Rize ili ve çevresindeki içme sularından izole edilen Escherichia coli suşlarında aktarılabilir direnç ve TEM-tipi β-laktamaz genlerinin araştırılması

Abstract
ÖZET Bu çalışmada Rize ili ve ilçesindeki içme sularından izole edilen Escherichia coli soslarının antibiyotik direnç profili, bu direncin aktarılabilirliği ve ampisilin dirençli olan E. coli suşlarında polimeraz zincir reaksiyonu ile TEM tipi P- laktamaz genlerinin araştırılması amaçlandı. Çalışmada 37 şehir şebeke ve 14 kaynak suyu olmak üzere Rize ili ve ilçelerindeki 51 istasyondan toplam 117 E. coli susu izole edilip il farklı antibiyotiğe karşı direnci tespit edildi. Bu suşların % 61.5'inde çoğul ilaç direnci gözlenmiştir. Direnç oranları sırasıyla; %48.7 sulbaktam- ampisilin, %47 ampisilin, % 30.7 seftazidim, %27.3 sefuroksim, %19,6 trimetoprim sulfametoksazol, %17,9 amikasin, %15.3 netiimisin, %13.6 gentamisin, % 12.8 tetrasiklin, % 10.2 sefazolin ve % 2.5 kioramfenikol olarak belirlenmiştir. Konjugasyon deneyleri sonucunda ampisilin, tetrasiklin ve trimetoprim- sulfametoksazol direnç genlerinin % 3.3, % 40, % 4.3 oranında ve 4.6xl0~i0 - 10-6 frekansları arasında aktarılabilir olduğu gözlemlendi. Aktarılabilir direnç genlerini taşıyan suşlar Rs 74, 134b, 171, 196, 268, 350, 411b, 450'dir. Toplam 117 E. coli susundan 55 (% 47) suş ampisiline dirençli bulunmuş ve bu suşiardan yedi tanesinde PCR ile W<xTem geni belirlenmiştir. Yapılan çalışmalar sonucunda çevrede yüksek düzeyde bir antibiyotik direncinin olduğu ve bu direncin bakteriler arasında yayılabildiği belirlenmiştir. Bu sonuç halk ve çevre sağlığı açısından büyük bir tehdit oluşturmaktadır. Anahtar Kelimeler: E. coli, antibiyotik direnci, aktarılabilir direnç, MaTEM, içme suyu IV SUMMARY Investigation of Transferable Resistance and TEM-Type of p -Lactamase Genes in Escherichia coii Strains Isolated from Rize and its Surroundings In this study, it was aimed that antibiotic resistance profiles, transferability and TEM-type of j3-lactamase genes in ampicillin-resistant (Amp1) Escherichia coii strains by polymerase chain resction (PCR) were investigated in E. coii strains isolated from Rize and its counties. In the study, a total of 117 E. coii strains from 51 station and its districts, which were 37 from city network system water and 14 from spring water samples, were isolated and then determined their resistance against 1 1 different antibiotics. Resistance rates were determined as 48.7% sulbactam-ampicillin, 47%ampicillin, 30.7% ceftazidime, 27.3% cefuroxime, 19.6% trimethoprime-sulfamethoxazole, 17.9% amikacin, 15.3% netilmicin, 13.6% gentamicin, 12.8% tetracycline, 10.2% cefazoline and 2.5% chloramphenicol. As a result of conjugation assays it was observed that ampiciilin, tetracycline and trimethoprim- sulfamethoxazole resistances were found to be transferable in rate of % 3.3, % 40 and % 4.3, respectively, and frequency at 4.6x10` J -10` '. Of 1 17 E. coii strains, 55 (47%) strains were found to be resistant to ampiciilin and, blajvM genes were determined in seven of them by PCR. As a results of the study, there was a high level of antibiotic resistance in the environment and, it was determined that this resistance was to be transferable amongst bacteria, constituting a great threat to public and environmental health.
