dc.description.abstractAmaç: Efüzyonlu otitis media (EOM), çocukluk çağı kazanılmış işitme kayıplarının en sık sebebidir. Pediyatrik hasta grubunda çoğunlukla, zaman alıcı bir test olan saf ses odyometri testine kooperasyon sağlanamamakta, ve de tanı genellikle otoskopik muayene ve timpanometri ile kısıtlanmaktadır. Bu çalışmanın amacı; cerrahi tedavi öncesi ve sonrası transient evoked otoakustik emisyon (TEOAE) paramtrelerini değerlendirmek ve orta kulaktaki efüzyon varlığını öngörmedeki rolünü belirlemektir.Yöntem: Bu prospektif çalışmaya; yaşları 2 ile 15 arasında değişen, 68 hasta (133 kulak) dahil edildi. Preoperatif her hastanın timpanometrik ölçümleri ve TEOAE testleri yapıldı, koopere olabilen 13 hastaya (26 kulak) ise preop. ve postop. saf ses odyometrisi uygulandı. Her hastaya genel anestezi altında miringotomi veya tüp tatbiki yapıldı. TEOAE ölçümleri, postoperatif 1.hafta, 1.ay, 3.ay ve 6.ayda kaydedildi ve klinikle korelasyonu izlendi.Bulgular: Preoperatif % 87,2 (116/133) oranında negatif TEOAE kaydedildi, bu oran cerrahi tedavi sonrası %14,3'e (19/133) geriledi. Tüp tatbiki ve miringotomi yapılan her iki grupta, postoperatif tüm takiplerde emisyonlarda preop. döneme göre anlamlı artış (p<0,001)ve odyometride hava-kemik gap değerlerinde anlamlı düzelme (p<0,05) izlendi. Operasyon sırasında efüzyon görülen kulakların %95,4'ünde (84/88) preoperatif TEOAE testi negatifti. Preop. emisyon elde edilmeyen kulaklarda %72,4 (84/116) oranında efüzyon görüldü. Preop. TEOAE testi pozitif olan kulaklarda %76 (13/17) oranında efüzyon yoktu ve efüzyonu olmayan kulaklarda emisyon elde edilme oranı %28,8 (13/45) idi.Sonuç: Efüzyonlu otitis media tanısında, efüzyonu öngörmede ve cerrahi tedavi sonrası takibinde TEOAE'ların kullanımı; hızlı, pratik ve objektif bir yöntemdir.Anahtar Kelimeler: Otoakustik Emisyonlar, Efüzyonlu Otitis Media, Cerrahi
dc.description.abstractObjectives: Otitis media with effusion (OME) is one of the most frequent causes of hearing loss in children. Mostly cooperation can not be achieved via Pure tone audiometry which is time-consuming and difficult to perform in pediatric patients, therefore diagnosis is restricted to otoscopic examination and tympanometry. The goal of this study is to evaluate otoacoustic emissions parameters before and after surgery and to reveal the role of transient otoacoustic emissions in predicting effusion in middle ear preoperatively.Materials and Methods: 68 patients (133 ears), aged between 2 to 15 years old, included in this prospective study. Preoperatively tympanometric examination and TEOAE was performed in all patients. Preoperatively and postoperatively pure tone audiogram was performed in 13 patients (26 ear) who were cooperated for test. Myringotomy and ventilation tube insertion was performed under general anesthesia. Transient evoked otoacoustic emission tests were also recorded in the preoperative and postoperative first week and postoperative first, third and sixth month. Otoacoustic emission results observed to be correlated with clinical results. Results: The preoperative TEOAEs were not recorded in %87, 2 (116/133) and after surgery TEOAEs were not recorded %14, 3 (19/133).İn both miryngotomy and tube insertion groups positive OAE values were significantly increased postoperatively comperad to preoperative values(p<0,001). Air-bone gaps were significantly decreased on puretone audiograms of 26 ears (p<0,05). Preoperative TEOAE values were negative in %95,4 (84/88) of patients whom determined to have effusion during operation. Effusion determined in %72,4 (84/116) of ears in which preoperative TEOAEs were not recorded. There was no effusion determined in 76 (13/17) of ears in which preoperative TEOAEs were recorded. Emission rate was %28,8 (13/45) in ears without effusion.Conclusion: In patients with OME, application of TEOAEs in diagnosis and predicting effusion preoperatively and follow-up of surgical effects postoperatively is a simple, objective and not time-consuming method.Key words: Otoacoustic emissions, Otitis media with effusion, Surgery.en_US
