dc.description.abstractHBV ve HDV Nedeniyle Karaciğer Nakli Yapılan Hastalarda HBV Nüksü Amaç: İnönü Üniversitesi karaciğer nakli enstitüsünde Mart 2003, Haziran 2013 tarihleri arasında, HBV ve HBV+HDV nedeniyle karaciğer nakli yapılan hastalardaki HBV ve HDV nüksünü araştırmayı amaçladık. Materyal ve Metod: İnönü Üniversitesi, Karaciğer Nakli Enstitüsü?nde Mart 2003, Haziran 2013 tarihleri arasında, HBV nedeniyle ve HBV+HDV nedeniyle karaciğer nakli olmuş ve halen yaşamakta olan 312 hastanın verileri geriye dönük olarak incelendi. Hepatoselüler kanserli olanlar (n=40), Alkolik sirozlu (n=2), Hepatit C?li (n=1), düzenli kontrole gelmeyen hastalar (n=14) dışlandığında, kalan 255 hasta çalışmaya dahil edildi. HBV nedeniyle KN yapılan 127 hasta grup 1, HBV+HDV nedeniyle karaciğer nakli yapılan 128 hasta grup 2 olarak ayrıldı. Nakil öncesi viral serolojisinde HDV Ab pozitifliği HBV+HDV olarak belirlendi. Sonuç : Çalışmaya dahil edilen 255 hastanın 127?si HBV (Grup 1); 128?si HDV(Grup 2) nedeniyle nakil yapılan hastalardı. Hastaların ortlama yaşı 47.31±10.97 yıl idi. Grup 1 için 47.67 ±11.63, Grup 2 için 46.96±10.34 idi. Ortalama takip süreleri 30.25±22.76 aydı. Grup 1 için 30.25 ±24.27, Grup 2 için 30.26 ± 21.26 aydı. Çalışmaya alınan 255 hastanın 13?ünde HBs Ag pozitif saptandı (%5,1). Grup 1?de 9 hastada HBs Ag pozitif (%7,1); Grup 2?de 4 hastada pozitif saptandı (%3,1). İki gurup arasında istatistiksel olarak anlamlı fark bulunamadı (p=0.150).Tartışma : Nakil sonrası HBV nüksü halen karaciğer nakli hastalarının büyük bir problemidir. Merkezimizde uygulamakta olduğumuz proflaksi protokolü sonucunda HBV nüksü oranımız literatüre kıyaslandığında daha düşüktür. HDV koenfeksiyon varlığında, HBV nüksü istatistiksel olarak anlamlı olmasa da, nakil sonrası HBV nüksü daha az oranda görülmektedir. Ayrıca Nakil sonrası hiçbir hastamızda HDV nüksüne rastlanmadı.
dc.description.abstract8.AbstractHBV recurrence in patients undergone liver transplantation for HBV and HDVObjective: The aim of this study was to evaluate HBV and HDV recurrence in patients undergone liver transplantation for HBV and HDV performed transplantation between March 2003 and June 2013 in İnstitute of Liver Transplantation at Inonu University. Materials and Methods : Data from medical records of 312 patients, undergone transplantation due to HBV enfection and HBV+HDV coenfection, between March 2003 and June 2013 in İnstitute of Liver Transplantation Inonu University, were retrospectively analyzed. In our study, Hepatocellular carcinoma (n = 40), alcoholic cirrhosis (n = 2) , hepatitis C (n = 1) and irregular fallow up control patients (n = 14) were excluded. The remaining 255 patients were included in the study .Patients included this study were devided into two groups.The first group was consisted of 127- patients operated due to HBV was named group 1 , the second group was consisted of 128 patients undergone liver transplantation due to HBV + HDV. Patients with HDV Ab positive viral serology before transplantation were determined HBV+HDV infection. Results:255 patients, transplanted, were included in the study , 127 HBV (Group 1); 128 HDV (Group 2). The mean age of the patients were 47.31 ± 10.97 years,.Group 1 was 47.67 ± 11.63 and group 2 was 46.96 ± 10.34. The mean follow-up time was 30.25 ± 22.76 months. 30.25 ± 24.27 for group 1 and 30.26 ± 21:26 months for group 2. Among 255 patients included this study, HBs Ag was positive in 13 patients ( 5.1%). HBs Ag was positive in 9 patients in group 1 (7.1%), HBs Ag was positive in 4 patients in group 2 (3.1%) . No statistically significant difference was found between the two groups (p = 0.150 ) . Conclusion: HBV recurrence in post-transplant liver transplant patients is still a major problem. We determined a lower HBV recurrence than literature with prophlaxis treatment protocol implemented in our center. In the presence of HDV coinfection, although HBV recurrence was not statistically significant, post transplantation HBV recurrence was seen lower. Furthermore, in post transplantation patients, no recurrent HDV was seen.en_US
