dc.description.abstract | ÖZET Hipertiroidi ve osteoporoz toplumda yaygın görülen durumlardır. Hipertiroidi sekonder osteoporoz nedenleri arasında yeralmaktadır. Ancak mekanizması henüz tam olarak aydınlatılmamıştır. Tiroid hormonlanmn osteoporozdan sorumlu olan osteoklast aktivitesini direkt olarak uyarmadığı ancak bir takım aracı mekanizmalar kullandığı düşünülmektedir. Çalışmanın amacı menapoz öncesi dönemdeki hipertiroidili kadınlarda tiroid hormonlarının ve sitokinlerin osteoporoz gelişimi üzerine etkisini araştırmaktı. Çalışmaya menapoz öncesi dönemde olan 22 hipertiroidili (H), 12 subklinik hipertiroidili (SH) ve 10 normal (N) sağlıklı kadın alındı. Hipertiroidi grubunda interlökin-2 (IL-2) reseptör düzeyi 1848±1006 U/ml SH grubunda, 989 ± 571 U/ml ve N grupta 636 ± 549 U/ml olarak bulundu (H-SH: p<0.05, H-N: p<0.001, SH-N: p<0.05 ). Hipertiroidi grubunda IL-4 düzeyi, 0.285±0.118 pg/dl, SH grubunda, 0.275 ±0.151 pg/ml ve N grupta 0.396 ± 0.269 pg/ml olarak bulundu. Gruplar arasında anlamlı bir farklılık saptanmadı. Hipertiroidi grubunda IL-8 düzeyi 18.12+32.15 pg/dl, SH grubunda, 5.78 ± 0.93 pg/ml ve N grupta 5.57 + 1.27 pg/ml olarak bulundu (H-SH: p<0.05, H-N: p<0.05). Hipertiroidi grubunda IL-13 düzeyi 0.041+0.03 pg/dl, SH grubunda, 0.055 ± 0.061 pg/ml ve N grupta 0.054 + 0.23 pg/ml olarak bulundu (H-N: p<0.05). Femur trochanter bölgesi kemik mineral yoğunluğu (BMD) H grubunda 0.650 ± 0.075 gr/cm2, SH grubunda 0.684 ± 0.075 gr/cm2 ve N grupta 0.709 ± 0.047 gr/cm2 olarak bulundu (H-N: p<0.05). Femur intertrochanter BMD H grubunda 0.981 +0.131 gr/cm2, SH grubunda 1.026 ± 0.102 gr/cm2 ve N grupta 1.099 ± 0.80 gr/cm2olarak bulundu (H-N: p<0.05). Tüm çahşma grubu ele alındığında, Tiroid stimulan hormon ile femur intertrochanter-BMD (r=0.354, p<0.05), Z skoru (r= 0.352, p<0.05) ve femur total-BMD (r= 0.404, p<0.05) arasında, IL-8 düzeyi ile femur 71neck BMD (r=0.482, p<0.05), T skoru (r=0.482, p<0.05) ve Z skoru (r=0.448, p<0.05) arasında ve IL-13 düzeyi ile lumbar vertebra-3 BMD (r=0.406, p<0.05), T skoru (r=0.411, p<0.05), Z skoru (r=0.358, p<0.05) arasında pozitif ilişki bulundu. Bu bulgular raenapoz öncesi dönemdeki hipertiroidili kadınlarda IL-2 ve DL-8 düzey artışının kemik mineral yoğunluğu üzerine olumsuz etkisi olmadığını, EL- 13 düzeyindeki azalmanın ise osteoporoz mekanizmasında etkili olabileceğini göstermektedir. 72 | |
dc.description.abstract | EFFECTS OF THE THYROID HORMONES AND CYTOKINES ON OSTEOPOROSIS IN WOMEN WITH HYPERTHYROIDISM IN THE PREMENOPAUSAL PERIOD SUMMARY Hyperthyroidism and osteoporosis are common disorders in the general population. Hyperthyroidism is one of the causes of secondary osteoporosis. But its mechanisms are still unclear. The thyroid hormones does not directly stimulate the activation of osteoclast which is responsible for osteoporosis, but it is considered that they may use some mediator mechanisms. Probably these mediators may be some cytokines. The aim of this study was to investigate the effects of the thyroid hormones and cytokines on the progression of osteoporosis in women with hyperthyroidism in the premenopausal period. 22 women with hyperthyroidism (H), 12 women with subclinic hyperthyroidism (SH), and 10 normal (N) healthy women in the premenopausal period are included in this study. Interleukin-2 receptor level was found 1848±1006 U/ml in group H, 989 ± 571 U/ml in group SH and 636 ± 549 U/ml in group N, respectively (H-SH. p<0.05, H-N: p<0.001, SH-N: p<0.05 ). Interleukin-4 level was found 0.285±0.118 pg/dl in group H, 0.275 ±0.151 pg/ml in group SH and 0.396 ± 0.269 pg/ml in group N. There was no significant difference between the groups. Interleukin-8 level was found 18.12±32.15 pg/dl in group H, 5.78 ± 0.93 pg/ml in group SH and 5.57 ± 1.27 pg/ml in group N, respectively (H-SH: p<0.05, H-N: p<0.05). Interleukin-13 level 73was found 0.041±0.03 pg/dl in group H, 0.055 ± 0.061 pg/ml in group SH and 0.054 ± 0.23 pg/ml in group N (H-N: p<0.05). The bone mineral density (BMD) of femur trochanter area was found 0.650 ± 0.075 gr/cm2 in group H, 0.684 + 0.075 gr/cm2 in group SH and 0.709 ± 0.047 gr/cm2 in group N (H-N: p<0.05). The BMD of femur intertrochanter area was found 0.981 ± 0.131 gr/cm2 in group H, 1.026 ±0.102 gr/cm2 in group SH and 1.099 ± 0.80 gr/cm2 in group N (H-N: p<0.05). There were positive correlations between thyroid stimulating hormone and the BMD of femur intertrochanter area (r=0.354, p<0.05), Z score (r= 0.352, p<0.05), the BMD of total femur area (r= 0.404, p<0.05); IL-8 level and the BMD of femur neck area (r=0.482, p<0.05), T score (r=0.482, p<0.05), Z score (r=0.448, p<0.05); BL-13 level and the BMD of lumbar spine-3 area (r=0.406, p<0.05), T score (r=0.411, p<0.05), Z score (r=0.358, p<0.05) in all study group. This results show that the increased levels of IL-2 and IL-8 don't have negative effect on the bone mineral density and reduced level of IL-13 may be effect on the mechanism of osteoporosis in women with hyperthyroidism in the premenopausal period. 74 | en_US |
