Abstract
AMAÇManyetik rezonans görüntüleme (MRG), vertebral kemik iliği patolojilerini göstermede duyarlılığı çok yüksek olan bir radyolojik görüntüleme yöntemidir. Ancak travma, primer malin ya da metastatik tümörler ve enfeksiyöz durumlar, rutin MR ve kontrastlı sekanslarda benzer görünümler oluşturabilmektedir. Bu durum özellikle vertebral kompresyon fraktürlerinin sık görüldüğü yaşlı populasyonda tanıda problemlere neden olmakta ve hastalar kesin tanıya göre tedavi almak için biyopsi gibi invaziv işlemlere maruz kalabilmektedir. Difüzyon ağırlıklı görüntüleme (DAG) ve biyolojik dokuların spesifik difüzyon kapasitesini belirten ADC (Apparent diffusion coefficient) ölçümlerinin bu tip hastalarda MRG' nin özgüllüğünü artırdığını gösteren çalışmalar mevcuttur.Bu çalışmanın amacı vertebral kemik iliği patolojilerinin DAG özelliklerine ve kantitatif değerlendirmeyi sağlayan ADC ölçümlerine göre ayırıcı tanılarını yapmak, sonuçta ADC için belirlenen optimal eşik değere göre benin ve malin olarak sınıflayabilmektir.GEREÇ VE YÖNTEMYaşları 18-89 arasında 48' i kadın, 49'u erkek toplam 97 olgudaki 147 vertebral patoloji çalışmaya dahil edimiştir. Lezyonlar benin, malin ve enfeksiyöz olarak 3 gruba ayrılmıştır. Hemanjiomlar ile benin kompresyon fraktürleri 1. grubu (benin), spondilodiskitler 2. grubu (enfeksiyonlar) ve metastatik tümörler, multipl myelomlar ve malin nedenlere bağlı kompresyonlar 3. grubu oluşturmuştur. Lezyonların son tanıları biyopsi sonucu, tipik klinik ve labaratuar bulgular ya da takip görüntüleme bulgularına göre konulmuş olup, benin ve malin olarak sınıflandırılmıştır. DAG sekanslar 1.5 T MRG cihazında, sagittal planda, single ? shot EPI' den b değerleri 0, 250, 500 mm² / sn olacak şekilde elde edilmiş olup, oluşturulan haritalar üzerinde elde edilen ADC değerleri ayrı iş istasyonunda ölçülmüştür.İstatistiksel analizlerde tüm ölçümsel değişkenler için normalite testleri, varyans analizi, Tukey HSD çoklu farklılık testleri uygulanmıştır. ROC eğri analizi kullanılarak optimal eşik ADC değeri bulunmuş, bu eşik değere göre duyarlılık, özgüllük, pozitif ve negatif öngörü değerleri hesaplanmıştır.BULGULARBenin lezyonlarda ortalama ADC değeri 1.77 x 10-³ mm²/sn olup en düşük 1.12x 10-³ mm²/sn, en yüksek 2.82 x 10-³ mm²/sn olarak bulunmuştur. Enfeksiyonlarda ortalama ADC değeri 1.54 x 10-³ mm²/sn olup ortalama ADC değerleri 1.34- 1.86 x 10-³ mm²/sn arasında değişmekteydi. Malin lezyonlarda ortalama ADC değeri 0.82 x 10-³ mm²/sn olup en düşük 0.11 x 10-³ mm²/sn, en yüksek 1.44 x 10-³ mm²/sn olarak bulundu. Bu üç grubun ortalama ADC değerlerine göre karşılaştırılmasında istatistiksel olarak anlamlı farklılık saptanmıştır (p<0,05).Vertebral kemik iliği lezyonlarını benin ve malin olarak ayırabilmek için belirlenen 1.32 x 10-³ mm²/sn optimal eşik değerine göre; duyarlılık %96.5, özgüllük % 95.2, pozitif öngörü değeri %96.5 ve negatif öngörü değeri %95.2 olarak bulunmuştur.SONUÇDAG ile elde edilen haritalar üzerinden hesaplanan ADC değerleri yüksek duyarlılık ve özgüllükle vertebral kemik iliği patolojilerini benin ve malin olarak ayırabilmiştir. Alt grupların ADC değerleri arasında istatistiksel olarak anlamlı farklılıklar saptanmıştır. Ölçüm yapılan ROI alanı ile ADC arasında istatistiksel olarak anlamlı ilişki saptanmamıştır. Objective:MRI is a radiological imaging technique with high precision used to indicate vertebral bone marrow pathologies. However, trauma, primary malinant or metastatic tumors and infectious events might create similar images on routine MRI and contrasting sequence. This situation leads to difficulty in diagnosis especially in the old population in whom vertebral compression fractures are frequently seen and the patients might undergo invasive practices such as biopsy to receive treatment in accordance with the definite diagnosis. Diffusion- weighted imaging is a method that investigates the structure of biological tissues extensively and it enhances the specificity of MRI in those kinds of patients. Quantitive investigation might be made via ADC (Apparent Diffusion Coefficient) which indicates specific diffusion capacity of the biological tissues. The objective of this study is to establish the differential diagnosis of the vertebral bone marrow pathologies in accordance with the DWI (Diffusion Weighted Imaging ) and ADC measurements which provide the quantitive evaluation and to be able to classify it as benin or malin in accordance with the optimal threshold value determined for ADC.Material and MethodOne hundred fourty seven vertebral pathologies in 97 patients (48 female, 49 male) whose ages ranged from 18 to 89 were included in the study. Lesions were divided into 3 groups as benin, malinant and infectious. The first group consisted of hemangiomas and benin compression fractures. The second group consisted of spondilodiscits and the third group consisted of metastatic tumors, multiple myeloma and malinancies. The final diagnosis of the lesions was established in accordance with the typical clinical and laboratory findings or imaging findings under the light of the biopsy and was classified as benin or malin.DWI sequences were obtained via 1.5 T MRI device in sagittal plane from single- shot EPI as 0, 250, 500 mm² / sec b values. ADC values which were obtained later were measured on the charts formed in a different workstation.Normality tests, variance analysis and Tukey HSD multiple comparison tests were done for all the measurements in statistical analysis. ADC threshold value was found to differentiate the benin group from the malin group via ROC curve analysis. Sensitivity, specificity, positive and negative prediction values were calculated.Findings:The mean ADC value was 1.77 x 10-3 mm2/ sec in benin lesions. The lowest value was 1.12 x 10-3 mm2/ sec and the highest value was 2.82 x 10-3 mm2/ sec. While the mean ADC value in infections was 1.54 x 10-3 mm2/ sec, the mean ADC values ranged from 1.34 to 1.86 x 10-3 mm2/ sec. The lowest value was 0. 11 x 10-3 mm2/ sec and the highest value 1.44 x 10-3 mm2/ sec. There were statistically significant differences in the comparison of these three groups in terms of ADC values (p< 0.05).In accordance with the 1.32 x 10-3 mm2/ sec cut off value determined so as to diagnose the vertebral bone- marrow lesions as benin or malin; sensitivity was 96.5 %, specificity was 95.2 %, positive prediction value was 96.5% and negative prediction value was 95.2 %.Result:ADC values were calculated via DWI, MRI and the charts managed to diagnose the vertebral bone- marrow pathologies as benin and malin with high precision and specificity. There were also statistically significant differences between the ADC values of the subgroups. Besides, there was no statistically significant relationship between ROI area and ADC.
