Esansiyel hipertansiyon tedavisinde beta blokerlerin plazma lipid fraksiyonlarına etkileri

Abstract

ÖZET Hafif ve orta derecede esaıısiyel hipertansiyonlu 62 hastada üç farklı beta blokerin plazma lipid fraksiyonlarına etkileri araştırıldı. Yirmi hastaya ISA'siz ve kardiyoselektif metoprolol günde iki kez 50 mg, yirmi hastaya ÎSA'siz ve nonselektif propranolol günde üç kez 20 mg, 22 hastaya ÎSA'li ve nonselektif pindolol günde iki kez 5 mg oral yolla verildi. Sistohk ve diyastolik kan basmcı her üç beta blokerle anlamlı derecede düştü (sırasıyla p<0.001, p<0.001). Nabız sayısı metoprolol ve propranolol ile önemli derecede yavaşladı (sırasıyla p<0.001, p<0.001), pindolol ile önemli değişiklik göstermedi. Bazal değerlere göre, metoprolol plazma trigliserid ve VLDL-K'unu anlamlı olarak arttırdı (sırasıyla p<0.05, p<0.05). Propranolol HDL-K'u azalttı, LDL-K'u arttırdı (sırasıyla p<0.(®l, p<0.05). Pindolol ise plazma total kolesterolünü ve HDL-K'unu arttırdı (sırasıyla p<0.05, p<0.001). Total kolesteroldeki artış HDL-K'un artması sonucuydu. Plazma lipidlerine etki bakımından üç beta bloker karşılaştirınca, metoprolol pindolola göre trigliserid düzeyini (p<0.05), propranolol ise pindolola göre LDL-K'unu (p<0.001), pindolol ise metoprolol ve propranolola göre HDL-K'unu anlamlı olarak arttırdı (p<0.001). Bu bulgulara göre ÎSA'siz beta bloker olan metoprolol ve propranolol plazma lipid profilini olumsuz, İSA'H pindolol ise olumlu etkilemektedir. 27

SUMMARY In 62 patients with mild and moderate essential hypertension the effects of three beta blockers on plasma lipid fractions were investigated. In twenty patients on cardioselective and non-ISA beta blocker metoprolol was given twice 50 mg daily per oral. In other 20 patients propranolol, a nonselective and with ISA beta blocker, was given three times a day with a dose 20 mg and the last group which consisted of twenty two patients received pindolol (with ISA and selective) twice 5 mg orally. Both systolic and diastolic pressures significantly fell down (pO.OOl, pO.OOl) with all three beta-blockers. The number of radial pulse significantly slowed down with metoprolol and propranolol (pO.OOl, pO.OOl). No significant change was observed with pindolol. Compared with basic values metoprolol significantly raised plasma trigliserid and VLDL-K (pO.OOl, p<0.05). Propranolol lowered HDL-K and raised LDL-K (pO.CDl, p<0.05). Pindolol on the other hand raised plasma total cholesterol and HDL-K (p<0.05, pO.OOl). The raise in the total cholesterol was a result of the raising of HDL-K. When compared according to the effects of them on the plasma lipids, metoprolol raised the level of trigliserids more than pindolol (pO.05). On the other hand, propranolol raised LDL-K more than pindolol (pO.OOl) and pindolol raised HDL-K significantly when compared with metoprolol and propranolol. In the light of these findings we reached to the conclusion that non-ISA beta-blockers metoprolol and propranolol effect plasma lipid profile negatively and pindolol a beta-blocker with ISA, effects positively. 28
