Meme kanseri taramasında DR-70 `in duyarlılığı ve özgüllüğü

Abstract

Amaç :Bu çalışmada kan serumu DR-70 ELISA test kiti ile fibrin yıkım ürünleri ölçümünün meme kanserinin saptanmasındaki duyarlılığı ve özgüllüğü ele alındı.Materyal ve metod :Çalışmaya 78 operable evre meme kanseri tanılı hasta ve kontrol grubu olarak 40 sağlıklı birey dahil edildi. Preoperatif serum örneklerinde ve sağlıklı gönüllülerden alınan kan serumunda AMDL DR-70 ELISA testi yardımı ile FDP ölçümü yapıldı. Olguların ve kontrol grubunun demografik özellikleri(yaş, menopozal durum, sigara kullanımı) kaydedildi. Hasta grubunda tümörün histolojik tipi, evresi(TNM), histolojik grade'i, Ki-67 mitotik endeksine göre DR-70 titresinin karşılaştırılması yapıldı. Hastalar ve sağlıklı kontrol grubu DR-70 düzeyleri karşılaştırıldı. AMDL DR-70 testi için hasta ve kontrollerin ayırt edilebileceği cut-off değeri hesaplandı ve testin sensitivitesi, spesifisitesi, pozitif ve negatif prediktif değerleri, yalancı negatif ve yalancı pozitif değerleri ölçüldü.Bulgular:Hasta ve kontrol grubu AMDL DR-70 titrasyon sonuçlarının medyan değerleri 3.85 µg/mL (ÇA 3.18-4.46) ve 0.15 µg/mL (ÇA 0.08-0.48) olup, iki grubun sonuçları arasında istatistiksel olarak ileri derecede anlamlı fark bulundu(p<0.0005). DR-70 'in duyarlılığı %93.75, özgüllüğü ise %90 bulundu. DR-70 ölçümü için optimal cut-off değer ise ≤0.89 µg/mL olarak hesaplandı. Pozitif prediktif değer %94.8, negatif prediktif değer %85.7, false pozitif oran %5 ve false negatif oranı %14.2 bulundu.İki grup arasında yaş ve açısından anlamlı fark saptanmadı(p=0.27). TNM evrelendirme sistemine göre T1-4 ve N0-3 ve evre I-III arası gruplara bölünen hasta grubu olguları arasında istatistiksel olarak anlamlı fark bulunamadı(p=0.154 ve p=0.491 ve p=0.655). Histolojik grade 1-3 arası gruplar arasındaki titre sonuçlarında anlamlı fark saptanmadı(p=0.884). Tümör biyolojisine göre Luminal A, Luminal B, HER2/neu pozitif ve Triple-Negatif gruplarına ayrılan olguların DR-70 titreleri arasında da istatistiksel olarak anlamlı fark saptanmadı(p=0.834). HER2/neu ekspresyonuna göre iki gruba ayrılan olguların titre değerleri arasında anlamlı fark bulunamadı(0.834). Ki-67≥%20 ve Ki-67<%20 oranlarına göre gruplandırılan olguların titre değerleri arasında anlamlı fark bulunamadı(p=0.540). Premenopozal ve postmenopozal gruplara ayrılan hasta ve kontrol grupları DR-70 titreleri arasında anlamlı fark bulunamadı(p=0.144 ve p=0.604). Sigara kullanımına göre alt gruplara ayrılan hasta ve kontrol gruplarında, hasta grubundaki olgularda anlamlı fark saptanmaz iken(p=0.457), kontrol grubu alt grupları arasında anlamlı fark saptandı(p=0.035).Sonuç:AMDL DR-70 ELISA testi ile kanda saptanan fibrin yıkım ürünleri titrasyon sonuçlarının meme kanserli olgularda ≤0.89 µg/mL cut-off değer ile %93.75 sensitivite ve %90 spesifite ile saptanabildiği gösterilmiştir.

Objective :The aim of the study was to assess AMDL DR-70 ELISA fibrin degradation products(FDP) test in screening and diagnosis of breast cancer was the aim of this study.Methods :A total of 78 patients with diagnosis of breast cancer and healthy control group of 40 individuals were included in this study. Blood samples were drawn preoperatively from patients and from healthy contols to study AMDL DR70 ELISA test. Demographic characteristics (age, menopausal status, smoking) of both groups were recorded. In cancer group DR-70 FDP titration results were compared for histological type, stage(TNM), histologic grade, Ki-67 mitotic index and tumor's biologic specifications. DR-70 FDP titers of cancer and control groups were compared. A cut-off value was set and sensitivity, specificity, positive and negative predictive values, false negative and false positive rates were calculated.Results :Median values of AMDL DR-70 titers ) in patients and control groups were found as 3.85 µg/mL (IQR 3.18-4.46) and 0.15 µg/mL (IQR 0.08-0.48) respectively and the difference was found to be statistically significant (p<0.0005). The sensitivity and specificity of DR-70 titers were 93.75% and 90% respectively and ≤0.89 µg/mL was found to be the cut-off for this test. Positive predictive value was calculated as 94.8% and negative predictive value was as 85.7%. False positive and false negative values were 5% and 14.2%, respectively.No statistical difference was found for age between the groups (p=0.27). Patients' DR70 titers were compared within the group for tumor size, nodal status and TNM stage and differences were statistically insignificant (p=0.154 and p=0.491 and p=0.655, respectively). No significant difference was found for DR70 titers among Luminal A, Luminal B, Her2/neu positive and Triple-Negative tumor groups (p=834). Also no significant difference was found for DR70 titers between total HER2/neu expression positive and negative groups (p=834). Titers for patients with Ki-67≥20% ve Ki-67<20% were compared and the difference was found to be statistically insignificant (p=540). DR-70 FDP titers showed no significant difference between premenopausal and post-menopausal subgroups in cancer groups (p=0.144) and healthy controls (p=0.604). DR70 titers was not affected by smoking in cancer group (p=0.457) but, in healthy controls smokers had higher titers and this difference was found to be statistically significant (p=0.035).Conclusion :AMDL DR-70 ELISA test is effective to diagnose breast cancer with cut-off value of ≤0.89 µg/mL with 93.75% sensitivity and 90% specificity.
