dc.description.abstractÇalışmanın amacı metabolik sendromlu koroner arter hastalarında periferik ve solunum kas eğitiminin solunum fonksiyonları, solunum kas gücü, fonksiyonel kapasite, yaşam kalitesi ve depresyon üzerine etkilerini araştırmaktır. Çalışmaya metabolik sendromlu stabil koroner arter hastalığı olan 60 hasta dahil edildi. Çalışma hastaları 20 hasta nöromüsküler elektrik stimulasyon (NMES) ve periferik kas eğitimi, 20 hasta inspiratuar kas eğitimi (İKE) ve periferik kas eğitimi, 20 hasta periferik kas eğitimi olmak üzere 3 gruba ayrıldı. Eğitim 6 hafta boyunca verildi. NMES uygulaması, rectus abdominise, 30 Hertz, haftada 3 gün, 30 dakika uygulandı. İnspiratuar kas eğitimi, İKE cihazı ile MIP'in %30'u şiddetle, haftada 7 gün, günde 2 kez 15'er dakika süreyle uygulandı. Periferik kas eğitimi, elastik band ve proprioseptif nöromusküler fasilitasyon egzersizleri ile verildi. Demografik ve klinik bilgileri, antropometrik ölçümleri, solunum fonksiyonları, solunum kas kuvveti, dispne, periferik kas kuvveti, fonksiyonel kapasite, yaşam kalitesi, depresyon düzeyi ve kan biyokimyası değerlendirildi. Çalışmanın sonucunda tüm gruplarda antropometrik ölçümlerde anlamlı iyileşmeler, MIP, MEP, FVC, FEV1, PEF, MMRC dispne skorunda, 6 dakika yürüme mesafesinde anlamlı artış, borg dispne ve yorgunlukta anlamlı azalma, periferik kas gücünde anlamlı artış sağlandı (p<0,05). Ayrıca SF-36 yaşam kalitesinde anlamlı iyileşmeler, Beck Depresyon Anketi skorunda anlamlı azalma sağlandı (p<0,05). Tüm gruplarda açlık kan glukozu, sedimantasyon, ASO, CRP'de anlamlı iyileşmeler (p<0,05) sağlandı. Her üç grupta tedavi etkinliği benzerdi. Sonuç olarak, metabolik sendromlu koroner arter hastalarında solunum kas eğitimi ve periferik kas eğitimi ile antropometrik ölçümler, solunum fonksiyonları, fonksiyonel kapasite, yaşam kalitesi ve depresyonda iyileşmeler sağlanabilir.
dc.description.abstractThe aim of the study was to investigate the effects of peripheral and respiratory muscle training on respiratory function, respiratory muscle strength, functional capacity, quality of life and depression in coronary artery disease patients with metabolic syndrome. Sixty patients with stable coronary artery disease with metabolic syndrome were included in the study. Patients were randomly divided into 3 groups: 20 patients with neuromuscular electrical stimulation (NMES) and peripheral muscle training, 20 patients with inspiratory muscle training (IMT) and peripheral muscle training, and 20 patients with peripheral muscle training. Exercise training was given to all patients. NMES application was performed to rectus abdominis as 30 Hertz, 3 days a week for 30 minutes. The inspiratory muscle training was applied at the 30 % intensity of MIP for 7 days a week, 2 times a day for 15 minutes using IMT device. Peripheral muscle training was performed to all groups as proprioceptive neuromuscular facilitation exercises using elastic band. The demographic and clinical data, the anthropometric measurements, respiratory functions, respiratory muscle strength, dyspnea, peripheral muscle strength, functional capacity, quality of life, depression level and blood biochemical studies of the patients were evaluated. Significant improvements in the anthropometric measurements, significant increase in MIP, MEP, FVC, FEV1, PEF, MMRC dispnea scores and 6 minute walking distance, significant decrease in borg dyspnea and fatigue and significant increase in peripheral muscle strength (p < 0.05) were observed in all groups at the end of the study. Significant improvement in the SF-36 quality of life total scores and significant decrease in the Beck Depression Questionnaire score (p <0,05) also were achieved. Significant improvements in fasting blood glucose, sedimentation, ASO, CRP were achieved in all groups (p <0.05). Effectiveness of treatments showed similarity in all groups. In conclusion, respiratory muscle training and peripheral muscle training in coronary artery patients with metabolic syndrome can improve anthropometric measurements, respiratory functions, functional capacity, quality of life and depression.en_US
