dc.description.abstractAmaç: Bu çalıĢmanın amacı 1 yıllık takipte levonorgestrel salgılayan intrauterin aracın (LNG-RIA) uterin arter, radial arter ve sipiral arter kan akımına olan etkilerini araĢtırmaktır.Yöntem: Mustafa Kemal Üniversitesi Eğitim AraĢtırma Hastanesinde menometroraji nedeniyle LNG-RIA kullanan 117 fertil hastaya ait uterin arter, radial arter ve spiral arter kan akımının 1 yıllık değiĢiminin incelenmesi.Bulgular: 91 olgu 1 yıllık kontrolü tamamladı. Uterin arter rezistans indeks (RI) ve pulsatil indekslerde (PI) anlamlı bir değiĢiklik olmazken, radial arter ve spiral arter rezistans indekslerinde artma izlenmiĢtir. Ayrıca endometrial kalınlıkta azalma, hemoglobin, hematokrit değerlerinde artma tespit edilmiĢtir. Koagülasyon parametrelerinde(PT,PTT,INR) değiĢiklik izlenmemiĢtirSonuçlar: LNG-RIA‟nın uterin artere anlamlı bir etkisi tespit edilememiĢtir. Radial arter,spiral arter rezistans indekslerinde artıĢ ve endometrium kalınlığını azaltarak endometrial baskılanma yapması ile menstrual kan kaybının azalması progestagenik lokal etkisini ön plana çıkartır. Koagülasyon parametrelerinde değiĢimin olmaması intrensek ve ekstrensek pıhtılaĢma yollarına etki etmediği ve sistemik etki ile kanamayı durdurmaya yardımcı olmadığına iĢaret eder.Anahtar kelimeler: LNG-RĠA,Uterin arter,radial arter,spiral arter
dc.description.abstractBackground and aim: The aim of this study was to investigate the effects of levonorgestrel-releasing intrauterine device(LNG-IUD)on blood flow of uterine, radial and spiral arteries.Methods: 117 fertile patients who admitted to Mustafa Kemal University Training and Research Hospital by using the LNG-IUD due to menometrorrhagia were included in the study. A 1-year changes in blood flow of uterine, radial and spiral arteries were recorded.Results: 91 patients completed 1-year control. There were no significant change in neither uterine artery resistive index nor pulsatile index. However, resistive index of radial and spiral arterieswere observed as increased. Additionally, decrease at endometrial thickness was accompanied to increase at hemoglobin and hematocrit values. No alteration was seen at coagulation parameters (PT, PTT, INR).Conclusion: Any significant effect of LNG-IUD on the uterine artery could not be detected. Progestageniclocal effect came to the fore by endometrial suppressionand decrease of menstrual blood loss as a result of increased radial and spiral artery resistance index and reduced thickness of endometrium. The lack of change in the coagulation parameters indicates that there was no effect over intrinsic and extrinsic coagulation pathways and no help effects on stop bleeding.Key words: LNG-RĠA,uterine artery, radial artery,spiral arteryen_US
