İodopovidonun malign ve persistan plevral efüzyonlarda etkinliğinin talk plöredezis ile karşılaştırılması

Abstract

AMAÇ: %0,9'luk salin solüsyonu içinde çözelti oluşturulan talk ile %0,9'luk salin solüsyonu içinde çözelti oluşturulan %10'luk iodopoviodonun plöredez etkilerinin karşılaştırılması.MATERYAL METOD: Çalışma prospektif olarak hastalar ve deneysel olarak iki platformda dizayn edilmiştir.Hastalar: Pamukkale Üniversitesi Tıp Fakültesi Göğüs Cerrahi sevisinde takip ve tedavi edilen plevral sıvılı 16 adet hasta tüp torakostomi sonrası takibe alındı ve bu hastalara plöredez amaçlı iodopovidone çözeltisi uygulandı. Hastaların tüp torakostomi öncesi ve sonrası kontrol akciğer grafileri ve tiroid fonksiyonlarında değişiklikler karşılaştırıldı.Deneysel çalışma: Bu deneysel çalışma Ocak 2008?Mart 2008 tarihleri arasında Pamukkale Üniversitesi Tıp Fakültesi Deney Laboratuarında, Pamukkale Üniversitesi Tıp Fakültesi Etik Kurulu tarafından onaylandıktan sonra, 10 adet Yeni Zelanda beyaz tavşanında yapıldı. I. Grup tavşanlarda talk, 2. Gruptaki tavşanlarda ise %10'luk iodopovidone %0,9'luk serum içinde çözeltilerek verildi. Patolojik inceleme makroskopik ve mikroskopik olmak üzere iki şekilde yapıldı.BULGULAR:Klinik: Hastalarda %87 plöredez başarısı sağlandı. Bu hastalarda T3 değerlerinde subklinik düşüş saptandı, ancak bu durumun iodopoviodondan olmadığı düşünüldü.Deneysel: Tavşanlarda talk grubunun daha iyi gradelerde yapışma sağladığı ancak iodopoviodon grubunun daha diffüz yayıldığı tespit edilmekle beraber tavşanların tiroid fonksiyonlarında herhangi bir değişiklik saptanmadı.SONUÇ: İodopoviodon güvenilir, etkili, kolay bulunabilen ve ucuz bir plöredez ajanıdır.

Aim: To compare the safety and efficacy of chemical pleurodesis with talc in 0,9% saline solution and 10% iodopovidone in 0,9% saline solution.Material and method: The study was desinged in two fields: A prospective clinical investigation and a comperative experimental investigation.Clinical investigation: Sixteen patients with pleural effusion treated and followed in Thoracic Surgery Clinic in Pamukkale University Hospital were included after tube thoracostomy and chemical pleurodesis with iodopovidone solution was performed to all patients. The effect of pleurodesis was evaluated according to the changes in chest x-rays before and after thoracostomy and thyroid function tests.Experimental investigation: That part of the study was carried out in the Experimental Studies? Laboratory of Pamukkale University between January and March 2008. In the study approved by Ethical Committe of Pamukkale University, 10 New Zeland white rabbits was divided into two groups. The pleurodesis was performed with talc in 0,9% saline solution in Group I and with 10% iodopovidone in 0,9% saline solution in Group II. The pleural samples were examined pathologically both in microscopic and macroscopic methods.Results:Clinical examination: The success rate of pleurodesis was 87% in the patient group. Serum T3 hormone levels were found decreased subclinically. But this situation are not related with iodopovidone application.Experimental investigation: The grade of adhesion was better in talc pleurodesis group while the distrubition was more diffuse in iodopovidone pleurodesis group. No changes in thyroid function tests in both rabit groups were occured.Conclusion: The iodopovidone solution is a safe, effective, easily available and cheap choice for pleurodesis.
