Açık kalp cerrahisinde kan transfüzyonunun akciğer fonksiyonları üzerine etkileri

Abstract
Açık kalp cerrahisi vücut perfüzyonun ana kaynağına yönelik olduğundan, bu cerrahi alanında kan ve kan ürünü kullanımı kaçınılmazdır. Kan ürününe ihtiyaç kalmasa bile acil cerrahi komplikasyonlardan dolayı her an hazırda bulundurulmaktadır. Açık kalp cerrahisinde etkilenen bir diğer organ da hem anestezi indüksiyonunun genel bir etkisi hem de kardiyopulmoner baypasta kullanılan kalp akciğer makinasına bağlı olarak gelişen akciğer komplikasyonlarıdır. Biz de çalışmamızda bu üç antitenin yani açık kalp cerrahisi, kan kullanımı ve pulmoner fonksiyonların birbiri ile olan ilişkisini inceledik. Bu amaçla Adıyaman Üniversitesi Eğitim ve Araştırma Hastanesi Kalp ve Damar Cerrahisi kliniğinde 2015-2018 tarihleri arasında izole koroner aortokoroner baypas greft cerrahisi olmuş 106 hastanın retrospektif incelemesiyle elde edilen verileri çalışmaya dâhil edilmiştir. Hastalar kan ürünü kullanılmayanlar Grup 1 (n=25), sadece otolog kan kullanılanlar Grup 2 (n=29), ve otolog dışı kan ürünü kullanılanlar Grup 3 (n=52) olmak üzere üç gruba ayrılmıştır. Elde edilen veriler ile de preoperatif, intraoperatif ve postoperatif olarak üç grupta istatistik analizi yapılmıştır. Hastalar preoperatif demografik veriler eşliğinde değerlendirildiğinde hasta grupları arasında istatistiksel anlamlı fark bulgulanmamış ve sonuçların literatüre benzer olduğu görülmüştür. İntraoperatif verilerde ise operasyon süresini uzatabilecek değerler özellikle Grup 3'te istatistiksel olarak anlamlı yüksek bulunmuştur. Postoperatif verilerdeki değerler intraoperatif verilerle paralel olarak yine Grup 3'te drenaj, revizyon, atelektazi, Pa/FiO2<300 istatistikleri yönünden anlamlı çıkmıştır. Açık kalp cerrahisi sonrası akciğer fonksiyonlarını olumsuz etkileyen sebeplerden biri de kan ve kan ürünü kullanımıdır. Bu yüzden bu hasta grubunda olası komplikasyonların önüne geçebilmek adına kan ve kan ürünü kullanımını en aza indirmeye çalışmak her zaman akılda tutulmalıdır. Another organ affected by open heart surgery is a general effect of anesthesia induction and as well as pulmonary complications due to the heart lung machine used in cardiopulmonary bypass. In our study, we examined the relationship between these three entities, namely open heart surgery, blood use and pulmonary function. For this purpose, the retrospective data of 106 patients with isolated coronary aortocoronary bypass graft surgery between 2015-2018 at Adiyaman University Education and Research Hospital Cardiovascular Surgery Clinic were included in the study. The patients were divided into three groups as patients without blood product used Group 1 (n = 25), only autologous blood used Group 2 (n = 29), and Group 3 (n = 52) with non-autologous blood product used. The obtained data were analyzed statistically preoperatively, intraoperatively and postoperatively in three groups too. When the patients were evaluated according to the preoperative demographic data, no statistically significant difference was found between the patient groups and the results were similar to the literature. In intraoperative data, the values that could extend the operation time were found to be significantly higher in Group 3. The values in the postoperative data were significant in terms of drainage, revision, atelectasis, Pa / FiO2 <300 statistics in Group 3 in parallel with intraoperative data. One of the reasons that negatively affects lung function after open heart surgery is the use of blood and blood products. Therefore, it should always be kept in mind to try to minimize the use of blood and blood products to prevent potential complications in this patient group.Keywords: Aortocoronary bypass, transfusion, pulmonary functions
