dc.description.abstractÖZET Medikal tedaviye refrakter TLE' li olgularda karşımıza en sık çıkan temporal lob patolojisi mezial temporal sklerozistir (MTS). Bu çalışmada hipokampusun kalitatif görsel analizi ile kantitatif volumetrik sonuçları karşılaştırılmıştır. Çalışmaya klinik olarak kesin temporal lob kompleks parsiyel nöbetleri olan 20 hasta ve kontrol grubu olarak 14 sağlıklı birey alınmıştır. MR protokolü olarak rutin sekanslara ek olarak hipokampusun uzun eksenine dik koronal Tl ve (FMPIR) sekansları kullanılmıştır. Görüntüler 3 mm kalınlığında ve aralıksız olarak elde edilmiştir. Sağ ve sol hipokampus normalize hacimleri, ve sağ-sol hacim oranları her hasta için hesaplanmıştır.TLE'li ve normal bireylerdeki hipokampus normalize hacim değerleri ve hacim oranları karşılaştırılmıştır. % 54 hastada EEG anormalliği, 10 olguda (% 50) ise MR'da görsel anormallik saptanmıştır. Volumetrik değerlendirmede minimum hacim oranına göre % 70, minimum hacim miktarına göre % 65, iki yöntem birlikte kullanıldığında ise toplam % 75 hastada atrofı saptanmıştır. Sonuç olarak, MR ve volumetrik incelemesinin TLE Ti hastalarda tanı amaçlı yararlı tetkik olduğu gösterilmiştir. 50
dc.description.abstractSUMMARY The most common temporal lobe pathology is mesial temporal sclerosis (MTS); several different imaging techniques have been utilized to detect this with varying success. In this study, visual analysis of MRI was compared to volumetric studies of the hippocampal formation. MR imaging was performed in 20 patients with certain temporal lobe epilepsy and 14 healthy volunteers. Following the routine brain imaging, Fast multiplaner inversion recovery (FMPIR) and 3D SPGR sequences were obtained in the plane perpendicular to the long axis of the hippocampus. Volumes of the right and left hippocampal formations (HF) were measured from these oblique coronal MR images by hand-method. Normalised volumes and volume ratios of the HF were calculated for each patient and normal volunteer. Visual analysis were performed with special attention to the hippocampus. Quantitative analysis also were performed inorder to obtain the diagnosis of TLE. Visual and quantitative analysis also were compared. Statistical evaluation also were performed by using Mann-Whitny-U test. EEG was found positive in 14 (54 %) patients, Visual analysis was positive inlO (50 %) of the patients. Abnormal signal was observed in 1 (5 %) patient. In the quantitative analysis, atrophy was detected in (70 %) of the cases by using the volume ratios while (65 %) of the patients by using minimum normalised volume amount and (75 %) whwn both of these techniques used together. As a result, MRI found to be the modality of the choise for TLE, volumetric measurement is not essential for every case but only in when there is a visual suspicion. 51en_US
