dc.description.abstractTopaloğlu.A Çocuklarda gastrointestinal ve solunum sistemi yabancı cisimlerinde tanı ve tedavi yaklaşımı. Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Çocuk Cerrahisi Anabilim Dalı Tıpta Uzmanlık Tezi, Eskişehir, 2015. Bu çalışmamızda 2000-2014 yılları arasında, yabancı cisimlerin aspirasyonları ve yutulması (YCAY) nedeni ile kliniğimize yatırılarak takip ve tedavi edilen 629 hastayı retrospektif değerlendirildi. Hastaların şikayetleri incelendiğinde; en sık öksürük 175 (%29,9), solunum sıkıntısı 82 (%14), morarma 64 (%10,9), kusma 39 (%6,7), hırıltılı solunum 37 (%6,3), öğürme 28 (%4,8), ve yutamama, yutkunmakla batma 27 (%4,6) şikayeti ile başvurdular. 99 hastanın herhangi bir şikayeti yoktu. 629 hastanın 47'si (%7,3) sadece yatırılarak gözlendi. 343 hastaya bronkoskopi (%54,6), 135 hastaya özefagoskopi yapıldı (%21,5). 18 (%2,9) hastaya özefagoskopi veya bronkoskopiye ek olarak, özofagus dilatasyonu, inguinal bölge cerrahisi, hemodializ için katater yerleştirilmesi gibi ek işlemlar yapıldı. 15 (%2,4) hastaya bronkoskopi ardından özefagoskopi de yapıldı. 35 (%5,6) hastada laringoskopi yeterli oldu. 10 (%1,6) hastaya laparotomi yapıldı. Lokalizasyonu tesbit edilen 592 hastadaki yabancı cisimler incelendiğinde, sağ ana bronşta 113 (%19,1) , sol ana bronşta 89 (%15) olmak üzere, sağ ana bronşta daha sık izlendi. Özofagus 1. darlıkta 74 (%12,5), 2. darlıkta 13 (%2,2) 3. darlıkta 15 (%2,5) yabancı cisme rastlandı. Yabancı cisimlere müdahele geciktirilmemeli, yerinde ve zamanında yapılan işlemle hastanın tedavisi düzenlenmelidir. Sonuç olarak hastaya özgü yaklaşım-doğru plan-zamanında yapılan işlemle çocuklar sağlığına kavuşacaktır. Anahtar kelimeler: Yabancı cisim, bronkoskopi, özefagoskopi
dc.description.abstractTopaloğlu A. The diagnosis and therapeutic approach of gastrointestinal and respiratory foreign bodies in children. Eskisehir Osmangazi University, Faculty of Medicine, Department of Pediatric Surgery, Specialty Thesis in Medicine, Eskisehir, 2015. In this study, 629 patients, who were observed and treated in our clinic because of aspiration and ingestion of foreign bodies (YCAY) between the years 2000-2014, were retrospectively evaluated. When the patient complaints were examined, it was seen that 99 patients did not have any complaints. The patients commonly resorted with these complaints; cough 175 (29.9%), respiratory distress 82 (14%), bruising, 64 (10.9%), vomiting 39 (6.7%), wheezing 37 (6.3%), retching 28 (4.8%), and inability to swallow and swallow sinking 27 (4.6%). 47 of 629 patients (7.3%) were only observed in hospital. Bronchoscopy was performed in 343 patients (54.6%). Esophagoscopy was performed in 135 patients (21.5%). In addition to esophagoscopy or bronchoscopy, some additional processings such as esophageal dilatation, groin surgery or inserting a catheter for hemodialysis were performed in 18 patients (2.9%). Bronchoscopy and then esophagoscopy were applied to 15 patients (2.4%). Laryngoscopy had enough in 35 (5.6%) patients. Laparotomy was done in 10 patients (1.6%). When the 592 patients, the localization of their foreign bodies were identified, were analyzed, it is seen that they were in the right main bronchus 113 (19.1%) and left main bronchus 89 (15%), so it was mostly observed in the right main bronchus. The foreign bodies were found in 74 patients (12.5%) in 1st Esophageal Stenosis, 13 patients (2.2%) in 2nd Esophageal Stenosis and in 15 patients (2.5%) in 3rd Esophageal Stenosis. The intervention to foreign bodies should not be delayed and the patient's treatment should be held with accurate operations.
