KOAH tanısı alan hastalara yapılan fan uygulamasının solunum fonksiyon testleri, dispne ve yorgunluğa etkisi

Abstract

Çalışma KOAH hastalarına uygulanan fan'ın solunum fonksiyon testleri, dispne ve yorgunluk düzeyine etkisini incelemek amacıyla yapıldı. Araştırmaya başlamadan önce gerekli tüm izinler alındı. Çalışma kriterlerine uygun olan hastalar müdahale (67) ve kontrol (64) grubuna ayrıldı. Hastaların solunum fonksiyon testleri için; zorlu vital kapasite (FVC), birinci saniyedeki zorlu ekspiratuvar volüm (FEV1) ve FEV1/FVC değerleri kullanıldı. Yorgunluk düzeyi; KOAH ve Astım Yorgunluk Ölçeği (KAYÖ), dispne düzeyi ise Medical Research Council Scale (MRCS) ve Visual Analog Scale (VAS) ile değerlendirildi. İlk görüşmede formların doldurulmasından ve solunum fonksiyon testlerinin kaydedilmesinden sonra, müdahale grubundaki hastalara fan kullanımı hakkında yüz yüze görüşme tekniği ile bireysel eğitim verildi ve haftalık telefon görüşmeleri ile hastaların evde fan kullanımına devam edip etmediği sekiz hafta boyunca kontrol edildi. Kontrol grubuna herhangi bir uygulama yapılmadı. Hem müdahale hem de kontrol grubuna; ilk değerlendirmeden sekiz hafta sonra KOAH ve Astım Yorgunluk Ölçeği, MRCS ve VAS yeniden uygulandı ve solunum fonksiyon testleri tekrarlanarak sonuçlar kaydedildi. Verilerin değerlendirilmesinde; Shaphiro Wilk, Student t, Mann whitney U, Willcoxen, Ki-kare, Mc Nemar, Kappa testleri kullanıldı. Çalışmanın sonunda; KOAH hastalarının `orta/şiddetli` düzeyde dispne ve yorgunluk yaşadığı, müdahale grubunun fan uygulaması sonrası dispne ve yorgunluk şiddetinin azaldığı, FVC, FEV1 ve FEV1/FVC düzeylerinin belirgin olarak arttığı görüldü. Bu doğrultuda KOAH tanısı alan hastalarda dispne ve yorgunluk düzeyinin azaltılması, solunum fonksiyon parametrelerinin olumlu yönde geliştirilmesi için hastalara fan uygulaması konusunda eğitim verilmesi önerilebilir.Anahtar kelimeler: KOAH, fan uygulaması, dispne, yorgunluk, solunum fonksiyon testleri, hemşirelik

The study was conducted to investigate the effect of the fan on pulmonary function tests, dyspnea and fatigue levels in COPD patients. All necessary permits were obtained before the investigation began. Patients who met the study criteria were divided into the intervention group (67) and control group (64). For respiratory function tests of patients; forced vital capacity (FVC), forced expiratory volume at first (FEV1) and FEV1 / FVC values were used. Fatigue level; COPD and Asthmatic Fatigue Scale (CAFS) and dyspnea were assessed by the Medical Research Council Scale (MRCS) and the Visual Analogue Scale (VAS). After filling out the forms at the first visit and recording the pulmonary function tests, individualized training was conducted with face-to-face interview techniques on the use of the patient's fan in the intervention group and checked for eight weeks during weekly phone calls and when patients continued to use the fan at home. No control was applied to the control group. Both intervention and control groups; Eight weeks after the initial evaluation, CAFS, MRCS, and VAS were reapplied and pulmonary function tests were repeated and results were recorded. The evaluation of the data; Shaphiro Wilk, Student t, Mann Whitney U, Willcoxen, Chi-square, Mc Nemar, Kappa test were used. At the end of the study; It was observed that the COPD patients had 'moderate and severe' level of dyspnea and fatigue, the intervention group had decreased dyspnea and fatigue severity after fan application, FVC, FEV1 and FEV1 / FVC levels were significantly increased. In this respect, the COPD patients with dyspnea and fatigue-reducing the level of pulmonary function parameters in a positive direction for the development of training about the fan application to patients may be recommended.Keywords: COPD, dyspnea, fatigue, fan application, pulmonary function tests, nursing
