dc.description.abstractOZET J Amaç : Bu çalışmada prostat kanserinin primer radyoterapisinde akut yan etkiler açısından konvansiyonel eksternal radyoterapi ile konformal radyoterapi teknik uygulamalarının karşılaştırılması amaçlanmıştır. Gereç ve Yöntem % Mart 1997- Temmuz 2002 tarihleri arasında lokalize prostat kanseri tanısı konmuş ve primer radyoterapi uygulanmış 75 hastanın dosyalan retrospektif olarak taranmıştır. Hastalar konvansiyonel eksternal radyoterapi alan (n=47) ve konformal radyoterapi alan (n=28) olmak üzere iki gruba ayrılmıştır. Tüm hastalar için medyan yaş 72 (55-85), medyan radyoterapi dozu 70 Gray (Gy) (63-76 Gy) olarak tespit edilmiştir. Taramada hormonoterapi almayan 23, eş zamanlı hormonoterapi alan 22, neoadjuvan hormonoterapi alan 30 hasta incelenmiştir. Hastaların en az 3 aylık takibi mevcut olup, görülen akut yan etkiler EORTC/RTOG (European Organisation for Research and Treatment of Cancer / Radiation Therapy Oncology Group) skalasi kullanılarak değerlendirilmiştir (1). Kullanılan tekniklerin hastaya ve tedaviye bağlı özeliklerle ilişkisi de araştırılmıştır. Bulgular: Lokalize prostat kanserinin primer tedavisinde uygulanan İM teknik ile hastaya bağlı özellikler ve tedaviye bağlı özellikler arasında anlamlı farklılık saptanmamıştır (p>0.05). Konvansiyonel radyoterapi grubunda hormonoterapi kullanımı eşit olarak dağılırken, konformal radyoterapi grubunda neoadjuvan hormon kullanımı %64.3 (p=0.004) olarak gerçekleşmiştir. Her iki grupta da grade DI-IV akut üriner ve rektal yan etki gözlenmemiştir. Grade I ve II akut üriner yan etkiler; konvansiyonel radyoterapi grubunda %29.7 ve %23.4 iken, konformal radyoterapi grubunda %60.7 ve %17.9 oranında gözlenmiştir (p=0.025). Grade II üriner yan etkiler; neoadjuvan hormonoterapi alanlarda %25, eş zamanlı hormonoterapi alan hastalarda %56 oranında saptanmıştır (p=0.02). Grade II rektal yan etkiler; ivkonvansiyonel radyoterapi grubunda %18.6, konformal radyoterapi grubunda %2.6 oranında tespit edilmiştir (p=0.022). Üriner-rektal yan etkiler ile hastaya ve tedaviye bağlı özellikler arasında anlamlı bir ilişki bulunmamıştır (p>0.05). Sonuç : Lokalize prostat kanserinin tedavisinde uygulanan dozlarda, her iki tekniğin de akut yan etkiler açısından tolere edilebilir olduğu tespit edilmiştir. Her iki teknikte de grade HI ve IV yan etki gözlenmemiştir. Konformal radyoterapi uygulaması ile grade II akut rektal ve üriner yan etkilerin azaldığı tespit edilmiştir. Eşzamanlı hormonoterapi kullanrmı ile grade II akut üriner yan etki neoadjuvan kullanıma göre daha sık gözlenmiştir. Anahtar kelimeler; Akut yan etki, Konvansiyonel-konformal radyoterapi. dc.description.abstractSUMMARY: COMPARISON OF CONVENTIONAL EXTERNAL AND CONFORMAL RADIOTHERAPY TECHNIQUES FOR THE TREATMENT OF LOCALISED PROSTATE CANCER IN TERMS OF ACUTE COMPLICATIONS Aim; The aim of the study was to compare conventional external radiotherapy and conformal radiotherapy techniques, which are used in primary radiotherapy of prostate cancer, in terms of acute complications. Material and Methods; The charts of 75 patients, who were diagnosed to have `localised prostate cancer` and treated with primary radiotherapy between March 1997 and July 2002, were examined, retrospectively. The patients were categorized into two groups as treated by conventional external radiotherapy (n=47) and conformal radiotherapy (n=28). The median age was 72 (55-85). The median radiotherapy dose was 70 (63- 76) Gy. 30 patients received neoadjuvant hormonotherapy, 22 patients concomitant hormonotherapy while 23 patients didn't receive hormonotherapy. The follow-up period was at least 3 months. Acute side effects were evaluated with the EORTC/RTOG (European Organisation for Research and Treatment of Cancer/Radiation Therapy Oncology Group) scale (1). Also, the relations between these acute side effects and the characteristics of the patients and treatment were investigated. Results: No significant difference was found between two techniques, applied in the treatment of localised prostate cancer and the characteristics of the patients and treatment (p>0.05). While the usage of hormonotherapy was equally dispersed in the conventional radiotherapy group, the usage of neoadjuvant hormone was 64.3% in the conformal radiotherapy group (p=0.004). In both groups, no grade IJJ-IV acute VIurinary and rectal side effects were noted. Grade I and II acute urinary side effects were noted as 60.7% and 17.9% in the conformal radiotherapy group, while they were 29.7% and 23.4% in the conventional radiotherapy group (p=0.025), retrospectively. Grade H urinary side effects was calculated as 25% in neoadjuvant hormonotherapy and grade II side effects as 56% in adjuvant hormonotherapy treatment (p=0.Q2). Grade II rectal side effects were 18.6% in conventional radiotherapy group and 2.6% in conformal radiotherapy group (p=0.022). There was also no significant difference between urinary-rectal side effects and the characteristics that were related to the patients and treatment (p>0.05). Conclusions In terms of acute side effects, both techniques that are used in localised prostate cancer are tolerable. In both techniques, there were no grade in and IV side effects. Besides, conformal radiotherapy is proved to be effective to reduce grade II acute rectal and urinary side effects. In adjuvant hormonotherapy, grade II acute urinary side effects were observed more often than concomitant hormonotherapy.
