dc.description.abstractBu çalışmadaki amacımız klopidogrel metabolizmasında etkili olan CYP2C9 ve CYP2C19 genlerin CNV'leri ile klopidogrel direnç mekanizması arasındaki ilişkiyi incelemektir.Araştırmaya elektif veya acil stent implantasyonlu PKG yapılan 176 hasta dahil edildi. 600 mg klopidogrel yükleme dozu alan ve en az 5 gün süreyle standart doz (75 mg/gün) klopidogrel kullanan hastalardan alınan kan örneklerinde PFA-100 P2Y12 kiti ile KZ ölçüldü. KZ değeri ˃106 sn olan hastalar klopidogrel dirençli, ≤106 sn ise klopidogrele dirençsiz olarak gruplandırıldı. Hastaların DNA'ları Purelink Genomic DNA izolasyon kiti kullanılarak dondurulmuş tam kandan izole edildi. CYP2C9 ve CYP2C19 genlerin CNV'leri TaqMan kopya sayısı analiz kiti kullanılarak RT-PCR ile tespit edildi.Çalışmamızda %55,11 bireyde klopidogrel direnci yok iken, %44,89 bireyde ise klopidogrel direnci olduğunu saptadık. Bu çalışmada, CYP2C9 (Hs0518754), CYP2C9 (Hs05165291), CYP2C9 (Hs05165291), CYP2C19 (Hs02932336) ve CYP2C19 (Hs05148033) CNV ile klopidogrel direnci arasında istatistiksel olarak anlamlı bir fark saptanmadı (p˃0.05). CYP2C19 (Hs05107177-intron 8) iki kopyaya sahip bireylerin bir ve iki kopyaya sahip bireylere göre sırası ile 4.91 ve 3,57 kat daha fazla klopidogrel ilaç direnci geliştirme riskine sahip olduğu belirlendi. Sonuçlarımız, CYP2C19 (Hs05107177- intron 8) CNV'lere sahip bireylerin klopidogrel direnci gelişme riskinin daha yüksek olduğunu göstermektedir.
dc.description.abstractThe aim of this study was to investigate the relationship between clopidogrel resistance mechanism and the CNVs of CYP2C9 and CYP2C19 genes which is effective in clopidogrel metabolism.176 patients who underwent elective or emergency percutaneous coronary intervention with stent implantation were included in our study. CT were measured with PFA-100 P2Y12 in blood samples taken from patients using clopidogrel with standard dose (75 mg/day) for at least 5 days after 600 mg clopidogrel loading dose. The patients were divided into two groups CT values >106 s as resistant to clopidogrel, ≤106 s were nonresistant to clopidogrel. DNA of patients was isolated from the frozen whole blood by a using PureLink Genomic DNA isolation kit. CNVs in CYP2C9 and CYP2C19 genes were detected by using TaqMan copy number assays kit in a RT-PCR.In our study, while clopidogrel resistance was not found in 55, 11% of the patients, 44, 89% of the patients were found clopidogrel resistance. This study, no statistically significant relationship was found between CNVs of CYP2C9 (Hs0518754), CYP2C9 (Hs05165291), CYP2C9 (Hs05165291) CYP2C19 (Hs02932336) and CYP2C19 (Hs05148033) genes and clopidogrel resistance. It was determined that individuals with the two copies of CYP2C19 (Hs05107177) have a 3.165 and 3,57 fold increase risk of developing clopidogrel drug resistance compared to the one and three copy, respectively.Our results suggest that CYP2C19 (Hs05107177) CNVs have higher risk of developing clopidogrel resistance.en_US
