplitüdleri grup B'de, grup A'ya göre emisyon amplitüd oranlarının 2000 ve3000 Hz'lerde p<0.001 olarak ve 4000-5000 Hz'lerde p<0.05 daha iyi elde edildi. GrupB'deki DPOAE amplitüdleri ise grup A'daki emisyon amplitüdlerine oranla 3000 ve 4000Hz'lerde p<0.05 daha iyi olarak tespit edildi, 1000, 2000 ve 6000 Hz'lerde ise p>0.05 olaraktespit edildi. Grup B'deki DPOAE I/O amplitüdleri ise grup A'daki emisyon amplitüdlerineoranla 2000 ve 3000 Hz'lerde 65, 60 ve 45 dB'de p<0.05 daha anlamlı tespit edildi. 1000,4000, 6000 Hz'lerde ise p>0.05 idi. Bu sonuçlar grup B oto akustik emisyon sonuçlarınıngrup A'dan daha iyi oldugunu göstermektedir.Sonuç olarak otoskleroz cerrahisinde, postoperatif geç dönemde genis çaplı teflonpiston protezleri, isitme sonuçlarını daha iyi etkilemektedir. Teflon piston çaplarının artısı ilekoklear emsiyon yanıtlarda da artıslar olmaktadır.

The aim of this study is to evaluate hearing results after stapes surgery by 0.3 mm and0.6 mm Teflon pistons prosthesis with otoacoustic emissions (OAE).Retrospective analysis studied 10 patients selected who underwent stapedotomy forotosclerosis with insertion of 0.3 mm (group A) Teflon piston prosthesis, 10 patients with 0.6mm (group B) Teflon piston prosthesis and 10 patients with normal control group (group K)between 2002 and 2005 years. The DPOAE amplitudes of group B was foundbetter than group A as p<0.05 at 3000 and 4000 Hz and p>0.05 at 1000, 2000 and 6000 Hz.The DPOAE input/output amplitudes of group B were found better than group A as p<0.05 at2000 and 3000 Hz with 65dB, 60 dB and 45 dB and p>0.05 at 1000, 4000 and 6000 Hz.The hearing outcomes would be better as the diameter of Teflon piston prosthesis werewide at the late postoperative period. The increasing in the diameter of Teflon pistonprosthesis would be also cause of increasing on cochlear emission responses.
