dc.description.abstractBu çalışmanın amacı kardiyopulmoner bypass ile koroner arter bypass cerrahisi uygulanan hastalarda preoperatif dönemde kullanılan angiotensin dönüştürücü enzim inhibitörü ilaçların, postoperatif renal fonksiyonlar üzerine etkilerini araştırmaktır. Kliniğimizde izole koroner arter bypass cerrahisi uygulanan 139 hasta (110 erkek, 29 kadın) çalışmaya alındı. Hastalar preoperatif dönemde angiotensin dönüştürücü enzim inhibitörü kullanmayanlar (grup 1) ve kullananlar (grup 2) olarak iki gruba ayrıldı. Kreatinin değerleri üzerinden veri toplandı. Veriler preoperatif, postoperatif 0., 1., 2., 4. ve 7. günlerde toplanarak kaydedildi. İki grup arasında preoperatif kreatinin değerleri açısından istatistiksel farklılık saptanmadı (p>0,05). Ortalama değerler göz önüne alındığında grup 1 de kreatinin değerleri preoperatif değerden başlayarak; postoperatif 0, postoperatif 1 ve postoperatif 2 de artmış, postoperatif 4 ve postoperatif 7 de önceki değere kıyasla düşmüş olarak bulundu. Yapılan istatistiksel çalışmada preoperatif değerine kıyasla mevcut olan kreatinin yüksekliği postoperatif 1 (p<0,001), postoperatif 2 (p<0,01) ve postoperatif 4 te (p<0,01) istatistiksel olarak anlamlı bulundu, ancak postoperatif 0 ve postoperatif 7 deki yükseklik istatistiksel olarak anlamlı farklılık oluşturmadı (p>0,05). Bu verilere ek olarak postoperatif 0 ile postoperatif 1 arasındaki yükseklik istatistiksel olarak anlamlı bulundu (p<0,05). Grup 2 de ise kreatinin değerleri preoperatif değerden başlayarak; postoperatif 0 ve postoperatif 1 de artmış, postoperatif 2, postoperatif 4 ve postoperatif 7 de önceki değere kıyasla düşmüş olarak bulundu. Yapılan istatistiksel çalışmada preoperatif değerine kıyasla mevcut olan kreatinin yüksekliği postoperatif 1 (p<0,001) ve postoperatif 2 de (p<0,01) istatistiksel olarak anlamlı bulundu, ancak postoperatif 0, postoperatif 4 ve postoperatif 7 deki yükseklik istatistiksel olarak anlamlı farklılık oluşturmadı (p>0,05).Bu çalışmanın sonucunda preoperatif angiotensin dönüştürücü enzim inhibitörü ilaçların kullanımının kardiyopulmoner bypass sonrası renal hasara neden olmadığı, aksine perfüzyon sonrası yüksek seyreden periferik vasküler rezistans'ın düşmesi yönünde etki ederek, renal fonksiyonların daha erken düzelmesini sağladığı gösterilmiştir.
dc.description.abstractThe purpose of this study is to search the postoperative effects of the preoperative use of angiotensin converting enzyme inhibitors on renal functions in patients undergoing CABG with cardiopulmonary bypass. 139 patients (110 male, 29 female) who underwent isolated coronary artery bypass in our clinic were enrolled to the study. The patients were separated into two groups as the patients who hadn?t used angiotensin converting enzyme preoperatively (group 1) and the ones who had used it (group 2). Collection of data was according to the creatinine levels. Data were collected and recorded preoperatively, and on the 0, 1st, 2nd, 4th and 7th day postoperatively. There was no statistically significant difference between the two groups in terms of creatinine levels (p>0,05). When the mean levels were considered, in the group that hadn?t used angiotensin converting enzyme inhibitor, compared to the preoperative levels, the creatinine levels were increased on the postoperative 0, postoperative 1st and postoperative 2nd days and decreased on the postoperative 4th and postoperative 7th days compared to the earlier days. In the statistical study, the increase in the creatinine levels on the postoperative 1st (p<0,001), postoperative 2nd (p<0,01) and postoperative 4th (p<0,01) days compared to the preoperative levels were found to be statistically significant while the increase on the postoperative 0 and postoperative 7th days were not statistically significant (p>0,05). In additin to this data, the increase between postoperative 0 and postoperative 1st day was found to be statistically significant (p<0,05). In the group that had used angiotensin converting enzyme inhibitor, compared to the preoperative levels, the creatinine levels were increased on the postoperative 0 and postoperative 1st days and decreased on the postoperative 2nd, postoperative 4th and postoperative 7th days compared to the earlier days. In the statistical study, the increase in the creatinine levels on the postoperative 1st (p<0,001)and postoperative 2nd (p<0,01) days compared to the preoperative levels were found to be statistically significant while the increase on the postoperative 0, postoperative 4th and postoperative 7th days were not statistically significant (p>0,05).The results of this study have showed that the preoperative use of angiotensin converting enzyme inhibitor drugs are not associated with the renal injury after cardiopulmonary bypass; on the contrary, they provide earlier improvement of renal functions by effecting to decrease the high peripheral vascular resistance after cardiopulmonary bypass.en_US
