dc.description.abstractLenke tip 1A Adolesan İdiopatik Skolyozda Aksiyel Plan Analizi ve Aksiyel Analizin Distal Füzyon Seviyesine KatkısıGiriş ve Amaç: Lenke 1A grubu eğrilikler, en sık rastlanan adolesan idiopatik skolyoz grubu olmasına karşın, optimal distal füzyon seviyesi tartışmalıdır. Çalışmamızın amacı Lenke 1A tipi adolesan idiopatik skolyozu olan olgularımızı aksiyel plan analizine göre sınıflayarak optimal distal füzyon seviyesinin belirlenmesidir.Gereç ve Yöntem: Cobb açısı 41-65° arasında değişen ve Lenke tip 1A skolyozu olan 69 (56 kız,13 erkek) hasta çalışmaya alındı. Standart ayakta ön-arka ve lateral omurga radyografilerinden koronal, sagital ve aksiyel parametreler ölçüldü. Aksiyel plan analizi Drerup metodu ile yapıldı. Aksiyel plan analizi sonucunda Lenke 1A olgular, lomber vertebra rotasyonuna göre 3 farklı gruba ayrıldı.Bulgular: Grup I olgularda, alt end vertebranın distalindeki lomber vertebralarda nötral rotasyon bulunur. Grup II olgularda, lomber vertebralarda ana torakal eğrilikle aynı yönde rotasyon bulunur. Grup III olgularda lomber vertebralarda ana torakal eğrilikle karşı yönde rotasyon bulunur. Major torasik eğriliğin ortalama cobb değeri 46,9° (41-65)'du. Apikal vertebra rotasyonu ortalama -23,4±4,1° olarak ölçüldü.Sonuç: Aksiyel plan analizi, Lenke 1A eğriliklerde distal füzyon seviyesinin belirlenmesinde güvenilir bir yöntemdir. Uygun distal füzyon seviyesi Grup I hastalarda nötral vertebra, Grup II hastalarda nötral vertebra -1 ve Grup III hastalarda stabil vertebra olabilir.Anahtar sözcükler: Lenke 1A; Aksiyel plan analizi; Vertebra rotasyonu; nötral vertebra; Drerup metodu
dc.description.abstractAxial Plane Analysis of Lenke 1A Adolescent Idiopathic Scoliosis and Contribution to the Distal Fusion Level of Axial AnalysisIntroduction and purpose: Lenke 1A scoliosis is the most common type of adolescent idiopathic scoliosis (AIS) however the optimum selection of distal fusion level has been area of controversy. The purpose of this study is to classify Lenke 1A curves according to lumbar axial plane analyses to determine optimal distal fusion level selection.Materials and methods: Sixty nine patients (56 women, 13 men) who had Lenke Type 1A curves with Cobb angles between 41° and 65° were included in the present study. Coronal, sagital, and axial parameters were measured from standard standing anteroposterior and lateral whole-spine radiographs. Axial plane analysis was performed with the method described by Drerup. In critically analyzing Lenke 1A curves in the axial plane, patients were divided into three groups depending on lumbar vertebral rotation.Results: In Group I, the rotation of lumbar vertebral rotation was accepted as neutral. In Group II, the direction of lumbar vertebral rotation was same with other vertebrae in the main curve. In Group III, the rotation of lomber vertebral rotation had opposite direction with vertebrae in the main curve. The mean thoracic Cobb angle of patients with Lenke1A idiopathic scoliosis was 46,9° (range 41°–65°). The mean apical vertebral rotation(AVR) angle of Lenke 1A idiopathic curves was -23,4±4,1°Conclusions: It seems that axial plane analyses might be a reliable method to select ideal distal fusion level in Lenke type 1A curves. In group 1 Neutral Vertebra(NV), in group 2 NV-1, in group 3 stable vertebra might be the appropriate distal fusion level.Keywords: Lenke 1A; Axial plane analysis; Vertebra rotation, Neutral vertebra, Drerup's method.en_US
