Abstract
ÖZET GİRİŞ VE ARSAÇ Metastaz gelişimi tümörün yerleşmesi ve tümör hücre klanlarının yayılımı ile başlayan çok basamaklı bir süreçtir. Anjiogenik süreç, tümör hücrelerinin kapasitesine ve dolaşıma geçmesine dayanmaktadır. Bu yeni damar oluşumunun plazma vWF düzeylerinde artış ile ilişkili olduğuna dair güçlü kanıtlar mevcuttur. vWF, megakaryosit ve endotelyal hücrelerde seniezlenen plateletlerin hasar yerinde sudendotelyal yüzeye adhezyonuna öncülük ederek ve faktör VIII için taşıyıcı protein olarak görev alarak primer hemostazda önemli bir rol oynar. Bu çalışmanın amacı kolorektal karsinomlu hasta gurubunda plazma vWF ve faktör VIII düzeylerini belirlemek ve bu değerleri öncelikle kontrol gurubu olmak üzere prognostik faktörlerle kıyaslamaktır. GEREÇ VE YÖNTEM Bu çalışma histolojik olarak doğrulanmış, 67 kolorektal karsinomlu hasta ve 37 sağlıklı kontrolden oluşmaktadır. Hastalar daha önce sitotoksik tedavi almamıştı. Kontrol gurubunda; hemostatik parametreleri etkileyebilecek diyabetes mellitus, koroner arter hastalığı veya herhangi bir ilaç kullanım öyküsü mevcut değildi. Evrelemede, Astler Coller sınıflaması kullanıldı. Faktör VIII için Dade Behring koagulometrik, vVVF için immunoturbidimetrik kit kullanıldı. Faktör VIII için referans aralığı normaiin %7Q-150 aralığı, vWF için ise %1 7,5-37,5 idi. Veriler, SPSS for Windows versiyon 11.0 (Statistical Package Social Science, SPSS Inc., Chicago, Illinois, USA) kullanılarak analiz edildi. Kolorektal kanserli hastalar ile sağlıklı kontrolleri, ve prognostik faktörler ile hastaların faktör VIII ve vWF düzeylerini kıyaslamak için; normal dağılıma uymadığından dolayı logaritmik değişim yapıldı, yine normal dağılımla uyumlu olmadığından nonparametrik testlerden Mann-Whitney U testi kullanıldı. IVKolorektal kanserli hastalarda (%114,97±42,58,n=67) kontrollere (%52,45± 27,65) kıyasla plazma vWF düzeyleri belirgin yüksek idi. Plazma vWF düzeyleri ile cinsiyet, yaş, plazma CEA düzeyleri, tümör histopatolojisi, tümör lokalizasyonu, semptom süresi, obstruksiyon-perforasyon, tümör evre arasında bir ilişki saptanmadı. Faktör VIII için istatistiki anlamlılığa ulaşılmadı. ANAHTAR SÖZCÜKLER: Kolorektal, kanser, faktör VIII, von Willebrand faktör, prognostik faktörler. v ABSTRACT BACKGROUND AND AIM The development of metastases is a stepwise process that starts with the establishment and expansion of tumor cell clones with angiogenic properties. The angiogenic process depends upon the capacity of tumor cells and their passage to the circulation. There is strong evidence this neovascularization is associated with the increase in plasma levels of von Willebrand factor which is a glycoprotein synthesized mainly in endothelial cells and in megakaryosites and playing an essential role in primary hemostasis, mediating adhesion of platelets to subendothelial surfaces at sites of vasculer injury and acting as a carrier protein for coagulation factor VIII, in plasma. The aim of this study was to determine plasma levels of vWF and factor VIII in a series of patients with colorectal carcinoma and evaluate the correlation of these values primarily with controls and the correlation of these values with spesific prognostic factors MATERİALS AMD METHODS This study is consisted of 67 newly diagnosed patients with histologically confirmed colorectal carcinoma, 37 healthy controls. None of the patients included no prior cytotoxic therapy. The patients were categorized in to stages according to Astler Colier Classification. None of the controls suffered from diabetes meilitus, coronary artery disease and no history of orderly medication causing coagulation disorders. Plasma levels of of vWf were measured by immuno-turbidimetric assay of Dade Behring, reference interval was %17,5- 37,5. Plasma levels of factor VIII were measured by coagulometric assay of Dade Behring, reference interval was 70-150 % of the norm. Statistical analysis was carried out using the SPSS 11,0 for WINDOWS statistical software package. Nonparametric tests were employed since none of the parameters elicited normal distrubution despite logarithmic transformation. Mann-Whitney U test was applied for statistical comparison of the patients and controls, and patients and prognostic factors. VIRESULTS Patients with colorectal cancer (%114,97±42,58, n=67) have significantly higher plasma levels of vWf than controls (%52,45±27,65, n=37) (p= 0,0002 ). There was not statistical significant correlation between plasma levels of vWf and gender, age, plasma levels of CEA, tumor histopathology, tumor localization, duration of symptoms, bowel obstruction and perforation, tumor staging, Statistical significance was not reached for factor VIII.
