dc.description.abstract50 ÖZET Bu çalışmada kan örneklerinde osteokalsin miktarını belirlediği miz, komplikasyonsuz 66 diabetik hastanın, 2 l'i Tip I diabetik, 45'i Tip II diabetik idi. Kontrol grubu olarak Tip I diabetik hastaların yaş grubuna uyan Kontrol Grubu(l) adını verdiğimiz grupta 21 sağlıklı gönüllüden alı nan kan örnekleri incelendi. Tip II diabetik hastaların yaş grubuna uyan Kontrol Grubu(2) ise 20 sağlıklı gönüllüden alınan kan örneklerinden oluş maktaydı. Çalışmamızda Kontrol Grubu(l)'in serum osteokalsin düzeyini, Kontrol Grubu(2)'ye göre, istatistiksel bakımdan anlamlı derecede yüksek olarak saptadık (p<0.05). Tip I diabetik hastaların diabet yaşı ortalamasını 5.19±5.33 yıl, serum osteokalsin düzeyini 8.87+5.10 ng/ml, alkali fosfataz düzeyini 167.47+99.35 U/l, HbAlc düzeyini 12.98±6.94 %, kan glukoz düzeyini 199.28tl05.62mg/dU kalsiyum düzeyini 9.35±0.64 mg/dL fosfor düzeyini 4.51±0.68 mg/dl, Magnezyum düzeyini 0.78±0.12 mmol/1 olarak saptadık. Çalışmamızda Tip II diabetik hastalar için ortalama ve standart sapma değerleri ise diabet yaşı 9.51 + 7.17 yıl, serum osteokalsin düzeyi 4.3412.71 ng/ml, alkali fosfataz düzeyi 84.31±35.42 U/L, HbAlc düzeyi 9.03±3.93 %, glukoz düzeyi 186.2+70.10 mg/dl, kalsiyum düzeyi 9.71±0.64 mg/dl, fosfor düzeyi 4.22+0.59 mg/dl, Magnezyum düzeyi 0.81±0.15 mmol/1 idi.51 Tip I diabetik hastların serum osteokalsin düzeyini kontrol gru bununki(l) ile istatistiksel olarak karşılaştırdığımızda anlamlı bir fark sap tamadık (p>0.05). Tip II diabetik hastaların serum osteokalsin düzeyi, kontrol gru bu (2) ile istatistiksel olarak karşılaştırıldığında, kontrol grubuna göre anlamlı derecede azalmıştı (p<0.01). Tip I ve Tip II diabetik hastaları diabet yaşına göre üç gruba ayırdık. 0-5 yıl arası diabet yaşı olan Tip I iabetiklerin serum osteokalsin değeri ile 6-10 yaş ve 10'dan büyük olanlarınki istatistiksel olarak karşı laştırıldığında, 0-5 yaş grubuna göre, diğer diabet yaşı gruplarında anlam lı bir azalma saptandı (p<0.05)(p<0.05). 10'dan büyük diabet yaşı olan Tip II diabetiklerin serum osteokalsin düzeyi, 0-5 ve 6-10 diabet yaşı grupları ile istatistiksel olarak karşılaştırıldığında diğer iki gruba göre serum osteokalsininde anlamlı bir artış belirlendi (p<0.05)(p<0.05). insulin kullanan Tip II diabetik hastalar ile OAD kullanan veya karbohidrat kısıtlaması uygulayan diabetiklerin serum osteokalsin değerle ri kontrol grubu(2) ile istatistiksel olarak karşılaştırıldığında OAD kulla nan veya karbohidrat kısıtlaması uygulayan diabetiklerde (p<0.01), insulin kullananlara göre (p<0.05) daha anlamlı bir azalma saptadık. Bu çalışmamız sonucunda, serum osteokalsininde Tip II diabetik lerde anlamlı azalma görülmesine rağmen, Tip I diabetiklerde sadece dia bet yaşı ile istatistiksel olarak azalma belirledik. Buna bağlı olarak çalışma mızın, bundan sonraki osteokalsin çalışmalarına ışık tutacağı kanısındayız.
dc.description.abstract52 SUMMARY In this study we determined blood osteocalcin levels of 66 diabe tic patients without any complications. 21 of the patients had type I diabe tes and 45 had type II diabetes mellitus. Control group (1) was made of 21 healthy volunteers approximately of the same age as the type I diabetics. Control group (2) was made of 20 healthy volunteers approximately of the same age as the type II diabetics. In our study we found that serum osteo calcin levels of the Control Group(l) were significantly increased compa red to those of the Control Group(2) (p<0.05). Average diabetes duration of the type I diabetics was 5.19 + 5.33 years. In these patients, we found the mean serum levels of various para meters as the following: osteocalcin: 8.87±5.10 ng/ml, alkaline phosphata se: 167.47±99.35 U/L, HbAlc: 12.98±6.94 %, glucose: 199.28±105.62 mg/dl, total calcium: 9.35+0.64 mg/dl, phosphorus 4.51 + 0.68 mg/dl, and magnesium: 0. 7810. 12 mmol/1. Average diabetes duration of the type II diabetics was 9.5117.17 years, and the mean serum levels of various para meters were as the following: osteocalcin: 4.3412.71 ng/ml, alkaline phosp hatase: 84.31±35.42 U/l. HbAlc: 9.03±3.93 %, glucose: 186.2±70.10 mg/dl, total calcium 9.7110.64 mg/dl, phosphorus: 4.22 + 0.59 mg/dl and magnesi um: 0.8110.15 mmol/1. When we compared the serum osteocalcin levels of type I diabe tis to those of the control group (1) we found no statistically significant difference (p> 0.05).53 Serum osteocalcin levels of the patients with type II diabetes were decreased compared to those of the control group (2), and the diffe rence was statistically significant (p<0.01). Patients with type I and type II diabetes were divided into three groups according to the duration of their disease. Serum osteocalcin levels of type I diabetics with 0-5 years of diabetes duration were compared, to those of type I diabetics with 6-10 years and more than 10 years of diabe tes duration. Type I diabetics with 6-10 years and more than 10 years of duration were found to have decreased serum levels of osteocalcin, and the differences were statistically significant (p<0.05 and p<0.05 respecti vely). In the group of patients with type II diabetes serum osteocalcin levels of patients with diabetes duration of more than 10 years were found to be increased. Compared to those of patients with diabetes duration of 0-5 years and 6-10 years, and the differences were statistically significant (p<0.05 and p<0.05, respectively). Serum osteocalcin levels of patients with type II diabetes melli tus who were treated with insulin were compared to those of patients who used OAD, and who took restricted amounts of carbohydrates. There was a statistically significant decrease in serum osteocalcin levels of patients who used OAD and who took restricted amounts of carbohydrates compa red to those of patients who were treated with insulin (p<0.01 and p<0.05, respectively). In this study we found a statistically significant decrease in serum osteocalcin levels of type II diabetic patients compared to those of the controls. There was no statistically significant difference between serum osteocalcin levels of type I diabetics and the controls, however, serum osteocalcin levels of type I diabetics with diabetes duration of 6- 10 years and more than 10 years were significantly lower than those of diabe tics with diabetes duration of 0-5 years. We think that our study will enlighten the further osteocalcin studies.en_US
