Fibromyalji sendromunda multidisipliner tedavi programının etkinliğinin değerlendirmesi

Abstract
46 ÖZET Çalışmamızda amacımız, Fibromyalji Sendromu (FMS) tedavisinde aerobik egzersiz ve eğitimin etkilerini kontrol grubuyla kıyaslamak ve bu yöntemle tedavi edilen hastaları tedavi programı sonrası tekrar değerlendirmekti. Bu amaçla ACR 1990 kriterlerine göre tanı konan 18-55 yaş arası olan 60 kadın hasta rastgele yöntemle 30 ar kişilik 2 gruba ayrıldılar. 1. gruba tedavi öncesi eğitim programı uygulandıktan sonra 8 hafta süreyle aerobik egzersiz ve peşinden germe egzersizleri uygulandı. 2. grup kontrol grubuydu. Her iki gruba Sertralin 50 mg/gün verildi. Tüm hastalar tedavi öncesi ve sonrası değerlendirildi. Ağrı; VAS, Hassas nokta sayısı, Total myaljik skor ile, depresyon; Beck Depresyon Ölçeği ile, genel değerlendirme; Hasta ve Doktorun genel değerlendirme ölçekleri ve Fibromyalji Davranış indeksi (FAI) ve Kendi kendine yeterlilik ölçeği (SELF) ile fonksiyonel değerlendirme; Fibromyalji Sendromu Etki Sorgulaması (FIQ) ve Günlük Yaşam Aktivitesi Sorgulaması (HAQ) ile, aerobik kapasite; Astrand Testi ile değerlendirildi. Her iki grup hastamız, VAS, hassas nokta ve total myaljik skor değerleri, BDI, FAI, FIQ, hasta ve doktor genel değerlendirme sonuçları tedavi öncesine kıyasla daha iyiydi ve aradaki fark anlamlıydı. V02 max Grup Fde anlamlı olarak düzeldi. FIQ'in bazı alt grupları, SELF fonksiyon alt skalası, FAI ve BDÖ' de gruplar arası tedavi sonrası değerlerde bir fark saptanamadı. Egzersiz ve eğitim grubu kontrol grubu hastalarından anlamlı derecede daha iyiydi. Sonuç olarak, Fibromyalji multifaktöriyel bir sendromdur, düzenli aerobik egzersiz ve stresle başa çıkma yöntemlerinin geliştirilmesi gibi multidisipliner tedavi yaklaşımları hastalık prognozunu olumlu etkiler. 47 ABSTRACT The purpose of this study was to compare the effectiveness of aerobic exercise and education with the control group in Fibromyalgia syndrome and to evaluate the patients treated with this method after the treatment program. Sixty women, aged 18-55 years, diagnosed as fibromyalgia according to 1990 ACR criteria were randomly divided into two groups. Group I was given aerobic exercise and stretching exercise for 8 weeks following an education program. Group II was the control group. Both groups took sertralin 50 mg/day. The patients were evaluated before and after the treatment. We used VAS, number of tender points, total myalgic score for pain; Beck depression Inventory (BDI) for depression; Patient and doctor global assessment scale, Fibromyalgia Attitudes Index (FAT), Self efficacy scale (SELF) for global assessment; Fibromyalgia Impact Questionnaire (FIQ), Health Assessment Questionnaire (HAQ) for functional evaluation; Astrand Test for aerobic capacity. The results of VAS, number of tender points, total myalgic score, BDI, FAI, FIQ, HAQ and patient and doctor global asssessment scale showed significant improvement in both groups. In the first group, the result of V02max showed significant improvement. The results of some subgroups of FIQ, SELF function subscale, FAI and BDI were not statistically different among the two groups. We found significant improvement in other parameters in exercise group than the control group. In conclusion, Fibromyalgia is a multifactorial syndrome, and the best treatment will encompass multiple strategies, that include education, stress management treatment and aerobic exercise.
