Kalça ve alt ekstremite cerrahisinde %1 ropivakain ve %0.5 bupivakainin epidural uygulamasının karşılaştırılması

Abstract
ÖZET Rejyonel anestezi, cerrahi girişimlerin çoğunda, sıklıkla ve başarıyla uygulanmaktadır. Çalışmanın amacı; kalça ve alt ekstremite cerrahisinde epidural aralığa uygulanan ropivakain ve bupivakainin oluşturduğu duyusal ve motor bloğun özelliklerini, hemodinamik parametreleri, hasta, cerrah ve anestezist memnuniyet skorlarını karşılaştırmaktır. Çalışmaya kalça ve alt ekstremite cerrahisi planlanan, yaşı 18-80, ağırlığı 50-100 kg arasında olan, boyu 150 cm'den uzun ve ASA I-II (American Society of Anesthesiologist) rastlantısal 50 hasta alındı. Birinci gruptaki 25 hastaya %1'lik 20 mi ropivakain, 2. gruptaki 25 hastaya %0.5'lik 20 ml bupivakain epidural aralığa uygulandı. Operasyon odasına alman hastaların kalp atım hızı (KAH), sistolik arter basıncı (SAB), diyastolik arter basıncı (DAB), ortalama arter basıncı (OAB), periferik oksijen satürasyonu (SpCh) monitorize edildi. Preoperatif ve epidural ilaç uygulamasından sonra 5, 10, 15, 20, 30, 60, 120, 180, 240, 360. dakikalarda KAH, SAB, DAB, OAB, SpÜ2, duyusal ve motor blok düzeyleri kaydedildi. Duyusal blok düzey saptanmasında `Pinprick` testi, motor blok düzey saptanmasında ise `Bromage` skalası kullanıldı. Epidural anesteziye bağlı intraoperatif komplikasyonlar ve tedavileri, cerrahi analjezi kalitesi (CAK), ek anestezik ilaç (midazolam ve fentanil) gereksinim süresi, operasyon sonrası ilk analjezik gereksinim süreleri ile hasta, cerrah ve anestezist memnuniyet skorları kayıt edildi. Her iki grup arasmda yaş, cins, eğitim düzeyi, operasyon tipi ve süresi açısından istatistiksel olarak anlamlı bir fark yoktu (p>0.05). Gruplar arasında ortalama KAH, SAB, DAB, OAB ve SpÛ2 değerleri açısından da istatistiksel olarak anlamlı bir fark bulunmadı (p>0.05). Duyusal blok düzeyleri yönünden her iki grup arasında istatistiksel olarak anlamlı bir fark bulunmadı (p>0.05) ve iki grupta da en yüksek duyusal blok düzeyi T3 olarak saptandı. Motor blok oluşumuna göre her iki grup arasında yapılan karşılaştırmada istatistiksel olarak anlamlı fark saptandı (p<0.001). Grup 2'de 9 hastada motor blok oluşmadı. Motor blok derecelerine göre yapılan karşılaştırmada 30, 60, 120, 180 ve 240. dakikalarda her iki grup arasında istatistiksel olarak anlamlı fark bulundu. IVGruplar arasında duyusal blok süresi (Grup l'de 342.4 dk, Grup 2'de 340.0 dk) ve 2 segment gerileme süresi (Grup l'de 168.2 dk, Grup 2'de 157.2 dk) karşılaştırıldığında istatistiksel olarak anlamlı bir fark bulunmadı (p>0.05). Motor blok süresi Grup l'de daha uzun bulundu (Grup l'de 222.6 dk, Grup 2'de 121.2 dk) ve her iki grup arasında istatistiksel olarak anlamlı fark saptandı (p<0.05). CAK Grup l'de istatistiksel olarak anlamlı derecede daha yüksek bulundu (p<0.05). Her iki grup arasında cerrah ve hasta memnuniyeti açısından istatistiksel olarak anlamlı bir fark yoktu (p>0.05). Ancak anestezist memnuniyeti Grup l'de 24 hastada mükemmel olarak bulundu ve her iki grup arasında istatistiksel olarak anlamlı fark saptandı (p<0.05). Grup l'de 9, Grup 2'de 20 hastada komplikasyon olmadı, her iki grup arasında istatistiksel olarak anlamlı fark bulundu (p<0.05). Her iki grupta da epidural enjeksiyon sonrası ve operasyon sonrası ilk analjezik yapım süreleri benzerdi ve aralarında istatistiksel olarak anlamlı bir fark yoktu (p>0.05). Ancak ek ilaç gereksinim gösteren hasta sayısı Grup 2'de daha fazla idi ve iki grup arasında istatistiksel olarak anlamlı fark bulundu (p<0.05). Sonuç olarak; kalça ve alt ekstremite operasyonlarında %1'lik ropivakain ve %0.5'lik bupivakain kullanılarak yapılan epidural anestezi ile yeterli cerrahi koşullar sağlanmaktadır. Ancak, %1'lik ropivakain konsantrasyonu %0.5 'lik bupivakaine göre yüksek derecede ve uzun süreli motor blok ve daha iyi cerrahi analjezi kalitesi sağladığı için bupivakaine iyi bir alternatif olabilir kanısındayız. Ropivacaine and bupivacaine