Sigara kullanan ve kullanmayan generalize kronik periodontitis hastalarında faz ı periodontal tedavi öncesi ve sonrası ve-cadherin, VEGF seviyelerinin araştırılması

Abstract

Amaç: Çalışmamızda, sigaranın periodontal damarlanma ve periodontal tedavi sürecine etkilerini detaylı araştırmak adına, sigara kullanan ve kullanmayan periodontitislilerde faz I periodontal tedavi öncesi ve sonrası dişeti oluğu sıvısı (DOS) VE-cadherin ve VEGF seviyelerinin tespit edilmesi ve karşılaĢtırılması amaçlanmıştır. Materyal ve Metot: Çalışmaya sistemik sağlıklı 15 sigara kullanan (grup 1) ve 15 kullanmayan (grup 2) generalize kronik peridontitisli hasta dahil edildi ve gruplara faz I periodontal tedavi uygulandı. Tedavi öncesi (0. gün) ve tedavi sonrası (7. ve 30. günler) periodontal durumu değerlendiren klinik ölçümler yapıldı ve en derin cep derinliğine sahip 2 diş bölgesinden DOS örnekleri toplandı. ELISA yöntemiyle DOS VE-cadherin ve VEGF seviyeleri tespit edilerek grupiçi ve gruplararası değerlendirmeler gerçekleştirildi. Toplanan klinik veriler ve DOS hacimleri de çalışma parametreleri olarak kullanıldı. Bulgular: Tedavi sonrası klinik parametreler ve DOS hacminde tüm gruplarda zamana göre düşüş görüldü. Grup 1'de 30. gün DOS hacmi daha yüksek bulundu (p< 0,05). VE-cadherin konsantrasyonu ve total VE-cadherin miktarının 0. günde ve VE-cadherin konsantrasyonunun 7. günde grup 2'de daha yüksek olduğu görüldü (p< 0,05). Tüm gruplarda 30. günde VE-cadherin konsantrasyonu ve total VE-cadherin miktarı azalma gösterdi (p< 0,05). Total VEGF seviyesi tüm gruplarda ve tüm zamanlarda azaldı (p< 0,05). VEGF konsantrasyonu grup 2'de 7. günde daha yüksek bulundu ve 0.-7. ve 7.-30. gün periodlarında azalma gösterdi (p< 0,05). Sonuç: Bulgularımız, sigara kullanımının damarsal yapı üzerine etkileriyle faz I periodontal tedavi çıktılarını değiştirebileceği fikrini desteklemektedir. Aim: In our study, it has been aimed to determine and compare the VE-cadherin and VEGF levels in gingival crevicular fluid (GCF) before and after phase 1 periodontal treatment in smoking and non-smoking periodontitis patients in order to investigate the effects of smoking on periodontal vascularization and treatment process more detailed. Material and Method: Fifteen smoker (group 1) and 15 non-smoker (group 2) systemically healthy male periodontitis patients were included in the study and they received phase I periodontal therapy. Clinical measurements were made before (day 0) and after (days 7 and 30) treatment and, gingival crevicular fluid (GCF) samples were collected from two tooth with the deepest pocket depths. VE-cadherin and VEGF levels were determined in GCF samples by ELISA and intra-/inter group assessments were done. The collected clinical data and GCF volumes were also utilized as the study parameters. Results: Decreases in the clinical parameters and GFC volumes in accordance with time following therapy were noted in all the groups (p< 0,05). GCF volume in group 1 was found to be higher at day 30 (p< 0,05). VE-cadherin concentration and total VE-cadherin amount were higher in group 2 at day 0 and VE-cadherin concentration were higher in group 2 at day 7 (p< 0,05). VE-cadherin concentration and total VE-cadherin amount demonstrated decreases in all the groups at day 30 (p< 0,05). Total VEGF amount decreased in all the groups and at all the periods (p< 0,05). VEGF concentration was higher in group 2 at day 7 and, it decreased at days 0-7 and 7-30 periods (p< 0,05). Conclusions: Our results suggest that smoking may alter the phase 1 periodontal therapy outcomes by its effects on vascular structure.
