Amaç: Komitan şaşılıklı hastalarda cerrahi başarı üzerine etki eden faktörleri araştırmak.Yöntem: Ocak 2007 ? Ekim 2010 tarihleri arasında Kahramanmaraş Sütçü İmam Üniversitesi Tıp Fakültesi Göz Hastalıkları Kliniği'nde komitan şaşılık tanısı ile ameliyat edilen 60 hastanın dosyası retrospektif olarak incelendi. Cinsiyet, operasyon yaşı, şaşılığın tipi, preoperatif şaşılık miktarı, refraksiyon kusuru, anizometropi ve ambliopi varlığının ve görme keskinliğinin cerrahi başarı üzerine etkisi incelendi. Tüm hastalara preoperatif tam bir şaşılık muayenesi yapıldı. Cerrahi sonrasında hastaların 1.gün, 7.gün, 1.ay ve 6.ay muayene bulguları kaydedildi. Postoperatif kayma miktarının birinci ayda 10 prizm dioprti (pd) ve daha düşük olması ?cerrahi başarı? kabul edildi.Bulgular: Hastalarımızın 24'ü (%40) erkek, 36'sı (%60) kadındı. Ortalama yaş 11,5 ± 8,3 yıl (10 ay- 47 yıl) idi. Cerrahi başarı oranı erkeklerde %71 ve kadınlarda %86 (p=0,147); 10 ay - 5 yaş grubunda %76, 6?12 yaş grubunda %79, 13 yaş üzeri grupta %85 (p=0,773); ekzotropyalı olgularda %83, ezotropyalı olgularda %79 (p=0,598); preoperatif 40 pd ve daha az kayması olan grupta %84, 40 pd üzerinde kayması olanlarda %74 (p=0,353); anizometropik hastalarda %67, anizometropisi olmayan hasta grubunda %81,5 (p=0,389); ambliyop grupta %70, ambliyop olmayan grupta %85 (p=0,171); anlamlı refraksiyon kusuru olan hastalarda %71, emetrop olan hastalarda %87,5 (p=0,121); görme keskinliği kötü olan grupta %62,5, görme keskinliği iyi olan grupta %86 olarak bulundu (p=0,041).Sonuç: Cinsiyet, operasyon yaşı, şaşılık tipi, preoperatif kayma miktarı, anizometropi, kırma kusuru varlığı ve ambliyopinin cerrahi başarı üzerine anlamlı etkisi gösterilemedi. İyi görme keskinliğinin ise cerrahi başarı oranını anlamlı derecede artırdığı saptandı.Anahtar Kelimeler: Komitan şaşılık; Şaşılık cerrahisi; Risk faktörleri; Cerrahi başarı oranı
Purpose: In patients which has comitant strabismus to investigate that the factors affecting the surgical success.Method: Sixty patients were operated with the diagnosis of comitant strabismus between January 2007 and October 2010 in Kahramanmaraş Sütçü Imam University, Faculty of Medicine, Department of Ophthalmology reviewed retrospectively. The risk factors affecting the surgical success rate including gender, age at operation, type of strabismus, preoperative deviation angle, significant refractive errors, presence of anisometropia or amblyopia, and poor visual acuity were examined. All patients underwent a full preoperative strabismus examination. After surgery, patients? examination findings were recorded at first day, seventh day, first month and sixth month. Ten or lower prism dioptre (pd) postoperative deviation was accepted as ?surgical success? at first month. Results: Twenty-four (40%) of our patients were males, and 36 (60%) were females. The mean age was 11.5±8.3 years (10 months-47 years). Surgical success rates, in men was 71% and in women was 86% (p=0.147); in 10 months-5 years age group was 76%, in 6-12 age group was 79%, in 13 and over age group was 85% (p=0.773); in patients with exotropia was 83%, in patients with esotropia was 79% (p=0.598); in cases had 40 pd and less preoperative deviation was 84%, in cases had over 40 pd preoperative deviation was 74% (p=0.353); in patients with anisometropia was 67%, in whom without anisometropia was 81.5% (p=0.389); in the cases which has amblyopia was 70%, the cases which has not amblyopia 85% (p=0.171); in patients which has significant refractive error was 71%, in emmetropic patients was 87.5% (p=0.121); in the group which has bad visual acuity was 62.5%, in the group which has good visual acuity was 86% (p=0.041).Conclusion: Gender, age at operation, type of strabismus, preoperative deviation angle, anisometropia, refractive error and amblyopia were not showed that any significant effect on surgical success. Good visual acuity was found that to increase surgical success rate, significantly.Key words: Comitant strabismus; Strabismus surgery; Risk factors; Surgical success rateen_US