are used hip and lower extremity operations at the epidural space. Aim of our study is to evaluate the sensory and motor characteristic and hemodynamic parameters of epidural ropivacaine and bupivacaine anesthesia and to compare the patient, surgeon and anesthetist satisfaction scores. ASA I-II (American Society of Anesthesiologists) 50 patients were included our study randomizely who were went to hip and lower extremity surgery. These patients were between 18-80 years olds, 50-100 kilograms weight and taller than 150 centimeters. Patients were divided into the two groups. In Group 1, 20 ml 1% ropivacaine was applied into the epidural space in 25 patients. In Group 2, 20 ml 0.05% Bupivacaine was applied into the epidural space in 25 patients. In the operation room, We monitorized heart beat rate (HBR), systolic arterial pressure (SAP), diastolic arterial pressure (DAP), mean arterial pressure (MAP) and peripheral oxygen saturation (SpC>2 ). We recorded all these parameters and sensory and motor block levels preoperatively and at the 5, 10, 15, 20, 30, 60, 120, 180, 240, 360 th minutes operatively and postoperatively. Sensory bloc level was detected with `Pinprick` test and motor bloc level was detected `Bromage` scale. We also recorded that intraoperative complications related to the anesthesia, surgical analgesia quality (SAQ), requirement of time for the additional anesthetic medicine (midazolam and fentanyl), requirement of time for first analgesia postoperatively, patient, anesthetist and surgeon satisfaction scores. There was no statistically difference as age, sex, education level, operation type and period between each group (p>0.05). SAP, DAP, MAP, HBF, Sp(>2 values were not different each other as a statistically (p>0.05). There was no statistically difference between sensory block levels (p>0.05). T3 was the highest sensory block level in the both group. In the comparison of motor block occurred, there was significant statistically difference (p<0.001). In bupivacaine group, motor block didn't occur in the 9 patients. In the comparison of motor block level there was statistically difference at the 30, 60, 120, 180 and 240 minutes (p<0.01, pO.Ol, p<0.001, p<0.01, p<0.01, respectively ). There was no statistical difference (p>0.05) VIwhen the sensory block period (ropivacaine group 342.4 minutes, bupivacaine group 340.0 minutes) and two segment regression period (ropivacaine group 168,2 minutes, bupivacaine group 157.2 minutes) were compared. In the ropivacaine group, motor block period were detected longer than bupivacaine group (ropivacaine group 222.6 minutes and bupivacaine group 121.2 minutes), so motor block period was significant statistically different between the groups (p<0.05). Surgical analgesia quality (SAQ) was detected higher degree in the ropivacaine group (p<0.05). In the both of the group, there was no statistically difference as a surgeon and patient satisfaction (p>0.05). In the ropivacaine group anesthetist satisfaction was perfect in the 24 patients and there was a statistically difference in the both group (p<0.05). In the ropivacaine group 9 patients hadn't complication and in the bupivacaine group, 20 patients hadn't complication. So statistically difference exist between two groups (p<0.05). After epidural injection and operation, first analgesia time was similar each other in the two groups (p>0.05). But, additional medicine requirement was higher in the bupivacaine group and there was a statistically difference in both group (p<0.05). In conclusion; optimal operation condition is provided by 1% ropivacaine and 0.5% bupivacaine in the hip and lower extremity surgery. 1% ropivacaine provide higher degree and longer period motor block so higher analgesia quality. We think that ropivacaine is good alternative for bupivacaine in the epidural anesthesia. Key words: Epidural anesthesia, ropivacaine, bupivacaine, sensory and motor block, analgesia quality VII
