dc.description.abstractBu araştırma, diabetes mellituslu 40-60 yaş arası hastalarda hastalık süresi ile glikozillenmiş hemoglobin düzeyleri arasındaki ilişki ve etkileyen faktörleri saptamak amacıyla yapılmıştır. Araştırmanın örneklemini, 20/02/2012-20/12/2012 tarihleri arasında Niğde Devlet Hastanesi diyabet ve endokrinoloji polikliniklerinde takip edilen ve dahiliye servislerinde diyabet tanısıyla yatan 166 hasta oluşturmuştur. Veriler tanıtıcı özellikler bilgi formu kullanılarak tespit edilmiş, araştırma kriterlerine uyan ve çalışmaya katılmayı kabul eden her bir hastanın üçer aylık periyodlarla yaptırdığı rutin üç kontrolünde belirlenen glikozillenmiş hemoglobin ve açlık kan glikozu değerleri hasta dosyalarından veya hastane laboratuvar kayıt sisteminden elde edilmiştir. Verilerin değerlendirilmesinde; tanımlayıcı istatistiklerden, Mann-Whitney U ve Kruskall- Wallis testleri ile Spearman Korelasyon analizinden yararlanılmıştır.Hastalık süresi 6-10 yıl, yaşı 51-60, beden kitle indeksi 30.0 kg/m2 ve üzeri olan, ilkokul mezunu ve bekâr hastalarda, yapılan tüm ölçümlerde glikozillenmiş hemoglobin düzeyinin daha yüksek olduğu tespit edilmiştir (p<0.05). Diyabetli bireylerin yapılan ölçümlerdeki glikozillenmiş hemoglobin düzeyi ile hastalık süresi arasındaki ilişkinin zayıf düzeyde ve pozitif yönde anlamlı bir ilişki olduğu belirlenmiştir (p<0.05). Açlık kan glikozu düzeyi ile hastalık süresi arasında da istatistiksel olarak anlamlı bir ilişki saptanmıştır (p<0.05). Araştırmanın sonucunda hastalık süresinin artmasıyla glikozillenmiş hemoglobin düzeylerinin yükseldiğinin belirlenmesi nedeniyle mikro ve makrovasküler komplikasyonların önlenmesi açısından glikozillenmiş hemoglobin düzeyinin özellikle de diyabet süresi 6 yıl ve daha uzun olan hastalarda düzenli olarak takip edilmesi, elde edilen sonuçlara göre diyabet tedavisinin yeniden düzenlenmesi gerektiği düşünülmüştür.
dc.description.abstractThis study was conducted with the purpose of identifying the relationship between the duration of disease of patients aged between 40–60 with diabetes mellitus and their levels of glycosylated hemoglobin, and the factors affecting this relationship. The sample of this study was composed of 166 patients who were monitored between February 20, 2012 and December 20, 2012 in the polyclinics of diabetes and endocrinology in Niğde State Hospital, and who were staying in the ward of internal diseases of this hospital with the diagnosis of diabetes. The data were collected utilizing the information form of descriptive characteristics; the values of glycosylated hemoglobin and fasting blood glucose, identified by means of three routine controls which were implemented once every three months by each of those patients who met the criteria of the study and gave consent to participate in the study, were obtained from the files of patients or from the system of hospital's laboratory records. descriptive statistics, the Mann-Whitney U Test, the Kruskall-Wallis Test, and the Spearman Correlation were utilized to assess the data obtained.The patients whose duration of disease was between 6-10 years, whose ages were between 51-60, whose body mass index was 30 kg/m2 or above,who were primary school graduates, and whose marital status was ''single'' were identified as having higher levels of glycosylated hemoglobin in all of the measurements carried out (p<0.05). It was found that there was a poor relation between the glycosylated hemoglobin levels of individuals with diabetes—which was determined through measurements—and the duration of disease, but this relation was found to be significant in a positive way (p<0.05). It was revealed that there was a statistically significant relation between the levels of fasting blood glucose and the duration of disease (p<0.05). It was concluded from the study that the levels of glycosylated hemoglobin, especially of those whose duration of diabetes extends over 6 years or more, must be monitored periodically in terms of prevention of microvascular and macrovascular complications, due to the fact that it was discovered that the levels of glycosylated hemoglobin increase as the duration of disease increases, and it was considered that the treatment of diabetes must be reorganized in accordance with the conclusions reached.en_US
