dc.description.abstractAmaç : Çalışmamızın amacı iskemik inme hastalarının erken radyolojik işaret ve bulgu izlenmeyen kontrastız kranial bilgisayarlı tomografi görüntülerinin , radiomics parametreleri üzerinden texture – doku analizi ile değerlendirilerek ; iskemik inme tanısına katkıda bulanabilecek kantitatif veriler elde etmektirGereç ve Yöntem : Çalışmamızda ocak 2012- kasım 2018 arasında acil servise inme kliniği ile başvuran hastaların , difüzyon MR ve kontrastsız BT tetkikleri retrospektif olarak taranmıştır. MR tetkikinde akut iskemik infarkt ile uyumlu difüzyon kısıtlılığı bulunan ve Kontrastsız BT tetkikinde radyologlarca erken radyolojik bulgu ve işaret saptanmayan 54-96 yaş aralığında , 19'u kadın 17'si erkek hasta olmak üzere toplam 37 hasta çalışmaya dahil edilmiştir. BT görüntülerinde infarkt alanı ve karşı hemisferdeki simetriği 1 cm çaplı ROI çemberi ile , difüzyon sekansları ve ADC haritaları baz alınarak manuel işaretlendi. 3DSlicer programına ait radiomics eklentisi ile 106 adet doku analizine ait parametre elde edildi. Daha önceki benzer çalışmalar taranarak ; literatürde sık kullanılan 21 parametre istatistiksel değerlendirme için kaydedildi.Bulgular: Taranan 21 parametre içinde anlamlı fark saptanan parametreler şunlardır; glcmjointavarage, glcmjointenergy, glcmcontast, firstorderuniformity, firstordermedian, firstorderenergy, firstordermax, firstordermean, glrlmrunlengthnonuiformty ve ngtdmcontrast. En anlamlı parametre ngtdm contrast : Karşıtlığı ifade eden gri tonlama seviyelerinin dinamik aralığıyla birlikte , gri seviye değişkenliğinin sıklığını da gösteren parametre olarak bulunmuştur.Sonuç : Kontrastsız BT incelemede inmeye ait erken radyolojik bulgu ve işaret saptanmayan zaman dilimde iken doku analizi ile infarkt alanına ait parametre değişimleri ortaya çıkmaktadır. dc.description.abstractPurpose: The purpose of this study was to evaluate the non-contrasted cranial computerized tomography images of ischemic stroke patients with no early radiological signs and findings with texture-tissue analysis over radiomics parameters; and to obtain quantitative data that may contribute to the diagnosis of ischemic stroke.Materials and Methods: In the present study of ours, the diffusion MRI and non-contrasted CT scans of the patients who were admitted to the Emergency Department with clinical symptoms of stroke between January 2012 and November 2019 were reviewed retrospectively. A total of 37 patients (19 females, 17 males) who were between 54-96 years of age, who had diffusion limitation that was consistent with acute ischemic infarction in MR scans, and who did not have early radiological findings and signs in non-contrasted CT scans were included in the study. In the CT images, the infarct area and itscounterpart in the other hemisphere were marked manually with a 1 cm-diameter ROI circle based on Diffusion Sequences and ADC Maps. A total of 106 tissue analysis parameters were obtained with the radiomics add-on of the 3DSlicer Program. A total of 21 parameters that were commonly used in the literature were recorded for statistical evaluation by reviewing similar studies in the literature.Findings: The following significant differences were found in 21 parameters that were scanned; glcmjointavarage, glcmjointenergy, glcmcontrast, firstorderuniformity, firstordermedian, firstorderenergy, firstordermax, firstordermean, glrlmrunlengthnonuiformty and ngtdmcontrast. The most significant parameter was ngtdmcontrast: It was found as the parameter, which showed the frequency of gray level variability together with the dynamic range of grayscale levels that showedthe contrast.Result:In the non-contrasted CT examination, the changes in the parameters of the infarct area appeared in the tissue analysis in the time period when no early radiological findings and signs of stroke were detected.
