dc.description.abstractElektriksel kardiyoversiyon (EKV) sonrası atriyum fibrilasyonu (AF) yinelemesini ve tromboembolik olayları öngören inflamatuvar ve protrombotik belirteçlerin düzey değişikliklerine ait çalışmaların sonuçları çelişkilidir. Çalışmadaki amacımız EKV başarısının CRP ve protrombotik belirteçlerle olan uzun dönem ilişkisini araştırmak ve neden sonuç ilişkisini ortaya koymaktır. Bu çalışmaya EKV işlemi uygulanan 89 AF'lu hasta ve sinüs ritminde (SR) olan 50 olgu olmak üzere toplam 139 hasta (ortalama yaş ve standart sapmaları sırasıyla 63,33 ± 9.14, 62,50 ± 5.54) dahil edilmiştir. Altmış altı hastada (% 74,1) EKV sonrası SR sağlandı. Yirmi üç olguda başarısız oldu. Sinüs ritmine dönen 36 hastada (%54,5) AF tekrarladı. SR'li olgularda D-dimer, fibrinojen ve antitrombin?3 anlamlı olarak daha düşüktü. EKV işlemi sonrası SR'ne dönmeyen ve altı ay sonrasında halen SR de olan hastalar karşılaştırıldığında CRP medyan değerleri 0,67 mg/dl ve 0,22 mg/dl bulundu (p< 0,05). SR korunan 30 hastada altı ay sonra 0,54 olan bazal medyan değeri 0,22'ye geriledi (p= 0,01). D-dimer medyan değeri EKV öncesi 0,175 mcg/ml iken altıncı ayda 0,290 mcg/ml saptandı (p< 0,05). CRP değerlerinin AF' na dönenlerde daha yüksek olması uzun dönemdeki yinelemelerle ilişkili olabilir. SR korunan olgularda CRP düzeylerindeki azalma AF'nun inflamasyonun nedeni olduğunu destekler niteliktedir. Protrombotik durumun altı ay boyunca SR'de kalan olgularda devam ettiği görülmektedir. Daha ileri çalışmalara gereksinim olmasına rağmen, bizim bulgularımız EKV öncesi CRP ölçümleri ile EKV başarısı hakkında prognostik bilgi sağlanabileceğini düşündürmektedir.Anahtar kelimeler: Elektriksel kardiyoversiyon, inflamasyon, protrombotik durum
dc.description.abstractThere are contradictory datas avaliable on different studies that estimate recurrences and thromboembolic events relation with inflamation and prothrombotic events in atrial fibrillation (AF). Our aim in this study is to investigate EC succees and AF relaps relation with CRP and prothrombotic markers in long term. This study was held with total 139 patients (mean ages and standart deviation are sequentially 63,33 ± 9.14, 62,50 ± 5.54) consist of 89 AF patients and 50 cases as sinus rhythm. In 66 cases (74,1) SR was obtained after EC. In 23 cases EC was failed. AF recurred in 36 cases who is SR post-EC. In control group D-dimer, fibrinogen, antithrombin-3 levels are significiantly lower than AF pre-EC. When we compare EC failed patients with cases SR after six months, CRP median values were found 0,67 mg/dl and 0,22 mg/dl (p<0,05). In 30 cases who saved SR after six months basal median CRP values decreased from 0,54 mg/dl to 0,22 mg/dl (=0,01). When D-dimer value was 0,175 µg/ml pre-EC, it was determined 0,29 µg/ml after six months (p<0,05). CRP values determined higher who returns to AF. So CRP can associates with relaps in long term. In cases saved SR prothrombotic state continues. CRP value decreasing in SR saved patients after six months support that AF is a cause of inflamation.
