Gebelikte folik asit kullanımı ve nöral tüp defekti ilişkisi farkındalığının demografik özellikleri

Abstract
Çalışmanın Amacı:Bu çalışmanın amacı, Ondokuz Mayıs Üniversitesi Kadın Hastalıkları ve Doğum Kliniği ve Samsun Doğumevi'ne başvuran gebelerin, gebelik öncesi ve gebeliğin ilk trimesterinde folik asit kullanım oranlarını, öğrenim durumları ile ilişkisini, folik asitin önemi ile ilgili bilgilerini ve farkındalıklarını, gebelik boyunca demir, multivitamin ve kalsiyum preparatı kullanım oranlarını araştırmaktır.Yöntem:Ondokuz Mayıs Üniversitesi Kadın Hastalıkları ve Doğum Kliniği ve Samsun Doğumevi'ne antenatal takip için başvuran 1500 gebe gönüllü olarak çalışmaya dahil edildi. Gebelerin demografik özellikleri ile folik asit, demir, multivitamin ve kalsiyum kullanımı, folik asit ve Nöral Tüp Defekti (NTD) ilişkisi farkındalığı bilgilerini değerlendirmek için özel olarak oluşturulan bilgi formları kullanıldı.Bulgular:Çalışmaya katılan tüm gebeliklerin %73'ü planlı olduğu halde %9,4'ü folik asit kullanmaya gebelik öncesi dönemde başlamıştı. Gebelik öncesi dönemde folik asite başlama oranı en fazla öğrenim durumu üniversite olanlar (%21) ve sağlık çalışanları (%27) ile öğretmenler (%23) arasında idi. Çalışmaya katılan tüm gebelerin %62'sinde prekonsepsiyonel ve/veya gebeliğin ilk trimesterinde folik asit kullanımı saptandı. Bilgi formundaki folik asit ve NTD ile ilgili sorulara doğru cevap verme ve folik asit kullanım gerekçesinin farkındalık oranları eğitim durumu daha iyi olanlarda yüksek oranda bulunmuştur.Sonuç:Bölgemizde folik asit ve NTD ile ilgili bilgiler ve folik asitin NTD'ni önlemek için doğru zamanda özellikle de gebelik öncesi kullanımı oranları ülkemizin diğer bölgeleriyle benzer olmakla beraber gelişmiş ülkelere göre oldukça azdır. Eğitim düzeyi arttıkça kullanım ve bilgi düzeyi oranları da artmaktadır. Ülkemizdeki NTD sıklığı ve kadınların eğitim düzeylerinin düşüklüğü göz önüne alınırsa doğurganlık dönemindeki tüm kadınların folik asit ile ilgili farkındalıkları ve folik asitin gebelik öncesi kullanımı artırılmalıdır.Anahtar Kelimeler: Folik asit, Perikonsepsiyonel kullanım, NTD. Objective:The objectives of this study were to determine folic acid usage rate of women in periconseptionel period and first trimester of pregnancy, to asses the importance and awareness of folic acid, iron, calcium and other multivitamin supplementation and to examine association between folic acid use and education among the pregnant women attending to Ondokuz Mayıs Universitiy Obstetrics and Gynecology Department and Samsun Women?s Health and Obstetrics Hospital.Methods:An inquiry form that was included the demographic properties such as the women?s age, parity, education, occupational status. There were also specific questions about whether they used folic acid, iron, calcium and multivitamin supplements and whether they knew something about folic acid , NTD?s (Neural Tube Defects) and relationship between folic acid and NTD?s. These questions were administered to 1500 pregnant women who attended Ondokuz Mayıs Universitiy Obstetrics and Gynecology Department clinic and Samsun Women?s Health and Obstetric Hospital for antenatal care between August 2007 and August 2008.Results:Although 73% of all the pregnant women who we interviewed to this study, were planned pregnancies only %9,4 of these patients had started folic acid before conception. The percentage of folic acid usage before conception was higher among health workers (27%) , teachers (23%) and educated patients (21% of educated university ). 63% of respondenrs took folic acid in the preconceptional period and/or in the first trimester of pregnancy. The percentage of folic acid awareness and right answers to questions about folic acid and NTDs were higher in the patients with higher educational status.Conclusions:The percentage of knowledge about folic acid, NTDs and folic acid usage in the right time for prevention of NTDs seems to be insufficient as compared with developed country. When the educational status is increased, the usage and awareness of folic acid are rising. In our country, NTDs frequency is high and the women?s educational status is low, so we must increase the folic acid usage and awareness in women before conception.Key Words: Folic Acid, Periconseptional use, NTDs
