dc.description.abstractAmaç: Time Lapse görüntüleme sistemi ile normal ya da anormal kromozom yapısına sahip embriyoların gelişimleri karşılaştırılması.Gereç ve yöntem: Preimplantasyon genetik tarama yapılan 14 infertil hastadan elde edilen 57 embriyonun morfokinetik özellikleri Primovision Time Lapse sistem ile değerlendirilmiştir. Bölünme zamanları, polar cisim, eşit olamayan bölünme, ters klivaj, multinukleasyon, vakuolizasyon, fragmantasyon, pronükleus oluşumu ve sayısı kaydedilmiştir. Genetik materyal preimplantasyon genetik tanı (PGT) amacıyla alınarak Array Comparative Genomic Hybridisation (aCGH) işlemine tabi tutulmuştur. Time Lapse sistemi ile saptanan morfokinetik parametrelerin Society for Assisted Reproductive Technology (SART) ve British Fertility Society and Association of Clinical Embryologist (BFS/ACE) embriyo skorlamalarıyla ilişkileri değerlendirilmiştir. Bulgular: aCGH analiz sonuçlarında embriyolar normal ya da anormal kromozomal yapıda olmak üzere iki gruba ayrıldı. Vakuol oluşumu normal grupta (0,24±0,43) anormal gruba (0,58±0,5) göre anlamlı olarak daha düşük bulundu (p=0,02). Eşit olmayan bölünme normal grupta (0,41±0,5) anormal gruba (0,7±0,4) göre anlamlı olarak daha düşük bulundu (p=0,04). Ters bölünme normal grupta (0,18±0,3) anormal gruba (0,53±0,5) göre anlamlı olarak daha düşük bulundu (p=0,01). Time Lapse parametrelerinden iki hücreye bölünme zamanı (T2) normal grupta (18,7±8,2 saat) anormal gruptan (24,2±8,5 saat) istatistiksel olarak anlamlı derecede daha düşük bulundu (p=0,02). 4 blastomerli evreye geçiş zamanı (T4) normal grupta ortalama (29±9 saat) anormal gruba IV(35,6±8,5 saat) göre daha kısaydı (p=0,02). Morula evresine geçiş zamanı (TM) normal grupta (69,4±18,1 saat) anormal gruba (83,2±16,1 saat) göre anlamlı derecede daha kısaydı (p=0,01). Sonuç: Time Lapse yöntemi ile analizde, anormal kromozom taşıyan embriyolarda normal kromozom yapısındaki embriyolara göre vakuol oluşumu, eşit olmayan bölünme, ters bölünme ve T2, T4, TM zamanları daha yüksektir.
dc.description.abstractAim: To investigate the correlation between embryos with normal or abnormal chromosomal structures by evaluating preimplantation genetic screening results of the embryos scored with Time Lapse imaging systems. Material and methods: Morphokinetic properties of 57 embryos derived from 14 infertile patients those had preimplantation genetic screening were evaluated by Primovision Time Lapse system. The parameters such as cleavage times, polar body, uneven cleavage, reverse cleavage, multinucleation, vacuolization, fragmentation, presence and number of pronucleus were recorded. Array Comparative Genomic Hybridisation (aCGH) procedure was performed by obtaining plenty of genetic material for Preoperative genetic diagnosis (PGD). The morphokinetic parameters obtained from Time Lapse system were evaluated considering Society for Assisted Reproductive Technology (SART) and British Fertility Society and Association of Clinical Embryologist (BFS/ACE) embryo grading scores.Results: The embryos were divided into two groups as normal and abnormal chromosomal structured after aCGH analysis. Presence of vacuole was 0,24±0,43 in normal group and while it was 0,58±0,5 in abnormal group which was significantly higher (p=0,02). Uneven cleavage was 0,41±0,5 in normal group while it was 0,7±0,4 in abnormal group which was significantly higher (p=0,04). Reverse cleavage was 0,18±0,3 and 0,53±0,5 in abnormal group which was significantly higher (p=0,01). As a Time Lapse parameter, cleavage time into two cells (T2) was 0,18±0,3 in normal group and significantly lower comparing with abnormal group (p=0,02). The time of 4 blastomer stage was 29±9 hours in VInormal group and was shorter than abnormal group (p 0,02). Time of morula stage was 69,4±18,1 and was shorter than abnormal group (p 0,01). There was good quality embriyos with increased T2 time lower SART scores while increased BFS/ACE score. There was a significant correlation observed between abnormal chromosomal structure detected by aCGH analysis and T2, T4, TM and S2.Conclusion: Increased number of of vacuole formation, uneven cleavage, reverse cleavage, T2 ,T4 , TM times, were seen in abnormal chromosomal structured embyos when compared to normal chromosomal structured embryos in Time Lapse analysis.en_US
