Abstract
ÖZET Pseudomonas aeruginosa (P. aeruginosa), hastane infeksiyonlannda saptanan Gram-negatif bakterilerden en sık görülendir. Konakçı defansının zayıf olması, antibiyotiklere dirençli olması ve ekstraselüler bakteriyel enzim ve toksinler üretmesi nedeniyle, infeksiyonlan yüksek mortalite oranı ile ilişkilidir. /?-laktamlar arasında, karbapenemler geniş spektrumlu aktiviteleri ve genişlemiş spektrumlu /Maktamazlar da dahil, /Maktamazlann çoğuna dirençli olmaları ile, P. aeruginosa 'nın neden olduğu infeksiy onların tedavisinde etkili ajanlardır. Önceki çalışmalarda, karbapenem duyarlılık testlerinde problemler olduğu bildirilmiştir. Bu çalışmada, Ondokuz Mayıs Üniversitesi Tıp Fakültesi Mikrobiyoloji Laboratuvan'nda, çeşitli klinik örneklerden izole edilen, 142 P. aeruginosa kökeninin meropeneme duyarlılığı, disk difüzyon ve Phoenix Otomatize Mikrobiyoloji Sistemi (BD Diagnostic Systems, Sparks, MD) ile araştırılmıştır. Sonuçlar, Mikrobiyoloji laboratuvarlarmda en sık kullanılan metodlann doğruluğunu değerlendirmek için, NCCLS buyyon dilüsyon sonuçlan ile karşılaştırılmıştır. Esensiyal uyum ve kategorik uyum veya çok büyük hata, büyük hata ve küçük hata, ABD `Food and Drag Administration` (FDA)'in antibiyotik duyarlılık cihazlarının değerlendirilmesi kriterleri derlemesinde verilen tanımlamalar kullanılarak hesaplanmıştır. P. aeruginosa 'da, meropenem için çok büyük hata oranı, Phoenix' le %2.9'dur. Çok büyük hatalar, test tekarı ile düzelmiştir. Disk difüzyon ve Phoenix'le büyük hata saptanmamıştır. P. aeruginosa' âz meropenemle küçük hatalar disk difüzyonia Phoenix'den sıktır. Bununla birlikte meropenem için kategorik uyum Phoenix'le %97.2, disk difüzyonia %93.7'dir. Phoenix'le esansiyel uyum %95.1'dir. Genel olarak, Phoenix sistemi tüm test günlerinde tekrarlanabilirdir. Phoenix' de sonuç zamanı ortalama 15 saattir. Değerlendirilen iki metodun da, P. aeruginosa izolatlarmda meropenem duyarlılığının test edilmesi için kabul edilebilir, pratik metodlar olmasına karşın, Phoenix sisteminin en önemli avantaj ımn, antibiyotik duyarlılık testi sonuçlarını hızlı ve MÎK değeri olarak vermesi olduğu sonucuna varılmıştır. IX ABSTRACT Pseudomonas aeruginosa (P. aeruginosa) is the most common gram-negative bacterium found in nosocomial infections. The high mortality rate associated with these infections is d ue t o a c ombination o f w eakened h ost d efences, b acterial r esistance t o antibiotics, and the production of extracellular bacterial enzymes and toxins. Among the ^-lactams, carbapenems are potent agents for treating infections caused by P. aeruginosa, because of their broad spectra of activity and stability to hydrolysis by most /^-lactamases including the extended-spectrum p-lactamases. Previous studies have documented susceptibility testing problems in carbapenems. In this study, meropenem sensitivities of 142 P. aeruginosa strains, isolated from various clinical specimens in Microbiology Laboratories of Ondokuz Mayıs University School of Medicine were investigated by disk diffusion and Phoenix Automated Microbiology System (BD Diagnostic Systems, Sparks, MD). The results were compared to NCCLS broth microdilution results to assess the accuracy of the methods most commonly used in Microbiology Laboratories. Overall essential agreement and category agreement or the numbers of very major, major and minor errors were calculated using the definitions given on `Review criteria for assessment of antimicrobial susceptibility devices` of the US Food and Drug Administration. For P. aeruginosa the number of very major errors for meropenem was %2.9 by Phoenix. Very major errors resolved upon repeat testing. Major error was not noted with meropenem testing by disk diffusion and Phoenix. Meropenem minor errors were more frequent with P. aeruginosa by disk diffusion than with Phoenix. However category agreements for meropenem testing by Phoenix and disk diffusion were %97.2 and %93.7 respectively. Overall essential agreement by Phoenix was %95.1. In general, the Phoenix system performed consistently on all test days. Over all average time to final result was 15 hours by Phoenix. It was concluded that although these two assessed methods were observed to be acceptable for use as routine and practical methods for meropenem susceptibility testing of isolates of Pseudomonas aeruginosa, one of the most important advantages of the Phoenix system is that it provides MIC values and rapid results.
