dc.description.abstractAMAÇ.Akut kolesistit en sık görülen akut batın nedenlerinden biridir.ÇalıĢmamızda, akut kolesistit olgularında peroperatif zorlukların preoperatifyapılan muayene, ultrasonografi, hemogram ve biyokimyasal tetkiklerleiliĢkisini inceleyip, preoperatif dönemde operasyon esnasındaki güçlükleriöngörmeyi amaçladık.METOD.Bu çalıĢma Ġstanbul Medeniyet Üniversitesi Göztepe Eğitim veAraĢtırma Hastanesi, Genel Cerrahi Kliniği'nde prospektif olarak yapıldı.ÇalıĢmaya Ocak 2019 – Ağustos 2019 tarihleri arasında Genel Cerrahikliniğinde akut taĢlı kolesistit tanısı ile opere edilen hastalar alındı. Veriler(SPSS) 22 programı ile değerlendirildi.BULGULAR. Olguların %29'u (n=9) kadın, %71'i (n=22) erkektir. OlguyaĢları 20 ile 86 arasında değiĢmekte olup, ortalama 53±1,77 yıldır ASASkoru I olan hastaların Evans skoru %55,6' sında (n=5) Evans I, %22,2'sinde (n=2) Evans II, %22,2' sinde (n=2) Evans III , ASA Skoru II olanhastaların Evans skoru %30,0' ında (n=6) Evans I, %40,0' ında (n=8) EvansII, %30,0' ında (n=6) Evans III, ASA Skoru III olan hastaların Evans skoru%50,0' sinde (n=1) Evans II, %50,0' sinde (n=1) Evans III gözlenmiĢ olupistatiksel olarak anlamlı farklılık saptanmamıĢtır(p>0,05). Preoperatifyapılan ultrasonografi değerlendirmesi ile vaskülerizasyon, sonogrofikmurphy belirtisi ve duvar kalınlık artıĢı ile ameliyat süresi arasında anlamlıfarklılık saptanmamıĢtır (p>0,05)SONUÇ. Akut kolesistit tedavisinde cerrahi ön planda olup bizimçalıĢmamızda hastaların Ģikayetlerinin baĢlangıç zamanı ile operasyonzorluğu arasında anlamlı farklılık bulunmamıĢtır.Akut kolesistitli olgulardapreoperatif laboratuar ve görüntüleme bulgularıyla peroperatif adezyonlararasında iliĢki saptanmamıĢtır.Anahtar Kelimeler: Evans, Adezyon, Akut Kolesistit
dc.description.abstractOBJECTIVE.Acute cholecystitis is one of the most common causes of acuteabdomen. In this study, we aimed to investigate the relationship of peroperative difficulties with pre-operative examination, ultrasonography,hemogram an d biochemical tests in acute cholecystitis cases and to predictthe difficulties during operation in the pre-operative period.METHODS. This study was performed prospectively at the IstanbulMedeniyet University Göztepe Training and Research Hospital GeneralSurgery Clinic. Patients who underwent surgery with the diagnosis of acutecholecystitis in the General Surgery Department between January 2019 andAugust 2019 were included in the study. Data were evaluated with SPSSStatistics V22.0.RESULTS. 29% (n = 9) of the cases were female and 71% (n = 22) weremale. The age of the patients ranged from 20 to 86 years with a mean age of53 ± 1.77 years. According to the Evans scores of the patients with ASAScore 1, 55.6% (n = 5) were Evans I, 22.2% (n = 2) were Evans II, 22.2% (n =2) were Evans III ; patients with ASA Score II, 30.0% (n = 6) were Evans I,40.0% (n = 8) were Evans II, 30.0% (n = 6) were Evans III; patients withASA Score III, 50.0% (n = 1) were Evans II and 50.0% (n = 1) were Evans IIIand there was no statistically significant difference (p>0,05). Preoperativeultrasonography was used to evaluate vascularization, sonographic murphysign and wall thickness increase, and no significant correlation was foundbetween these ultrasonographic findings and operation time (p> 0.05).RESULT. Surgery is the first choice in the treatment of acute cholecystitisand in our study, no significant difference was found between the preoperative pain duration and operation difficulty of the patients. Preoperative laboratory and imaging findings were not correlated with peroperative adhesions in patients with acute cholecystitis.Keywords: Evans, Adhesion, Acute Cholecystitisen_US
